Bilateral primary vitreoretinal lymphoma masquerading as fungal endophthalmitis- a case report.

PURPOSE: To report a case of bilateral primary vitreoretinal lymphoma (PVRL) masquerading as endophthalmitis in a patient with a history of bilateral cataract surgery and COVID-19. OBSERVATION: A 60-year-old male patient presented with diminution of vision in both the eyes. There was a history of bilateral cataract surgery done 2 months back at a gap of one week and COVID-19 infection treated with high dose systemic corticosteroids. Patient presented with dense vitritis with yellowish sub-retinal pigment epithelium (RPE) deposits in both the eyes six weeks after cataract surgery. It was clini

[Epiretinal gliosis].

Epiretinal membranes represent avascular cellular proliferations on the retinal surface, preferentially in the area of the macula. Idiopathic, primary epiretinal membranes are a relatively common finding, especially in elderly people. Other secondary pathomechanisms include retinal tears, trauma, ophthalmic surgical procedures including retinal detachment surgery, laser coagulation and cryotherapy of the retina, or as a result of inflammatory diseases. Individual symptoms depend on the degree of cellular proliferation and associated tangential traction forces at the vitreoretinal interface res

Descemet-stripping automated endothelial keratoplasty for vitrectomized cases with traumatic aniridia and aphakic bullous keratopathy.

The surgical indication for Descemet-stripping automated endothelial keratoplasty (DSAEK) is largely limited to phakic or pseudophakic cases of endothelial dysfunction with normal pupils, because the endothelial lenticule is generally attached to the recipient cornea by use of gas tamponade into the anterior chamber. Although it may be desirable for vitrectomized cases with aniridia and aphakic bullous keratopathy without capsule support to undergo DSAEK, one of the major problems is lenticule detachment during surgery or in the postoperative period. To perform DSAEK in such cases, special sur

[Contractile elements in proliferative retinal diseases].

Contraction of epiretinal membranes remains the leading cause of failure in retinal surgery. The mechanism of contraction of cellular units is as yet unknown. We have used immunohistochemical methods to demonstrate the intracellular localization of the proteins actin, myosin, tropomyosin and vinculin, which are thought to be responsible for cellular contraction, in 26 surgically obtained epiretinal traction membranes from patients with traumatic (n = 12) and idiopathic (n = 7) proliferative vitreoretinopathy, and proliferative diabetic retinopathy (n = 7). The use of cell marker proteins for g

Biological and therapeutic aspects of proliferative vitreoretinopathy.

Conventional vitreoretinal surgery for proliferative vitreoretinopathy is unsatisfactory despite numerous innovations over the past 20 years. Silicone oil injections in this series resulted in 70% reattachment of these end-stage cases, but within twelve months over 90% had no useful visual result. The development of an in vitro model for 3-dimensional culture of cells has allowed evaluation of numerous potential drugs with anti-proliferative or anti-contractile activities. Amongst these, taxol has shown the most promise. There remain, however, doubts about possible long-term toxicity to the op…

Cystoid Macular Edema Secondary to Acute Retinal Necrosis: The Role of Fundus Fluorescein Angiography in Guiding Treatment.

Cystoid macular edema (CME) is an uncommon but vision-threatening sequelae of acute retinal necrosis (ARN). We report a patient who developed CME following vitreoretinal surgery for ARN-associated retinal detachment. Multimodal imaging, particularly fundus fluorescein angiography (FFA), revealed diffuse capillary leakage suggestive of persistent retinal vascular inflammation. Initial therapy with topical interferon alpha-2b yielded no anatomical or visual benefit. In contrast, intravitreal sustained-release dexamethasone resulted in rapid resolution of CME and significant visual improvement wi…

Biostatistical analysis of pseudophakic and aphakic retinal detachments.

Removal of the crystalline lens increases the risk of rhegmatogenous retinal detachment (RD) by creating changes in the ocular environment that predispose to development of retinal breaks. The evolution of cataract surgery from intracapsular cataract extraction (ICCE) to extracapsular cataract extraction (ECCE) and phacoemulsification has reduced the incidence of RD, while advances in vitreoretinal surgery have resulted in improved outcomes when retinal detachment does occur. The incidence of RD varies between 0.4-3.6% for ICCE and between 0.55-1.65% for ECCE. In eyes having undergone phacoemu…

[Temporary keratoprosthesis, vitrectomy and autokeratoplasty in endophthalmitis treatment].

Postsurgical endophthalmitis frequently requires emergency vitrectomy. Loss of corneal transparency can preclude vitreoretinal surgery. Due to the narrow time frame in which surgical treatment needs to be performed, donor corneas are often not available for grafting. In addition, the risk of primary graft failure is higher in eyes with acute endophthalmitis. Later regrafting after primary graft failure carries a higher risk of graft rejection. By a clinical case of endophthalmitis after cataract surgery, we demonstrate the surgical technique of combined pars plana vitrectomy, keratoprosthesis,…

Surgical Antimicrobial Prophylaxis in Patients of Neonatal and Pediatric Age Subjected to Eye Surgery: A RAND/UCLA Appropriateness Method Consensus Study.

Ocular surgery encompasses a wide range of procedures, including surgery of the tear ducts, eyelid, cornea and conjunctiva, lens, ocular muscle, and vitreoretinal and iris surgery. Operations are also performed for the removal of tumors, repairs of ocular trauma and, finally, corneal transplantation. Antibiotic prophylaxis for the prevention of surgical site infections (SSIs) in ocular surgery is a complex field in which shared lines of action are absent. In light of the scarcity of shared evidence in the use of ocular antimicrobial prophylaxis for the pediatric population, this consensus docu
