Psychosomatic study of 60 patients with vertigo.

The authors examined 60 consecutive patients hospitalized in Modena University Otorhinolaryngological Clinic for vertigo by means of an interview and of three self-rating scales (Zung's SDS, SAS and the Middlesex Hospital Questionnaire). The control group was composed of an equal number of patients hospitalized in the same ward and period for different nonsurgical otiatric diseases; the two groups were matched for age, sex, residential area, sociocultural conditions, duration of hospitalization and disease. Paroxysmal vertigo with and without cochlear symptoms.

The entity paroxysmal vertigo with hearing impairment and tinnitus can be distinguished into six different types, of which only one can be named Ménière's disease. Out of 1474 patients suffering from paroxysmal vertigo only 31% (457) met the requirements set for Ménière's disease. Attention is drawn to the fact that many patients suffer from syndromes partly resembling Ménière's disease but lack one or more of the cardinal symptoms.

The dizzy patient. Symptomatic treatment of vertigo.

Vertigo is a distressing symptom, but in the majority of cases it can be treated effectively. Whenever possible, treatment should be directed at the underlying disorder. Usually, however, symptomatic treatment alone or in combination with specific therapy is effective. Symptomatic treatments include antivertiginous medications, vestibular exercises, and surgical therapy. The choice of medications is determined by the known side effects of the drugs and the time course of symptoms. An early vestibular exercise program ensures a more rapid return to normal physical activity. Surgical therapy is …

Positional vertigo and nystagmus of central origin.

Four cases are described illustrating the clinical features of positional vertigo and nystagmus due to posterior fossa tumors and a case of obstructive hydrocephalus. In these cases positional vertigo was the first and only presenting symptom of central nervous system disease. One case of subependymoma of the fourth ventricle and one with hydrocephalus had characteristic symptoms of benign positional vertigo; each showed positional nystagmus of the benign paroxysmal type.

Abnormalities of the stapedius reflex in patients with vertigo.

Analysis of the latency, amplitude, and rise characteristics of the stapedius reflex as demonstrated on a strip recorder revealed abnormalities in patients with vestibular neuronitis and other vertigo syndromes. The findings in vestibular neuronitis, acute labyrinthitis, perilymphatic fistulas, Meniere's disease, cerebrovascular insufficiency, and metabolic vertigo are described. Alterations in the stapedius reflex included increase in latency, decrease in amplitude, stepping of the rise, and absence of the reflex. This study suggests that absence of the reflex can be associated with vertigo a…

Vertigo as reflected by the nystagmogram. A clinical analysis.

Efforts to evaluate findings in the nystagmogram were made in a material of 338 vertiginous and dizzy patients. A spontaneous nystagmus, a p … ontaneous nystgmus on eye-closure or by dysrhythmic nystagmus in caloric tests. Cases of functional vertigo seem to differ from cases of vertigo from other sources by their increase of spontaneous nystagmus (when present) or by persistence of caloric nystagmus on eye-closure.
