Comparison of arteriovenous fistulas constructed with main or internal branch of the cephalic vein: a retrospective analysis of 32 cases.

OBJECTIVE: To evaluate the effect on the maturation of arteriovenous fistulas (AVFs) when using the internal branch of the cephalic vein compared with the main branch of the cephalic vein. METHODS: The study enrolled patients with end-stage renal disease and divided them into an internal branch group (AVF constructed using the internal branch of the cephalic vein) or a main branch group (AVF constructed using the main branch of the cephalic vein). The surgical outcomes including complications were observed in these patients after 12 weeks. RESULTS: Thirty-two patients with end-stage renal di

Percutaneous arteriovenous fistula creation.

The first percutaneous systems used to create an arteriovenous fistula for hemodialysis were approved by the US Food and Drug Administration in 2018 and included the Ellipsys and WavelinQ devices. Early results and longer-term studies of these catheter-based devices suggest that they offer important benefits and potentially improved outcomes in patients with appropriate anatomy. The deep communicating vein in the cubital fossa and its relationship to the proximal radial artery or proximal ulnar artery are key vascular elements of both systems. The devices differ significantly in technical desi

Extraparenchymal renal artery aneurysms: is hypertension an indication for revascularization surgery?

We report here the surgical management of extraparenchymal renal artery aneurysms associated with hypertension and the results of this treatment. From January 1978 through December 1999, 19 consecutive patients with 23 extraparenchymal renal artery aneurysms underwent surgery with renal revascularization techniques. Of these 19 patients, 89.5% had systemic hypertension, and 12 of 16 patients had associated renovascular hypertension. Twenty of the aneurysms were patent, one was chronically thrombosed, and one patient presented with acute thrombosis of abdominal aortic and bilateral renal aneury

Endovascular stents: preliminary clinical results and future developments.

At present, there is an exponential use of new interventional techniques whose proper role and value have not yet been defined. The intracoronary stent is just one example. There is no doubt that stents can be implanted with a high technical success rate associated with highly predictable immediate angiographic results and that they appear to be superior to all other interventional techniques. However, the intrinsic thrombogenicity of all devices currently available for clinical use warrants a vigorous anticoagulation, exposing the patient either to the risk of (sub)acute stent thrombosis or t

Acute limb deterioration during intra-arterial thrombolysis.

Approximately 12 per cent of limbs undergoing intra-arterial thrombolysis (IAT) develop distal embolism or extension of thrombus during the procedure. These are usually of little clinical consequence and can be treated by increasing the rate of administration of the lytic agent. However, in some patients the clinical condition of the limb deteriorates rapidly. In an attempt to define the incidence of acute limb deterioration during IAT, information was collected from five centres in the UK with experience of the technique. A total of 866 treatments were recorded, with 20 limbs (2.3 per cent) u

Preoperative saphenous vein mapping for coronary artery bypass.

Saphenous vein mapping by real-time duplex ultrasound scanning allows for the confirmation of the presence of patent vein and determination of the anatomical course of the vein prior to aortocoronary bypass procedures. This technique has proven to be of value in patients whose veins are not evident by physical examination and in those whose length of conduit may be limited by previous vein stripping or thrombophlebitis.

Techniques and outcomes for endovascular treatment in the tibial arteries.

Interventional therapy for tibial arteries is a key part of the vascular specialist armamentarium. Tibial artery interventional therapy has been proven to lead to limb salvage with low morbidity and mortality in patients with critical limb ischemia and should be used as a first line treatment mode in the majority of patients, especially in those with significant medical comorbidities. The exceptions are the patients with extensive tissue loss and infection, where endovascular therapy may not restore enough flow to achieve rapid healing and limb salvage, and those patients with tissue loss in t

Nitric oxide: what a vascular surgeon needs to know.

Atherosclerosis in the form of peripheral arterial disease results in significant morbidity and mortality. Surgical treatment options for peripheral arterial disease include angioplasty with and without stenting, endarterectomy, and bypass grafting. Unfortunately, all of these procedures injure the vascular endothelium, which impairs its ability to produce nitric oxide (NO) and ultimately leads to neointimal hyperplasia and restenosis. To improve on current patency rates after vascular procedures, investigators are engaged in research to improve the bioavailability of NO at the site of vascula

Percutaneous treatment of severe carotid stenosis due to Takayasu's arteritis early after carotid endarterectomy.

Takayasu's arteritis is a chronic inflammatory disease primarily involving large vessels. The diffuse, multifocal and progressive nature of the vascular lesions make surgical repair difficult. We describe successful carotid stenting of a 39-year-old female who presented with symptomatic, aggressive, long-segment carotid restenosis 6 months following left carotid endarterectomy. Repeat angiography 6 months later revealed continued patency.

Developing an anticoagulant microfibrous vascular graft with enhanced kink resistance and self-sealing capabilities.

For clinical treatment of end-stage renal disease (ESRD) patients, the development of vascular grafts possessing both puncture resistance and anticoagulant properties remains crucial for arteriovenous fistula establishment. In this study, small-diameter vascular conduits were engineered through electrospinning of polyurethane (PU) microfibers, incorporating polyethylene coil reinforcement within the graft wall architecture to confer kink resistance. The microporous structure of the grafts demonstrated effective self-sealing capabilities following needle perforation. Additionally, heparin immob

Biomaterials for vascular tissue engineering.

