Can We Prevent Recurrent UTIs Without Antibiotics, in Both Those Who Do and Do Not Use Catheters? ICI-RS 2024.

AIMS: Urinary tract infections are one of the most common reasons for antibiotic prescription. The widespread use of antibiotic treatments contributes to the global health problem of antimicrobial resistance development. To slow down the progression of antimicrobial resistance, it is essential that we explore nonantibiotic preventive treatments for this common condition. We aim to report discussions regarding nonantibiotic preventive strategies for recurrent urinary tract infections in both catheterized and non-catheterized patients that took place at the International Consultation on Incontin

The tapered and reimplanted small bowel as a variation of the Mitrofanoff procedure: preliminary results.

Ten patients with a urethra that could not be catheterized and with absent appendixes underwent neobladder construction using an ileal segment fashioned to serve as the anti-incontinence mechanism. The latter was tapered and reimplanted following the guidelines of the Mitrofanoff procedure. Urinary reservoirs were constructed from detubularized segments of right colon, sigmoid colon and composite gastro-ileal combinations. Followup ranged from 9 to 21 months (mean 14.5). All patients presently catheterize the reservoir satisfactorily and are free of urinary leakage. Three patients (30%) experi

Urinary infection before and after prostatectomy.

To determine the prevalence of pre and post prostatectomy related urinary tract infection and its correlation with peri-operative events, we studied 120 patients who underwent prostatectomy due to benign prostatic hypertrophy from September 2005 to September 2006. Urine cultures were performed before the operations, after a week, and three months later. Data including prostate volume, prostatic specific antigen (PSA), post voiding residue (PVR) and histopathological reports as well as the duration of urinary leak, bladder irrigation, hospitalization, and catheterization were studied. The mean

Postoperative urinary retention associated with total hip and total knee arthroplasties.

Urinary retention following total hip and knee arthroplasty is a common problem frequently necessitating either prolonged urethral catheter drainage or intermittent catheterization. The direct relationship of urinary tract instrumentations, procedures, and infections to deep sepsis in total hip replacements is well documented. Pharmacologic therapy to stimulate voiding or augment bladder emptying is thus theoretically preferable to the use of catheterization. Prasozin hydrochloride, an alpha blocker, relaxes the smooth musculature of the posterior urethra and prostrate and has been used to tre

[Acute ureteral obstruction (renal colic)].

Acute ureteral obstruction is always associated with high intrapelvic hydrostatic pressure. Objective diagnosis of renal colic can be made by direct measurement of intrapelvic pressure (IPP). We propose a very simple device for estimation of IPP consisting of ureteral catheter and polyethylene tube 150 cm long. The device gives intrapelvic pressure in centimeters of urinary column. The figures obtained corresponded to those in measurement of IPP in cm of water column. In normal 187 examinees IPP ranged from 0.5 to 14.2 cm urinary column. Mean normal IPP in females was higher than in males. In

Post-operative urinary tract infection and wound infection in women undergoing caesarean section: a comparison of two study periods in 1985 and 1987.

In 1985 and 1987 women undergoing Caesarean section were studied for the development of post-catheterization bacteriuria, urinary tract infection and wound infection. In 1985, 34% developed bacteriuria compared to 25% in 1987. Post-catheterization bacteriuria within two days was reduced by improved catheterization techniques. Late urinary tract infection after 5 days occurred in 2% of women in 1985 and 6% in 1987. The commonest bacteria were Escherichia coli and enterococci. Post-catheterization bacteriuria was only confirmed in a second urine specimen in 53%. The incidence of wound infection

Bladder volume determination using a dedicated, portable ultrasound scanner.

Urethral catheterization, the standard method for measurement of bladder volume, is associated with patient discomfort plus the risks of urethral trauma and urinary tract infection. A portable ultrasound instrument that automatically determines bladder volume was used for 164 determinations and the digital readout correlated with the catheterized volume (r2 equals 0.79). This instrument is a noninvasive alternative to urethral catheterization for the determination of bladder volume in most patients.

[Learning of self-catheterization: For whom? Why? How?].

