We evaluated the role of vagal reflexes in a mouse model of allergen-induced airway hyperreactivity. Mice were actively sensitized to ovalbumin then exposed to the allergen via inhalation. The small intestine vagotomy influence on gastric secretion function, stimulated by pentagastrin and carbachol, was investigated while chronic experiment on dogs with gastric fundal and duodenal fistulas. The comparative analysis of influence of extragastric selective proximal, selective distal, selective and truncal vagotomy on gastric secretion in various terms after operative intervention was also conducted. In 6 months after conduction of various kinds of vagotomy the reduction of hydrochloric acid secretion, stimulated by pentagastrin, was noted.

The authors have performed a computerized processing of the electron data base of 1706 patients with duodenal ulcers. We have performed a suprapyloric antrectomy with anterior pylorotomy and truncal vagotomy in 106 patients with chronic duodenal ulcer between 1975 and 1990. Follow-up was carried out in 94 patients, during a mean time of 6 years. We have had no postoperative mortality and no long-term recurrence. The percentage of Visick I patients is similar to that after truncal vagotomy and antrectomy. We, therefore, believe that this procedure is safe and can be performed when an antrectomy is mandatory to avoid the operative morbidity and mortality of classic antrectomy.

We studied the effect of erythromycin on gastric emptying in nine patients with gastroparesis following truncal vagotomy and antrectomy, and assessed their clinical response to chronic oral erythromycin. The authors conducted experiments on 30 dogs for periods of 1-7 days to 12 months and established that a sharp disorder of microcirculation occurs in the postoperative period, particularly in the cardia and fundus of the stomach, due to denervation and devascularization which lead to stasis in the venous system, hypoxia, and ischemia. The microcirculatory disorders are restored in 6 to 12 months. The study showed that the use of selective proximal vagotomy in true gastric ulcers is not expedient.

Between 1971 and 1987, highly selective vagotomy (HSV) was carried out in 984 patients suffering from duodenal ulcer disease.
