The study of the temporal region has been improved by technologic progress from conventional radiography to linear tomography, then to pluridirectional tomography, CT and, finally, MRI. Conservative shunting procedures, i.e. ESS, ultrasonic irradiation and cryosurgery, are based on the assumption that there is increased volume and/or pressure of endolymph in Menière's disease. The histologic findings in the temporal bones of three patients who died from viral encephalopathy are presented. Study is performed on 133 patients with vestibular disturbances with no data for inflammatory diseases. Their age group is from 30 to 60, divided into three clinical syndromes: peripheral, central and combined. Thirteen individuals with unilateral Menière's disease were chosen for experimental chemical labyrinthectomy. All subjects suffered from frequent attacks of disabling vertigo and had mean pure-tone thresholds greater than 50 dB in the affected ear. The surgical procedure was similar to stapedectomy; after removal of the stapes, several crystals of NaCl were placed in the vestibule which was then covered with a vein graft. A guinea pig model of viral labyrinthitis was established by inoculating guinea pig cytomegalovirus into the inner ear and then determine its effect on vestibular function. Neither spontaneous nor positional nystagmus could be detected postoperatively. Nine out of 18 (50%) animals had caloric hyperfunction in the virus-inoculated ear at 1 to 3 weeks postoperatively. Five of these 9 animals also had caloric hyperfunction in the control ear. Neurotoxic volatile organic solvents used by house and car painters may lead to professional toxic encephalopathy after several years of exposure. The symptoms are memory impairment, fatigue, personality changes, headache and dizziness.
