Dynamic left ventricular outflow tract (LVOT) obstruction has traditionally been associated with hypertrophic obstructive cardiomyopathy. Recently, acute dynamic LVOT obstruction has been described as a complication of myocardial infarction (MI). Herein the cases of 3 patients are described, all of whom presented with a systolic murmur and electrocardiographic evidence of MI. All 3 patients developed cardiogenic shock and were subsequently found by echocardiography to manifest an acute dynamic LVOT obstruction. Cardiogenic shock persisted until therapy was directed toward decreasing the degree

Acquired right ventricular outflow tract obstruction due to extrinsic compression of the pulmonary artery is a rare manifestation of non-Hodgkin's lymphoma (NHL). We report a case of a 17 year old boy who was referred for evaluation of a large anterior mediastinal mass, causing dyspnea and cough and resulting in a harsh systolic murmur. Echocardiography demonstrated compression of the pulmonary artery by the mass, with a severe pressure gradient. Biopsy revealed intermediate grade, diffuse large cell NHL. Systemic chemotherapy rapidly led to a significant decrease in the size of the mass, and

To determine predictors of exercise benefit in patients with hypertrophic cardiomyopathy after operative relief of left ventricular (LV) outflow tract obstruction, 30 patients underwent catheterization and exercise testing before and 6 months after operation, and hemodynamic measurements were obtained. The increase in maximal oxygen consumption (VO2max) during treadmill exercise testing was chosen as an index of exercise benefit. Univariate analysis showed a significant positive correlation of operative change in VO2max with preoperative LV end-diastolic and pulmonary arterial wedge pressures,

Left Ventricular Outflow Tract Obstruction (LVOTO) is a dynamic and often underrecognized cause of hemodynamic instability in critically ill patients. While more common in those with anatomical features like septal hypertrophy or hypertrophic cardiomyopathy, it can also occur in hyperdynamic states such as septic shock, stress-induced cardiomyopathy, acute hemorrhage/hypovolemia, or after excessive inotrope administration-even in patients without any anatomical predisposition. Its clinical presentation is often subtle and can mimic other causes of shock, making early recognition essential. A h

Severe left ventricular outflow tract obstruction (LVOTO) of hypertrophic cardiomyopathy is an acutely life-threatening, must-not miss, cardiology emergency that infrequently presents to the emergency department (ED). Patients with this condition usually manifest chest pain, syncope, cardiogenic shock, and severe ischemia. LVOTO is easy misdiagnosed as acute coronary syndrome. In our patient, the ECG showed a significant ST-segment depression and a 0/0 mmHg blood pressure when the peak left ventricular outflow tract gradient was abruptly increased by provocable activities. However, the patient

We present a case of a 58-year-old man for surgical myectomy due to recurrent left ventricular outflow tract (LVOT) obstruction, who had prior transaortic septal myectomy and embolization of the septal branch. On admission, transthoracic echocardiography showed a typical hypertrophic obstructive cardiomyopathy (HOCM) with asymmetric septal hypertrophy, significant LVOT obstruction and severe mitral regurgitation due to the systolic anterior movement of the anterior mitral valve leaflet. We performed a novel procedure of the transapical beating-heart septal myectomy, following which the LVOT ob

A woman in her 60s was brought to the operating room directly from the trauma bay for emergent intervention to repair a tracheal rupture sustained during a reported fall from standing height. She was intubated and sedated prior to arrival by paramedics for respiratory distress. Her family reported that the patient had dizziness after standing followed by the patient fainting and striking her neck. During a preprocedural point-of-care cardiac ultrasound examination, we discovered severe left ventricular hypertrophy with a significantly increased ejection fraction and decreased left ventricular

BACKGROUND: A subset of patients with takotsubo cardiomyopathy will develop significant dynamic left ventricular outflow tract (LVOT) obstruction leading to cardiogenic shock. However, traditional therapies for cardiogenic shock that focus on increased inotropy and afterload reduction can be detrimental in this situation.
CASE SUMMARY: We describe a 71-year-old woman who presented to the emergency department with typical, substernal chest pain found to be hypotensive with ST-elevations in the lateral leads. Coronary angiography showed no significant coronary artery disease, but a left ventric