Since the 1970s, self-catheterization is the preferred method of urine drainage in case of urinary retention of neurological etiology (paraplegia, multiple sclerosis…) and non-neurological cause (benign prostatic hypertrophy, acontractile bladder of the elderly subject…). The main objective is to allow the physiological alternation of filling and complete emptying phases, in order to preserve the renal function and prevent urinary infections. The learning of self-catheterization is simple but requires a therapeutic education session with trained personnel, at home or in a specialized center. C

A novel insertion technique for urinary catheters in female dogs with the use of a guidewire.

BACKGROUND: Complicated transurethral urinary bladder catheterization in female dogs is a commonly encountered urologic problem, often causing inadvertent trauma to the lower urinary tract and breach in aseptic technique. KEY CONCEPTS: A novel method for transurethral insertion of urinary catheters in female dogs is described. The main variation from current procedures involves the application of the Seldinger technique with the use of an introduction catheter that is specifically designed to feed into the urethra, and the use of a guidewire to replace the introducer with an indwelling balloo

The effective management of catheter-related UTIs.

The majority of patients currently residing in Britain's hospitals are older people, many of whom have continence problems that may result in catheterization. Older people are also particularly vulnerable to infection and the consequences of sub-optimal care. Urinary tract infection is the most common health-care-acquired infection and most urinary tract infections are a result of urinary catheterization. This extended case study illustrates how ritualistic catheter care can increase the risks of an older person developing an infection as well as a host of other problems as a result of that in…

Use of cytomorphometry and cytology in the diagnosis of transitional-cell carcinoma of the upper urinary tract.

Because of the rise in incidence of upper urinary tract tumors, there is a need for a simple and reliable method for diagnosing these tumors, especially in people in a "high-risk" group. This retrospective study showed the usefulness of cytology and cytomorphometry in making the diagnosis of transitional-cell carcinoma of the upper urinary tract. The study also emphasized that the methods of collection and processing are of the utmost importance: the cytologic evaluation of ureteral catheterized urine specimens gave 100% accuracy as compared with a 40% false-negative rate in the cytologic diag

Managing bladder dysfunction using intermittent self-catheterization.

When an individual is unable to empty the bladder fully they are at risk of developing long-term bladder and renal damage. Urinary catheterization may be required if other treatments fail to resolve the problem. Urinary catheters can be indwelling or intermittent. Intermittent catheterization (IC) can be performed by healthcare professionals or carers. Intermittent self-catheterization (ISC) is performed by the person with bladder dysfunction and is considered to be the 'gold standard' in managing bladder dysfunction. ISC can spare the individual the unpleasantness of an indwelling catheter an…

Securing the indwelling catheter.

Each year, millions of Americans are catheterized to ensure adequate bladder drainage. But despite the high rate of catheterization in acute care facilities, clinicians often pay little attention to the decision to insert an indwelling catheter, its optimal management, or especially its timely removal. A physician or NP typically orders the insertion of a urinary catheter, but a nurse often performs the catheterization and is responsible for its management. Reimbursement policy changes recently mandated by the Centers for Medicare and Medicaid Services-including one stipulating that Medicare w

The prevention and management of urinary tract infections among people with spinal cord injuries. National Institute on Disability and Rehabilitation Research Consensus Statement. January 27-29, 1992.

The Urinary Tract Infection Consensus Conference brought together researchers, clinicians, and consumers to arrive at consensus on the best practices for preventing and treating urinary tract infections in people with spinal cord injuries; the risk factors and diagnostic studies that should be done; indications for antibiotic use; appropriate follow-up management; and needed future research. Urinary tract infection (UTI) was defined as bacteriuria (102 bacteria/ml of urine) with tissue invasion and resultant tissue response with signs and/or symptoms. Asymptomatic bacteriuria represents coloni

Can we really make catheter-associated urinary tract infections a never event? A level 1 trauma center's experience with prophylactic antibiotic bladder irrigation.

BACKGROUND: Hospital-acquired catheter-associated urinary tract infections (CAUTIs) are considered "never events" and are reportable to Centers for Medicare and Medicaid Services as a quality indicator. Despite protocols to determine appropriate removal of urinary catheters as soon as possible, severely injured trauma patients often require prolonged catheterization during ongoing resuscitation or develop retention requiring catheter replacement, exposing them to risk for CAUTI. We evaluated whether prophylactic antibiotic bladder irrigation reduces the incidence of CAUTI in critically ill tra
