High flow infravesical obstruction in men: symptomatology, urodynamics and the results of surgery.

High flow infravesical obstruction in male patients with severe prostatism is defined by a maximum flow rate of more than 15 ml. per second and a vesical pressure exceeding 100 cm. water at maximum flow rate. During a 9-month period urodynamic screening of 225 patients referred to our hospital with prostatism revealed high flow infravesical obstruction in 16 (7 per cent). Transurethral surgery of the prostate or bladder neck was performed in 11 of the patients, with good results observed in symptomatology as well as in maximum flow rate. A urodynamic screening program is suggested to establish

An assessment of surgical treatment of vesical outlet obstruction in spinal cord injury: a review of 471 cases.

An assessment of 139 cases of outlet obstruction following spinal cord injury, in a consecutive series of 471 patients, is presented. The figures in relation to the level and extent of the spinal lesion and the type of operation (transurethral resection and external sphincterotomy), are reviewed. The incidence of repeat operation is recorded and assessed. It is concluded that transurethral resection and external sphincterotomy, either alone or combined, will relieve outlet obstruction in most cases of neuropathic bladder following spinal cord injury.

A second look at injection of vesical neck contractures with triamcinolone.

Adjuvant therapy for vesical neck contractures with triamcinolone injection has given good results with long-term followup but similar results have been obtained with transurethral resection alone in our current re-evaluation. Herein we review 220 consecutive vesical neck contractures treated by the aforementioned modalities.

Success rate of modified Pereyra bladder neck suspension determined by outcomes analysis.

The reported success rates of the modified Pereyra bladder neck suspension vary from 51 to 90%. Retrospective chart review studies have reported cure rates of 77 to 90%. In contrast, a questionnaire based outcomes analysis, using a strict definition of cure, reported 51% of the patients to be cured of stress urinary incontinence after modified Pereyra bladder neck suspension. We conducted a questionnaire based outcomes analysis of the modified Pereyra bladder neck suspension at our institution. The objectives of the study were to determine the success rates of this procedure using strict crite

Internal Urethrotomy in Treatment of Female with Anatomical Bladder Outlet Obstruction.

BACKGROUND: Bladder outlet obstruction (BOO) is a relatively infrequent urologic condition in women, but can cause bothersome symptoms. In this article, transurethral incisions of the bladder neck (TUIBN) and urethra in the treatment of anatomical BOO were assessed. MATERIALS AND METHODS: A total of 23 women who referred with chronic lower urinary tract symptoms, urinary retention, and difficulty in micturition were assessed. Diagnose was made on the basis of urethrocystoscopy, voiding cystourethrography, and urodynamic studies. All patients underwent transurethral incisions of bladder neck c

The Marlex sling operation for the treatment of recurrent stress urinary incontinence: a 16-year review.

Between 1968 and 1983, 281 patients with recurrent stress urinary incontinence and nine patients with primary stress urinary incontinence were treated with a two-team (suprapubic vaginal) Marlex urethral sling operation. Selection was based on the patient's history and the clinical demonstration of an anatomic defect in the closure mechanism of the urethra. The operative technique as described in 1970 remains unchanged. The bladder neck is released from the scar and replaced in a midretropubic position on a hammock of Marlex mesh attached to Cooper's ligaments. Patients with a sloughed urethra

Urethral calibre measurements on micturition cystourethrograms in adult males. Part two: Subvesical obstruction.

Urethral calibres were measured on six locations in 73 patients with obstruction of the bladder neck or prostatic urethra. The results indicate that the bladder outlet is compatible with normal micturition at calibres of 6 mm. or more and abnormally narrowed at 4 mm. or less. The very narrow overlapping area between the diameter values in normal and obstructed urethras makes MCU an accurate method of measurement the diagnosis of subvesical obstruction. More distal parts of the urethra although proven to be normal may appear on the roentgenograms as less distended than an abnormally narrowed bl

Therapeutic intervention for fetal lower urinary tract obstruction: Current evidence and future strategies.

In-utero vesica-amniotic shunting for fetal lower urinary obstruction (LUTO) is known to improve perinatal survival. More recently, studies including centres performing fetal cystoscopy, have suggested benefit on longer-term survival and renal outcome - within the limitations of small numbers and limited follow-up. These interventions carry significant risk, and therefore patient selection, and optimal timing, are key. The aim of this article is to explore ways of improving the accuracy of prenatal diagnosis, and of identifying risk factors for fetal and postnatal renal failure. The next step

Nerve-sparing Mid-urethral Obstruction (NeMO) in Female Small Rodents.

Partial bladder outlet obstruction (pBOO) has a high prevalence, causes significant patient burden, and immense health care costs. The most common animal model to investigate bladder remodeling in pBOO are female rodents undergoing partial obstruction at the proximal urethra. Variability in the degree of obstruction and animal mortality are major concerns with proximal obstruction. Furthermore, dissecting around the proximal urethra and bladder neck jeopardizes bladder innervation. We developed a nerve-sparing mid-urethral obstruction (NeMO) model for pBOO avoiding the disadvantages of the tra

[Further experience with the use of gastrointestinal segments in bladder reconstruction in the complex of exstrophy-epispadias].

It is a matter of discussion if bladder augmentation should or should not utilize in the treatment of exstrophy-epispadias complex. When staged functional bladder reconstruction is adopted in the most difficult cases, or when one desires to avoid early and/or subsequent urinary diversions, sometimes a bladder enhancement could be necessary. The indications should be: a progressive damage to the upper urinary tract, after bladder closure at birth, due to a very small, no compliant detrusor plate (this is a rare condition: only one case in our series); a bladder capacity < or = 80 mls at the tim
