We report the case of an 83-year-old woman who presented with an abnormal pulmonary nodule suspected to be lung cancer in the left S3 segment. Bronchoscopy showed that the left main bronchus branched off into the B1+2, B3 plus lingular bronchus, and lower bronchus. Video-assisted thoracic surgery was performed, and the nodule was pathologically diagnosed as a primary lung cancer. Subsequently, left upper lobectomy was performed, and an abnormal bronchus was observed behind the main pulmonary artery. Intraoperative bronchoscopy indicated that the bronchus was the displaced B1+2. The B3 plus lin

OBJECTIVE: Video-assisted thoracoscopic surgery (VATS) has been a surgical intervention of choice for the treatment of spontaneous pneumothorax (SP) with lung bulla. Our objective was to introduce a uniportal VATS approach for simultaneous bilateral bullectomy and to evaluate its therapeutic efficacy.
METHODS: Between May of 2011 and January of 2012, five patients underwent bilateral bullectomy conducted using this approach. All of the patients presented with bilateral SP. Preoperative HRCT revealed that all of the patients had bilateral apical bullae. We reviewed the surgical indications, su

Solitary mediastinal metastasis of hepatocellular carcinoma (HCC) is extremely rare. We report a case of solitary mediastinal metastasis arising from HCC treated by video-assisted thoracic surgery (VATS). A 75-year-old man underwent hepatic resection and transcatheter arterial embolization for HCC. The level of protein induced by vitamin K antagonist was subsequently found to be elevated. Computed tomography showed a 2-cm mediastinal tumor. (18)F-fluorodeoxy-glucose positron emission tomography revealed slight uptake by the tumor. VATS was performed. The histopathological diagnosis was metasta

INTRODUCTION: Erdheim-Chester disease is a rare, non-Langerhans histiocytosis in which pericardial involvement is diagnosed with increasing frequency and is associated with high mortality rates.
PRESENTATION OF CASE: A 53-year-old woman presented with progressive exertional dyspnea and pericardial effusion was discovered. Further investigations revealed the presence of a diffuse, infiltrating process and a diagnosis of Erdheim-Chester disease was made. An emergent pericardiocentesis by subxiphoid approach was completed but recurrent drainage obviated removal of the pigtail catheter. A pleuro-

A 32-year-old woman with ocular-typed myasthenia gravis (MG) without thymoma was admitted to the hospital to undergo surgical treatment. Video-assisted thoracoscopic surgery (VATS) extended thymectomy was successfully performed with three small ports using manual manipulators, the Radius Surgical System (RSS). The device may facilitate many degrees of freedom to general thoracic surgery.

BACKGROUND: Concerns remain unresolved regarding the safety of unplanned conversion to open thoracotomy during video-assisted thoracic surgery (VATS) lobectomy. We analyzed both early and late outcomes after thoracotomy conversion from VATS.
METHODS: From December 2003 to December 2008, a total of 738 VATS lobectomies were attempted. Among them were 34 unplanned conversions to open thoracotomy. Patient characteristics, operative data, and early and late postoperative outcomes were analyzed retrospectively.
RESULTS: Among the 34 conversion cases, 26 patients had lung cancer and 8 had benign l

The use of central venous catheters (CVCs) nowadays is a routine practice in the treatment of severely acute-diseased children. However, the procedure still carries a risk of morbidity, and severe complications are reported. When respiratory and/or hemodynamic instability develop after the procedure, prompt patient evaluation to exclude iatrogenic damage is mandatory, regardless of the primary patient condition. If a vascular injury related to CVC placement procedure is detected, the availability of an interventional radiologist and/or any surgical facilities plays an important role in the man

The aim of this report was to evaluate the effectiveness of video-assisted thoracoscopic surgery (VATS) in staging, diagnosis, and treatment of lung cancer. Fifty-two patients were scheduled for mediastinal lymph node VATS biopsy at the Oncologic Thoracic Surgery Department of the National Cancer Institute in Milan. Fifty patients underwent lymph nodal thoracoscopic biopsy (96%), whereas for the other 2 patients, histologic diagnosis was done by pleural metastatic nodule thoracoscopic biopsy (4%). We performed 17 lymph nodal biopsies in level 5 (33%), 14 in level 6 (27%), 12 in level 7 (23%),

We report the case of a 45-year-old man treated with intrathoracic hyperthermo-chemotherapy (IPHC) under video-assisted thoracoscopic surgery (VATS) for malignant pleural dissemination originating from invasive thymoma. The clinical course of invasive thymoma with malignant pleural dissemination is characterized by early and frequent metastasis and poor survival. In this case, we performed IPHC under VATS with good results. IPHC under VATS is considered a less invasive and more effective approach for malignant pleural dissemination.