Cardiovascular disease is the leading cause of mortality in the USA. The limited availability of healthy autologous vessels for bypass grafting procedures has led to the fabrication of prosthetic vascular conduits. While synthetic polymers have been extensively studied as substitutes in vascular engineering, they fall short of meeting the biological challenges at the blood-material interface. Various tissue engineering strategies have emerged to address these flaws and increase long-term patency of vascular grafts. Vascular cell seeding of scaffolds and the design of bioactive polymers for in

Lower Extremity Bypass Using Bovine Carotid Artery Graft (Artegraft): An Analysis of 124 Cases with Long-Term Results.

INTRODUCTION: Although biological grafts have been utilized as a vascular conduit in leg bypass for many years, reports of a bovine carotid artery graft (BCAG) in lower extremity revascularization have been scarce. This study analyzed the outcome of lower leg bypass using BCAG. METHODS: A retrospective review of a prospectively collected database of all patients undergoing lower extremity bypass using BCAG from 2002 to 2017 was performed. Clinical outcomes including graft patency and limb salvage were evaluated. RESULTS: A total of 124 BCAG (Artegraft, North Brunswick, NJ) were implanted in

Experimental rupture of atherosclerotic lesions increases distal vascular resistance: a limiting factor to the success of infarct angioplasty.

Rupture of atherosclerotic lesions, resulting in localized thrombi and marked falls in distal blood flow, is a pivotal event in unstable coronary syndromes. We tested the hypothesis that after lesion rupture, vasoconstrictor mechanisms are major contributors to a marked rise in distal microvascular resistance, which is responsible for much of the interruption in blood flow. Cholesterol-fed rabbits underwent endothelial denudation of their left iliac arteries to induce angiographically severe, fatty, American Heart Association type IV-like atherosclerotic lesions. After lesion disruption with a

Experience with Arteriovenous Fistula in Elderly Patients with Chronic Kidney Disease.

OBJECTIVES: In recent years, advances in medical care have expanded the boundaries of aging, leading to an increasing number of elderly individuals requiring hemodialysis (HD). The use of arteriovenous (AV) fistulas for HD in elderly patients remains controversial due to concerns about life expectancy, vascular aging, and comorbidities. This study aimed to compare AVF outcomes in patients aged ≥ 70 years with those aged < 70 years to evaluate the safety and efficacy of AVFs in elderly patients. MATERIALS AND METHODS: This retrospective, single-center study included 202 patients who underwent

Early cannulation of bovine carotid artery grafts (Artegraft) after primary vascular access and fistula revision procedures.

OBJECTIVE: Tunneled dialysis catheter (TDC) use has been associated with increased infectious complications and mortality in hemodialysis-dependent patients. Unfortunately, patients who undergo fistula revisions or creation of a new arteriovenous fistula frequently require a TDC during the postoperative period. Bovine carotid artery grafts (BCAGs) can be used as an early-access dialysis conduit to reduce TDC dependence. This study describes the performance of BCAGs that were cannulated early (<3 days) after implantation and associated clinical outcomes. METHODS: BCAGs were implanted in 63 con

Femoral-popliteal bypass with endoscopically harvested saphenous vein in patients with TASC D disease of the superficial femoral artery.

BACKGROUND: This study evaluated patients undergoing femoropopliteal bypasses using endoscopically harvested vein to treat Trans-Atlantic Inter-Society Consensus (TASC) stage D lesions. Primary patency and primary assisted patency were evaluated, as were perioperative morbidity and mortality and hospital length of stay (LOS). Results for this minimally invasive alternative to femoropopliteal bypass with conventional open vein harvesting were analyzed. METHODS: A retrospective analysis was performed on patients who underwent femoral-popliteal bypass with endoscopic saphenous vein harvest and a

Clinical autologous in vitro endothelialization of infrainguinal ePTFE grafts in 100 patients: a 9-year experience.

BACKGROUND: Clinical in vitro endothelialization was assessed for its ability to improve the long-term patency of prosthetic femoropopliteal bypass grafts. METHODS: Between June 1989 and May 1998, 100 patients received 113 in vitro endothelialized expanded polytetrafluoroethylene grafts (ePTFE). Bilateral implantations were performed in 13 patients. In phase 1 of the study, 24 patients received 27 endothelialized grafts and 16 patients received 17 untreated grafts. In phase 2, endothelialization was offered to all patients who did not have a suitable saphenous vein available. Phase 2 began in

Follow-up and surveillance of vein grafts: when and how to intervene to prevent complications.

Despite advances in endovascular strategies, surgical infrainguinal lower limb revascularization remains the gold standard treatment for critical lower limb ischemia and disabling claudication. Maintaining patency of infrainguinal vein bypass grafts has been a challenging task for vascular surgeons as they are prone to developing stenoses which may precipitate failure of the bypass. Duplex ultrasound scanning has evolved as the investigation of choice to identify vein graft lesions but graft surveillance programs using this technique remain controversial. The rationale for graft surveillance i
