Management of Postradical Prostatectomy Urinary Incontinence: A Review.

Postprostatectomy urinary incontinence has a significant impact on the quality of life of patients who undergo radical prostatectomy. Stress and overflow incontinence may result from the procedure, with sphincteric incompetence and detrusor hypocontractility implicating their development, respectively. In many cases, treatment begins with conservative approaches, including pelvic floor muscle training or biofeedback. Pharmacotherapy can be used to treat overactive bladder. For stress incontinence, transurethral bulking agents are utilized in select patients; however, artificial urinary sphinct…

[Recurrent urinary incontinence in women: causes and treatments].

Quite often patients assume that any new onset of urge incontinence or nocturia is recurrent urinary incontinence. Is it possible to consider any similar situation as the recurrence of urinary incontinence? No, of course not. However, if you put at the forefront a patient satisfaction with the overall result of the surgery, rather than a formal negative cough test, the picture will be somewhat different. On the one hand, it brings us back to criteria of surgery success, and on the other to the indications for repeat surgery. All this highlights the importance of thorough examination of patient…

The management of stress urinary incontinence: A case report.

INTRODUCTION: Conservative management is the first option for patients with stress urinary incontinence (SUI). However, successful management of women diagnosed with SUI is dependent on a proper assessment and a tailored treatment plan. This case report aims to show the effectiveness of physiotherapy management in a 42-year-old patient diagnosed with SUI. PATIENT PRESENTATION: The patient's main complaints were involuntary loss of urine on coughing, sneezing and lifting of heavy objects, which started following the birth of her third child. MANAGEMENT AND OUTCOME: The patient was taught the …

Development of quality indicators for women with urinary incontinence.

AIMS: To develop a means to measure the quality of care provided to women treated for urinary incontinence (UI) through the development of quality-of-care indicators (QIs). METHODS: We performed an extensive literature review to develop a set of potential quality indicators for the management of UI. QIs were modeled after those previously described in the Assessing the Care of Vulnerable Elders (ACOVE) project. Nine experts ranked the indicators on a nine-point scale for both validity and feasibility. We analyzed preliminary rankings of each indicator using the RAND Appropriateness Method. A …

Medical therapy for acquired urinary incontinence in dogs.

The objective of this case report was to review acquired canine urethral incontinence in a case of a 2-year-old beagle that presented for inappropriate urination. Commercially available medications, as well as compounded medical therapeutic options, were reviewed. Diagnostic results revealed cystitis, which responded to the prescribed antibiotics. The incontinence issue remained. Compounded diethylstilbestrol was prescribed and dispensed to achieve successful clinical results. The role of the compounding pharmacist was vital to achieve appropriate dosing of a drug no longer commercially availa

A case of urinary incontinence by hydroxychloroquine in a geriatric patient.

WHAT IS KNOWN AND OBJECTIVE: Rheumatoid arthritis is an autoimmune disorder characterized by persistent synovitis and systemic inflammation. Genetic factors account for approximately 50% of cases of rheumatoid arthritis and environmental factors include smoking. Urinary incontinence may occur as a medication adverse effect. We present the first report of a case of hydroxychloroquine-induced urinary incontinence in rheumatoid arthritis. DETAILS OF THE CASE: A 71-year-old female with a history of rheumatoid arthritis developed urinary incontinence as an adverse drug reaction to hydroxychloroqui…

Urinary incontinence in persons with Prader-Willi Syndrome.

OBJECTIVE: To assess and identify the frequency and type of urinary incontinence (UI), as well as associated symptoms in persons with Prader-Willi syndrome (PWS). PWS is characterized by mental retardation, short stature, obesity and hypogonadism. The behavioural phenotype includes eating problems, temper outbursts, affective disorders, stereotypies and speech abnormalities. UI is common in children with mental retardation in general, but has not been reported systematically in children with PWS so far. MATERIALS AND METHODS: The Dutch version of the 'Parental Questionnaire: Enuresis/Urinary …

[Urinary incontinence in the elderly: distinctive feature and management].

Urinary incontinence is very common among the elderly and results in a series of personal and economic complications. This population is often very heterogeneous with the aetiologies or dependences which are not very obvious. It is thus important to carry out among these patients a total assessment which will make it possible to detect the affections or the dependence from which it suffers. The BGMST (Belgian Geriatric Minimum Screening Test) includes a series of non aggressive tests which will answer this need. The performance of urodynamic tests is not easily achieved in case of not very aut…

Non-surgical management of urinary incontinence in children.

Urinary incontinence and neurogenic bladder are common in children, and can be difficult to treat. This themed issue includes contributions by experts in the management of these disorders. Dr. John Kryger discusses the nonsurgical management of neurogenic bladder in children with spina bifida. Drs. Lori Dyer and Israel Franco summarize the literature and their experience with the use of botulinum toxin in neurogenic and non-neurogenic incontinence in children. Dr. Paul Austin summarizes the use of alpha-adrenergic blockers. These drugs are primarily used to treat bladder outlet obstruction due…

Maintaining standards for surgery for female urinary incontinence.

MAINTAINING STANDARDS IN SURGERY FOR FEMALE URINARY INCONTINENCE: Operations for female stress urinary incontinence (SUI) are among the most common operations performed. In particular, recent advances in surgical technique and materials available for this type of surgery have made these operations more accessible, but even if an operation is characterized as "minimally invasive" surgeons must aim to maintain the highest possible standards in the interest of their patients. STANDARDS IN PATIENT SELECTION: Great care has to be taken to do a thorough workup of a patient about to undergo surgery. …

Open retropubic colposuspension for urinary incontinence in women.

BACKGROUND: Urinary incontinence is a common and potentially debilitating problem. Open retropubic colposuspension is a surgical treatment which involves lifting the tissues near the bladder neck and proximal urethra in the area behind the anterior pubic bones to correct deficient urethral closure. OBJECTIVES: To assess the effects of open retropubic colposuspension for the treatment of urinary incontinence. SEARCH STRATEGY: We searched the Cochrane Incontinence Group Specialised Register (searched 30 June 2008) and reference lists of relevant articles. We contacted investigators to locate e…

[Urinary incontinence and descensus genitalis--the silent sickness].

In the last few years, urogynaecology has become a very innovative gynaecological subspecialty. New findings have led to a multitude of different medicinal and operative therapy options. Stress incontinence can be improved by sling procedures in more than 80% of patients. Likewise, new drugs to treat stress incontinence have been available for some time. Injections of botulinum toxin A into the detrusor muscle are a promising treatment strategy in cases of therapy-resistant overactive bladder.