We report a case of pulmonary cryptococcosis diagnosed by video-assisted thoracoscopic surgery (VATS) with CT-guided localization. A 61-year-old man was admitted to our hospital for the precise diagnosis of a solitary mass in the left upper lung. Because the mass was too small to be detected under VATS, we tried CT-guided localization for surgery. One day before VATS, we placed a marking device besides the lesions after percutaneous puncture. We used a Naruke point marker as a marking device. After this procedure, the wedge resection was performed. The pathological diagnosis was cryptococcosis

PURPOSE: The aim of this study was to analyse the effectiveness of patent ductus arteriosus (PDA) closure by the video-assisted thoracic surgery (VATS).
METHODS: The technique was utilized in 6 newborn lambs firstly. Three to four small incisions (3 to 10mm) were used in each animal to permit the introduction of lung retractors, video equipment, dissectors and clip appliers. The procedure was accompanied by video monitoring and after the dissection, the PDA was closed by 2 titanium clips. Seven days after, the animals were sacrificed and submitted to pathological study. Based on this initial

BACKGROUND: Non-intubated uniportal video-assisted thoracoscopic surgery (niVATS) is a novel approach to major and minor lung resection. It benefits from a holistic anesthesiological concept with adequate pain relief and sedation in a minimal-invasive setup allowing thoracic procedures under spontaneous breathing. At present no anesthesiological gold standard for niVATS exists. The primary aim of our retrospective observational study was to evaluate feasibility and safety of minimally invasive niVATS for both minor and major pulmonary resections at our institution.
METHODS: All 88 consecutive

We report a case of an entrapped lung after the pectus bar repair of a pectus deformity. The entrapped lung was found incidentally during video-assisted thoracoscopic surgery (VATS) for pneumothorax. Based on VATS exploration, multiple bullae seemed to be the cause of the pneumothorax, but the entrapped lung was suspected to have been a cause of the air leakage.

OBJECTIVES: Postoperative air leakage following lung surgery prolongs chest drainage duration and length of hospitalization and the evidence regarding treatment with autologous blood pleurodesis (ABP) is scarce. We decided to investigate if ABP would influence postoperative air leakage when used on the first postoperative day following elective video-assisted thoracoscopic surgery (VATS).
METHODS: In this multicenter randomized controlled trial, patients with an air leakage greater than 40 ml/min during cough and mobilization on their first postoperative day following elective VATS were rando

BACKGROUND: Uniportal video-assisted thoracoscopic surgery (uVATS) has emerged as a less invasive alternative to conventional multiportal video-assisted thoracoscopic surgery (mVATS) for lung cancer resections. While uVATS is associated with short-term perioperative advantages, uncertainty remains regarding its mid- to long-term oncologic outcomes. This systematic review and meta-analysis compares perioperative outcomes and mid-term survival following uVATS versus mVATS for non-small cell lung cancer (NSCLC).
METHODS: Four electronic databases were searched from inception to August 2025 in ac

BACKGROUND: Comprehensive informed consent is a core surgical responsibility but is increasingly challenged by rising patient volumes and limited personnel resources. Digital solutions in healthcare offer opportunities to optimize patient education. Video-assisted informed consent may reduce time expenditure without compromising informational quality. The objective was to compare video-assisted with conventional informed consent regarding duration, patient understanding, patient, and physician satisfaction.
METHODS: In this prospective, cluster quasi-randomized clinical trial at the Universit

OBJECTIVES: Postoperative pain after thoracic surgery remains a significant challenge, underscoring the need to identify patients at risk of severe pain. While prior research across various surgical types highlights the Pain Catastrophizing Scale (PCS) as a key psychological predictor of acute postoperative pain, evidence specific to thoracic surgery is limited. This study aimed to investigate the association between PCS and postoperative pain in patients undergoing multiportal video-assisted thoracoscopic surgery (VATS) lobectomy.
METHODS: We conducted a prospective observational study inclu

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) is associated with pulmonary function impairment due to the nature of the surgical site. The use of esketamine, which acts on multiple receptors, has become widespread in general anesthesia, but its function in pulmonary protection remains unclear. We examined whether esketamine can improve pulmonary function during VATS, further investigated the underlying mechanisms of its effect, and evaluated its dose-effect relationship.
METHODS: Patients aged 18-65 years scheduled for VATS were included in the study. Patients were assigned randomly