OBJECTIVES: To determine the factors influencing left ventricular outflow tract (LVOT) area reduction after a mitral valve-in-valve (VIV) or a valve-in-ring (VIR) procedure.
BACKGROUND: Transcatheter heart valves (THVs) are increasingly used in performing a VIV or a VIR procedure in high-risk patients. Although less invasive, a potential complication is LVOT obstruction. However, the factors predisposing to LVOT obstruction are ill defined.
METHODS AND RESULTS: To understand the effects of the various factors, the study was carried out in three parts: To understand the effect of VIV and VIR

In hypertrophic cardiomyopathy (HCM), rupture of mitral chordae tendineae is infrequent and causes acute haemodynamic deterioration. A 38 year old male patient had chordae rupture leading to prolapse of both mitral leaflets and severe regurgitation, without change in symptomatic status. One year before, he had had mild mitral regurgitation and a resting left ventricle outflow tract of 105 mm Hg that disappeared in the present evaluation. In this unique case, worsening of mitral regurgitation was counterbalanced by total relief of the severe obstruction. This case report highlights the role of

We describe a new method of aortoventriculoplasty in a patient with calcified mitral stenosis, aortic valvular stenosis, severe left ventricular outflow tract obstruction, and aneurysm of the ascending aorta. This complex pathology was successfully treated with replacement of both the valves and a tubular dacron graft. The proximal end of the dacron tube was tailored as a patch for the repair of the ventricular septum and the aortic root, and the distal end was anastomosed to the distal ascending aorta. The patient had an uneventful recovery and postoperative echocardiography showed no signifi

UNLABELLED: Hypertrophic obstructive cardiomyopathy (HOCM) is an uncommon familial disorder, traditionally characterized by asymmetric septa … tension, aortic stenosis, or sigmoid-shaped septum (2).
IMPLICATIONS: During surgery, dynamic left ventricular outflow obstruction (LVOT) can potentially occur frequently, but diagnosis may be less frequent. When circulatory disturbance occurs with suspicion of LVOT obstruction, transesophageal echocardiography can provide exact proof of diagnosis and basis for immediate treatm …

Aneurysm of the membranous ventricular septum causing subpulmonic obstruction in patients with corrected transposition is rare but well documented. Higher left-sided (morphologic right) ventricular pressure causes protrusion of the aneurysm into the pulmonary outflow tract of the right-sided morphologic left ventricle, causing obstruction related to lack of conus and proximity of the aneurysm to subpulmonary region. Echo Doppler studies and angiography are useful in making a correct diagnosis. Surgical resection of the aneurysm along with closure of the ventricular septal defect is the treatme

BACKGROUND: Assessment of maximally provoked LVOT obstruction (LVOTO) guides treatment decisions in obstructive hypertrophic cardiomyopathy (HCM). Guidelines recommend various maneuvers, with exercise stress echocardiography (ESE) as the preferred method if others fail. Comparative data are limited. Therefore, in this prospective study, 20 patients with obstructive HCM underwent transthoracic echocardiography (TTE) with continuous wave Doppler measuring LVOT gradients at rest, during the Valsalva maneuver, handgrip exercise at 30% and 50% of maximal hand force, squats, and bicycle ESE, with th

OBJECTIVE: We sought to explore the role of timing on outcomes of the arterial switch operation + ventricular septal defect closure.
METHODS: Neonates undergoing the arterial switch operation + ventricular septal defect closure from the Pediatric Health Information System database (2004-2022) were identified. Patients with outflow tract obstruction were excluded. Baseline features and outcomes were compared by dividing the cohort by age at the arterial switch operation + ventricular septal defect closure: very early (0-7 days), early (8-14 days), late (15-21 days), and very late (>21 days). A

Giant pseudoaneurysms of coronary arteries in patients with Behçet's disease is a uncommon finding. It has been described exceptionally in the literature. We present a case of giant pseudoaneurysm of the left anterior descending coronary artery with obstruction of the right ventricular outflow in a patient with Behçet's disease. He improved after surgical resection and steroid treatment.
