Dynamics of left ventricular apex rotation during angioplasty: a sensitive index of ischemic dysfunction.

BACKGROUND: Apex rotation has been shown to provide a reliable index of the dynamics of left ventricular (LV) twist. In this study, we aimed to characterize twist at baseline and during acute ischemia in 20 patients undergoing percutaneous transluminal coronary angioplasty to the left anterior descending (LAD) artery and to test whether an old myocardial infarction or collateral flow affected twist dynamics. METHODS AND RESULTS: Among patients with no previous infarction, five had no collaterals (group A) and six had angiographically visible collaterals (group B). Previous anterior infarction

[Transluminal coronary angioplasty in patients with left ventricular dysfunction; immediate and long term results].

Transluminal coronary angioplasty is increasingly performed in patients with left ventricular dysfunction. However, there is little data available concerning the outcome of these patients. The aim of this retrospective study was to assess the immediate and long-term results in 90 patients (76 men and 14 women) with left ventricular ejection fractions < or = 35% (average 29 +/- 5%) undergoing angioplasty between 1980 and December 1992. Eighty-seven patients (96%) had a history of infarction, 27 (30%) had already at least one episode of left ventricular failure and 34 (38%) had unstable angina a

[Can patients with severe left ventricular dysfunction be treated by coronary artery bypass surgery?].

Forty three men and 3 women, with an average age of 59 years (13 to 78 years) underwent aorto-coronary bypass surgery despite severe left ventricular dysfunction (ejection fraction < 35%); 96% of the patients had previous infarction; 60% (N = 28) had unstable angina, 52% (N = 24) had had pulmonary oedema or an episode of congestive cardiac failure. The average ejection fraction was 29 +/- 4%, range 17 to 35%. Thirteen patients had ventricular aneurysms, 4 had grade 3 or 4 mitral regurgitation. The coronary lesions were usually multivessel left main coronary (6), triple vessel disease (27), dou

[Long-term prognosis in patients with echocardiographic left ventricular dysfunction masquerading as dilated cardiomyopathy].

The long-term prognosis and prognostic factors were assessed in patients with left ventricular dilatation with impaired function secondary to idiopathic dilated cardiomyopathy (DCM) or to heart diseases of various pathogeneses masquerading as DCM (DCM-like). The echocardiographic criteria for DCM-like disease were 1) left ventricular end-diastolic dimension exceeding 60 mm and 2) fractional shortening less than 15%. Those who showed improvement in either of these 2 parameters within 3 months were excluded from this study. One hundred and fifty-eight of 35,250 serially examined patients fulfill

Prevalence and prognostic value of left ventricular dysfunction in non-Q-wave myocardial infarction.

Forty-two consecutive patients with non-Q-wave acute myocardial infarction (AMI) and 40 consecutive patients with transmural AMI were comparatively examined by equilibrium gated radionuclide angiography (ERNA) to assess prevalence and prognostic value of left ventricular dysfunction in non-Q-wave AMI. Left ventricular ejection (LVEF) was generally preserved both in anterior and inferior non-Q-wave infarctions and sharply reduced only in anterior transmural infarctions. ERNA cannot assist in identifying any subgroup of patients with non-Q-wave AMI at risk of major complications other than left

Comparison of the electrocardiographic features of complete left bundle branch block in patients with ischemic and nonischemic left ventricular dysfunction.

BACKGROUND: Differentiating ischemic (ILVD) from nonischemic left ventricular dysfunction (NILVD) is important prognostically and therapeutically but might be difficult clinically. The differentiating role of electrocardiographic (ECG) features in the presence of left bundle-branch block (LBBB) is debatable on differentiating ILVD from NILVD. OBJECTIVE: The present study assessed whether there is the role of certain ECG features in differentiating ILVD from NILVD in the presence of the complete LBBB. METHODS AND RESULTS: Patients who had LBBB were divided into two groups based on the presenc

Left atrial appendage thrombosis in a patient with Friedreich Ataxia-related cardiomyopathy, left ventricular systolic dysfunction, and atrial fibrillation.

Friedreich ataxia is the most common form of hereditary ataxia. Heart involvement in Friedreich ataxia is common and can include increased left ventricular wall thickness, atrial fibrillation, and in the later stages, a reduction of left ventricular ejection fraction. We present the case of a 45-year-old man with a history of paroxysmal atrial fibrillation and a congestive heart failure, hypertension, age ⩾ 75 years, diabetes mellitus, stroke, vascular disease, age 65-74 years, and female sex (CHA2DS2-VASc) score of only 1 (because of reduced left ventricular ejection fraction) who presented w

Multiple myocardial bridges associated with left-ventricular dysfunction, intermittent left bundle branch block, and cardiac memory: A case report.

Myocardial bridges are a congenital anomaly in which a segment of the coronary artery takes a "tunneled" intramuscular course. Few reports have associated myocardial bridges with left-ventricular dysfunction in patients with ischemia. Intermittent left bundle branch block is a conduction disturbance that has been described to be associated with myocardial bridges and cardiac memory. This study reports unusual associations of multiple myocardial bridges, angina, left-ventricular dysfunction, intermittent left bundle branch block, and cardiac memory.

Endothelial Nitric Oxide Synthase-Induced Hypertrophy and Vascular Dysfunction Contribute to the Left Ventricular Dysfunction in Caveolin-1-/- Mice.

