Primary malignant melanoma of the female urethra is a rare tumour with great agressivity and poor prognosis. An early diagnosis may benefit of radical surgery and adjuvant immunotherapy with curative effect. We present a sixty-five years old female with a malignant melanoma of urethra and a secondary additional melanoma in vulva, that was treated with both radical ureterectomy and vulvectomy. A bilateral inguinal lymphadenectomy and immunotherapy with alpha-interferon were added. The patient is free of disease one year postoperatively.

The authors report a retrospective series of 18 cases of rupture of the anterior urethra, with a mean age of 25 years (range: 10 to 45 years). The mechanism of the lesion was trauma by falling onto the perineum (12 cases), road accident (4 cases) and sexual intercourse (2 cases). Ureteral iatrogenic lesions occur with variable frequency in the different types of surgery, mainly in uncomplicated abdominal hysterectomy. This paper contributes 29 patients (2 male and 27 female) with several types of lesions, secondary to gynaecological, intestinal and vascular surgery. The series produced 16 cases of clamping, 6 ureterovaginal fistulae, 6 ureteral sections (5 total and 1 partial) and 1 ureterouterine fistula. The terminal ureter was the most frequently damaged. The diagnostic and therapeutical methods used in this type of lesions are analyzed.

A case of primary malignant melanoma of the female urethra is presented. A 65-year-old Japanese woman was referred with dysuria and urethral bleeding. A hemorrhagic blue-black tumor, 3 cm in diameter, was diagnosed as a malignant melanoma by urinary cytology and biopsy. In spite of radical surgery followed by adjuvant immunochemotherapy with beta interferon, dacarbazine, nimustine and vincristine (IFN beta-DAV), the patient died of the disease one year after surgery because of lung metastasis which developed six months after diagnosis. Supernumerary urethra, a rare anatomical anomaly, can be explained by various embryological theories, accounting for the multitude of classifications proposed. Its diverse, minor or very severe clinical manifestations suggest the diagnosis, which is confirmed by retrograde and voiding cystourethrography. Treatment is surgical or, more rarely, endoscopic.

Primary involvement of the urethra by lymphoma is rare. We report the first case of a primary urethral B-cell lymphoma as the initial manifestation of the acquired immune deficiency syndrome (AIDS). A fifty-seven-year-old black man was evaluated for obstructive voiding symptoms and bloody urethral discharge. Workup revealed a urethral mass which, on pathologic examination, was a large B-cell lymphoma.

A 36-year-old man was hospitalized for self-insertion of a metallic body into the urethra. The self-inserted foreign body (SFB) was removed by endoscopic means; psychiatric evaluation of the patient showed a disturbed schizoid personality. After a review of the current literature, including clinical features, diagnostic manoeuvres and methods of treatment of SFB, the authors suggest that a psychiatric evaluation is always recommended in order to discover mental underlying disorders, thus reducing the risk of recurrence.

Histological examination was performed on the anterior penile urethra from 21 asymptomatic trans-sexual patients taking oestrogens prior to surgery. Squamous metaplasia was present in 15 of these patients and in 1 man taking progesterones. Immunohistochemical staining for oestrogen receptors in the urethra was negative. Severe squamous metaplasia was associated with patchy chronic inflammatory cell infiltration but this was also present in normal controls. Seven patients with complete rupture of the membranous urethra secondary to trauma were treated by internal optical urethrotomy (OU). All underwent initial suprapubic catheter drainage. The diagnosis of complete urethral rupture was confirmed by ascending and descending urethrograms and subsequent urethroscopy. OU was performed 3 weeks to 8 years following injury. Urethral strictures recur in about 30% of the cases whatever the treatment. We describe a new urethral stent, originally developed in our Institution for vascular use after transluminal angioplasty. Since January 1986, we have controlled the biocompatibility and tolerance in the normal urethra of dogs with a one year follow-up. Numerous reports have been published concerning intraurethral foreign bodies. Two patients required endoscopic intervention: one for the removal of screws from the urethra and the other for extraction of a calculus.
