Pre-endoscopic antibiotics for the prevention of bacterial endocarditis: do we use them appropriately?

OBJECTIVE: Because native valve bacterial endocarditis following endoscopic procedures is extremely rare, recent guidelines have discouraged indiscriminate use of pre-endoscopic antibiotics. The purpose of this study was to determine whether these guidelines have had an impact in prescribing antibiotic prophylaxis prior to endoscopic procedures. METHODS: All ambulatory endoscopic procedures carried out from January 1, 1992, through December 31, 1992, in a 412-bed community hospital in Northern Virginia were reviewed to identify cases receiving antibiotic prophylaxis. Inpatients, percutaneous …

Using high-frequency monitoring data to quantify city-wide suspended-sediment load and evaluate TMDL goals.

Excess sediment is a common reason water bodies in the USA become listed as impaired resulting in total maximum daily loads (TMDL) that require municipalities to invest millions of dollars annually on management practices aimed at reducing suspended-sediment loads (SSLs), yet monitoring data are rarely used to quantify SSLs and track TMDL progress. A monitoring network was created to quantify the SSL from the City of Roanoke, Virginia, USA (CoR), to the Roanoke River and Tinker Creek and help guide TMDL assessment and implementation. Suspended-sediment concentrations were estimated between 202…

Injured Infants Act: study compares definition, newborn records.

In a study of a teaching hospital's newborn records 1986-1987, the definition of severe neurological injury in Virginia's Injured Infants Act was found to be associated with neonatal death, prematurity, and a preexisting maternal condition likely to contribute to poor outcome. The incidence of severe neurologic injury did not approach the prevalence of severe mental retardation and cerebral palsy. Thus the ability of the Act to preempt litigation substantially is questionable, the authors state, and they recommend a broader definition.

An epizootic of cerebrospinal nematodiasis in cottontail rabbits.

A progressively debilitating neurologic syndrome characterized by torticollis, ataxia, circling, and abnormal posture was observed in 16 of approximately 60 cottontail rabbits (Sylvilagus floridanus) captured in a 12-hectare portion of a woodlot in southwestern Virginia. Inflammatory and degenerative alterations in cerebellum and midbrain regions were characterized principally by malacia, perivascular cuffing, and gliosis. Single Ascaris columnaris larvae were found in the cerebellum of 2 rabbits and the spinal cord of 1 rabbit.

Puerperal inversion of the uterus.

Acute puerperal inversion of the uterus is an uncommon but potentially fatal obstetric complication. In the 15 year period ending December 31, 1974, 11 inversions have been managed at the University of Virginia Hospital. All were recognized immediately and manually replaced and there was no significant postpartum morbidity. Successful management appears to depend largely upon prompt recognition, which can be achieved by routine postpartum examination of the vagina and cervix and manual exploration of the uterus.

Fueling the fall migration of the monarch butterfly.

Monarch butterflies in eastern North America accumulate lipids during their fall migration to central Mexico, and use them as their energy source during a 5 month overwintering period. When and where along their migratory journey the butterflies accumulate these lipids has implications for the importance of fall nectar sources in North America. We analyzed the lipid content of 765 summer breeding and fall migrant monarch butterflies collected at 1 nectaring site in central Virginia over 4 years (1998-2001), and compared them with 16 additional published and unpublished datasets from other site…

Developing an identity model for transgender and intersex inclusion in lesbian communities.

The article briefly highlights past ideologies of Essentialism and Constructionist thought, identifying their weaknesses as arguments of exclusion. By combining corporeality and mentality, the article posits an identity model, the spherical characterization model that argues for transgender inclusion in lesbian space by deconstructing the ideology behind identity group construction. Next the article applies the spherical characterization model to the characters in Virginia Woolf's 'Orlando' and Jeffrey Eugenides' 'Middlesex' to delineate how the model argues for transgender inclusion within le…

Case Reports of Heroin Injection Site Necrosis: A Novel Antecedent of Nicolau Syndrome.

Heroin injection-site necrosis (HISN) is a novel and poorly understood complication of intravenous drug abuse (IVDA). We present three cases of HISN that were evaluated and treated in Charleston, West Virginia, in 2019 and 2020. The documented cases show similarities involving patient care, follow-up, clinical progression, patient demographic, and dermatologic sequelae. We discuss these similarities, provide clinical recommendations, review proposed etiologies of HISN, and introduce Nicolau syndrome as a potential mechanism.

Endobronchial blastomycoses: A rare pathogen in a unique location.

A middle-aged woman from Southwest Virginia presented to pulmonary clinic with 4 months of dry cough. Further imaging with Computed Tomography (CT) of the chest showed an infiltrative lung mass. The patient underwent bronchoscopy that showed an endobronchial lesion on right and left main stem bronchi. Endobronchial biopsy of the lesion showed acute and chronic granulomatous inflammation and tissue cultures grew Blastomycoses dermatitides. We hereby present a rare case of endobronchial blastomycoses with pulmonary infiltrates presenting as chronic cough.

Evaluating the feasibility of pharmacist-facilitated tobacco cessation interventions in independent community pharmacies in rural Appalachia.

BACKGROUND: Smoking rates in the United States are the highest in underserved rural regions. Thus, more points of contact are needed to link smokers to evidence-based cessation programs. OBJECTIVES: The purpose of this study was to conduct an evaluation to determine the feasibility, acceptability, and interest among rural pharmacists in implementing a pharmacist-facilitated smoking cessation program in independent community pharmacies in rural Appalachian communities in Virginia, North Carolina, Tennessee, and West Virginia. METHODS: This study utilized a complementary sequential mixed-metho…

Associations Between Surface Mining Airsheds and Birth Outcomes in Central Appalachia at Multiple Spatial Scales.

A considerable body of research exists outlining ecological impacts of surface coal mining, but less work has explicitly focused on human health, and few studies have examined potential links between health and surface coal mining at fine spatial scales. In particular, relationships between individual birth outcomes and exposure to air contaminants from coal mining activities has received little attention. Central Appalachia (portions of Virginia, West Virginia, Kentucky, and Tennessee, USA), our study area, has a history of resource extraction, and epidemiologic research notes that the region…

Barriers to using new needles encountered by rural Appalachian people who inject drugs: implications for needle exchange.

BACKGROUND: Using a new needle for every injection can reduce the spread of infectious disease among people who inject drugs (PWID). No previous study has examined new needle use barriers among PWIDs residing in the rural Appalachian part of the United States, an area currently in the midst of a heroin epidemic. OBJECTIVE: Therefore, our primary aim was to explore self-reported barriers to using a new needle by PWID attending a needle exchange program (NEP). METHODS: We conducted a cross-sectional survey of PWID attending two NEPs in rural West Virginia located in the heart of Central Appala…