BACKGROUND: Caveolin-1 (Cav1)-/- mice display impaired development of left ventricular pressure and increased left ventricular wall thickness but no dilated ventricle; these are typical findings in patients with heart failure with preserved ejection fraction (HfpEF). Aiming to clarify if dysfunctional endothelial nitric oxide synthase (eNOS) influences cardiomyocyte contractility, cardiac conduction system, or afterload/vascular resistance, we studied Cav1-/-/eNOS-/- mice. METHODS: Cardiac function was assessed in vivo by pressure-volume-catheterization of the left ventricle, echocardiography

MANAGEMENT OF ATRIAL FIBRILLATION, VENTRICULAR ARRHYTHMIA, LEFT VENTRICULAR DYSFUNCTION, AND SUBSEQUENT CARDIOPATHOLOGIC EVALUATION IN AN ORANGUTAN (PONGO PYGMAEUS × ABELII).

A 37-yr-old male vasectomized hybrid orangutan (Pongo pygmaeus × abelii) was diagnosed with left ventricular dysfunction during a preventative health care examination. Treatment was initiated with carvedilol. The following year, this orangutan was evaluated for intermittent lethargy. Following observation of an irregular cardiac rhythm during an echocardiogram, a lead II electrocardiogram revealed atrial fibrillation and ventricular arrhythmia. Additional treatment included amiodarone, furosemide, spironolactone, clopidogrel, and aspirin. An improved activity level was noted, and follow-up tes

Inhaled Pulmonary Vasodilator Therapy for Management of Right Ventricular Dysfunction after Left Ventricular Assist Device Placement and Cardiac Transplantation.

Right ventricular failure (RVF) after cardiac transplant (CTX) or implantation of a continuous-flow left ventricular assist device (CF-LVAD) is associated with significant postoperative morbidity and mortality. A variety of modalities have been used to treat postoperative RVF, including management of volume status, intravenous inotropes and vasodilators, and right-sided mechanical support. Inhaled vasodilator agents are a unique treatment option aimed at minimizing systemic absorption by delivering therapy directly to the pulmonary vasculature. Current LVAD and CTX guidelines endorse inhaled v

PNPLA3-I148M reduces hepatic triacylglycerol secretion and mitigates diet induced left ventricular diastolic dysfunction.

PNPLA3-I148M, the greatest genetic determinant for metabolic dysfunction-associated steatotic liver disease (MASLD), paradoxically associates with reduced cardiovascular disease (CVD); however, the mechanisms are poorly understood. To investigate how PNPLA3-I148M alters hepatic triacylglycerol (TAG) metabolism and influences cardiac function, we expressed human WT-PNPLA3, PNPLA3-I148M, or GFP in the liver of PNPLA3-/- mice and fed them chow or Metabolic Dysfunction-Associated Steatohepatitis (MASH) diet at thermoneutrality. After 4 weeks of MASH diet, PNPLA3‑I148M mice showed reduced hepatic T

Arterial stiffness and left ventricular diastolic dysfunction: insights from the cardio-ankle vascular index.

BACKGROUND: Arterial stiffness is a contributor to cardiovascular morbidity, but its role in elevated left ventricular (LV) filling pressure and heart failure with preserved ejection fraction (HFpEF) remains incompletely understood. OBJECTIVES: This study aimed to evaluate the relationship between arterial stiffness and the ratio of early diastolic transmitral flow velocity (E) to early diastolic mitral annulus velocity (e'), as well as its association with LV diastolic dysfunction. METHODS: This study cross-sectionally analyzed 335 patients with cardiovascular disease and an ejection fracti

Early subclinical left ventricular dysfunction after surgical menopause: a prospective speckle-tracking echocardiography study.

BACKGROUND: Menopause is associated with adverse cardiovascular changes, while surgical menopause results in a more abrupt loss of ovarian function than natural menopause. However, its early impact on subclinical left ventricular systolic function remains insufficiently defined. This study aimed to compare longitudinal changes in left ventricular global longitudinal strain (LV GLS) between women with recently confirmed natural menopause and women undergoing surgical menopause. METHODS: This prospective observational study included 42 women with recently confirmed natural menopause and 39 peri

Association of Presystolic Wave with Subclinical Left Ventricular Dysfunction in Obstructive Sleep Apnea Syndrome.

BACKGROUND: Obstructive sleep apnea syndrome (OSAS) is associated with subclinical myocardial dysfunction. This study aimed to evaluate the association between presystolic wave (PSW) and left ventricular global longitudinal strain (GLS) in patients with confirmed OSAS and to determine whether PSW may serve as a simple echocardiographic marker of subclinical left ventricular systolic dysfunction. METHODS: A total of 103 patients with polysomnography-confirmed OSAS were prospectively enrolled. Patients were classified according to the presence or absence of PSW. Clinical characteristics, polyso

Surgical aortic valve replacement in severe left ventricular dysfunction: mortality, echocardiographic, and strain outcomes.

BACKGROUND: Patients with severe left ventricular (LV) dysfunction (ejection fraction [EF] ≤ 35%) undergoing surgical aortic valve replacement (SAVR) are considered high-risk patients. This study evaluated contemporary outcomes in such patients undergoing SAVR for severe aortic stenosis (AS) or aortic regurgitation (AR) and identified clinical and echocardiographic predictors of mortality. METHODS: This retrospective cohort study included 84 patients (> 18 years) with EF ≤ 35% who underwent SAVR between 2015 and 2024. The cohort comprised 53 patients with AS and 31 with AR.
