The effect of intratympanic gentamicin as a prehabilitation strategy for objective and subjective vestibular function in patients undergoing microsurgery for a unilateral vestibular schwannoma.

OBJECTIVE: To review the literature on intratympanic gentamicin treatment as prehabilitation for patients undergoing surgery for a unilateral vestibular schwannoma. DATA SOURCES: A systematic literature search was conducted up to March 2023 in Pubmed, Embase, Cochrane, Web of Science, Academic Search Premier, Google Scholar and Emcare databases. REVIEW METHODS: Articles on the effect of intratympanic gentamicin followed by vestibular schwannoma surgery were reviewed. Data on objective vestibular function and subjective outcomes were compiled in tables for analysis. Relevance and methodologic

The history and future of neuro-otology.

PURPOSE OF REVIEW: The field of neuro-otology has advanced substantially over the past 150 years. The function of the vestibular system was presumed to be audiologic prior to the groundbreaking work of Prosper Ménière in the late 1800s. Since that time, scientific discovery has advanced our knowledge of the physiology and the pathophysiology of the vestibular system. This article describes where we have been, where we are now, and where we need to go from here. RECENT FINDINGS: Some of the important recent advances include: (1) improved imaging of the brain and inner ear, (2) development of n

Short-latency EMG potentials elicited by head taps in sternocleidomastoid muscles: a study on normal human subjects and patients with central or vestibular lesions.

The aim of the present study was to determine whether a tap on the forehead evokes a vestibulospinal reflex in sternocleidomastoid muscles in normal human subjects and patients with central or vestibular lesions. While first positive-negative short-latency EMG potentials with a mean positive potential peak of about 12 ms and a mean negative potential peak of about 17 ms were found in the bilateral sternocleidomastoid muscles in normal subjects, they disappeared or were attenuated in patients with central or peripheral vestibular lesions.

[Classification of acute vestibular syndrome].

Objective:To explore the epidemiological characteristics and to help accomplish accurate diagnosis and treatment strategies by analyzing the composition and clinical features of various diseases with acute constant vertigo.Method:We retrospectively analyzed medical records (including name, sex, age, diabetes, hypertension, history of vertigo, family history, etc.),otoneurological examination, vestibular function tests and radiological examination of patients with acute vestibular syndrome. We classified various diseases according to diagnostic criteria, and then analyze the clinical data.Result…

Vestibular neurectomy: a histological and clinical study of results.

Posterior fossa vestibular neurectomy has become one of the most effective surgical procedures for control of vertigo symptoms in Menière's disease. A small group of patients continue to have vertigo and demonstrable vestibular function by electronystagmography (ENG) post-operatively. Another group of patients may have no vertigo, despite residual vestibular function on caloric testing. In an effort to correlate histological findings with clinical outcome, nerve biopsies were taken from seven patients undergoing vestibular neurectomy. Fibre counts of nerve biopsies were compared with clinical,

Comparing the Impact of High-Intensity Exercise on Vestibular-Oculomotor Function Between Sex, Sport, and Activity Level.

OBJECTIVE: To investigate the impact of high-intensity exercise on the vestibular and oculomotor systems in a group of sedentary and active males and females. DESIGN: Prospective observational study. SETTING: Performance laboratory; Institute of Sport, Exercise and Health, London. PARTICIPANTS: Forty-five collegiate athletes and 30 age-matched sedentary participants. INTERVENTIONS: A baseline Vestibular and Oculomotor Screening (VOMS) test was performed on all participants before undertaking a self-paced 5-minute treadmill run maintaining an intensity of 17/20 on the Borg scale. Post-run,

Postural Control in Adults With Age-Related Hearing Loss.

PURPOSE: The purpose of this study was to evaluate postural control in adults with age-related hearing loss (ARHL) by adding a variation of a 30° lateral head tilt for each measure. METHOD: Individuals between the ages of 50 and 70 years were recruited (10 with normal hearing, 32 with ARHL) and evaluated using pure-tone audiometry, vestibular function assessments, and postural control measures. Vestibular function assessments used were video head impulse test (vHIT), cervical vestibular evoked myogenic potentials (VEMPs), and ocular VEMPs. Postural control measures used were single leg stance

Inferior vestibular neuritis: a novel subtype of vestibular neuritis.

OBJECTIVE: To report eight cases of inferior vestibular neuritis, in order to raise awareness of this new subtype of vestibular neuritis. MATERIALS AND METHODS: We retrospectively analysed 216 patients (104 males and 112 females; age range 10-64 years; mean age 38.4 years) with full clinical documentation who had attended our hospital's vertigo clinic between May 2007 and December 2008. All patients underwent systematic investigation, including hearing tests, radiology, caloric testing and vestibular evoked myogenic potential testing. RESULTS: Of 216 patients with vestibular neuritis, eight

Retrolabyrinthine vestibular nerve section: a viable alternative to the middle fossa approach.

Vestibular nerve section, whether by the middle fossa or retrolabyrinthine approach, is effective in relieving intractable vertigo while preserving hearing. However, the potential morbidity and technical difficulty of the middle fossa approach have limited its usefulness. In an attempt to determine if the two approaches produce comparable results, we evaluated 52 patients who underwent retrolabyrinthine vestibular neurectomy between April 1981 and March 1983 at the Otologic Medical Group. We compared their audiometric data and questionnaire responses with published data on patients who had a m

The Efficient Dizziness History and Exam.

Dizziness is a common complaint in otolaryngology clinics and can present a diagnostic challenge. A thorough history including onset, duration, and exacerbating and alleviating factors, along with physician persistence, can help differentiate between otologic and nonotologic forms of dizziness. An otologic and neurotologic physical examination, including vestibulo-ocular reflex and cranial nerve function evaluation and postural examination, can shed further light on symptom etiology. Otologic forms of dizziness often result in vertigo and may be associated with unilateral symptoms of hearing l

[The otoneurological symptoms in giant adenomas of the hypophysis].

Analysis of the otoneurological symptomatology in 32 patients with hypophyseal tumors showed that they extended most frequently into the sphenoidal sinus, the cells of the ethmoidal sinus, the superoposterior parts of the nose, and the nasopharynx. The focal symptoms of compression of the adjoining parts of the brain (frontal, temporal) were manifested in 11 among 32 cases by olfactory disorders (loss of olfaction in 4 patients, diminished olfaction in 2, olfactory hallucinations in 4 patients, and impaired recognition of odors in 1 patient). Asymmetric action of the tumor on the subcortical s
