Australian interdisciplinary healthcare providers' perspectives on the effects of broader pre-exposure prophylaxis (PrEP) access on uptake and service delivery: a qualitative study.

UNLABELLED: Background The addition of pre-exposure prophylaxis (PrEP) for HIV prevention to the Australian Government-subsidised Pharmaceutical Benefits Scheme (PBS) enables any doctor or nurse practitioner to prescribe it and has increased accessibility options. However, understanding of Australian healthcare providers' (HCP) knowledge and preparedness to prescribe PrEP remains limited. METHODS: Semistructured interviews, conducted before PBS listing (October 2016-April 2017), explored PrEP knowledge and prescription experiences of 51 multidisciplinary HCPs involved with the Queensland Pre-

Training Clinicians in Serious Illness Communication Using a Structured Guide: Evaluation of a Training Program in Three Health Systems.

Background: Failure to initiate discussions about patients' values and goals in serious illness remains a common problem. Many clinicians are inadequately trained for these discussions. Objective: Evaluate whether a novel train-the-trainer model results in high-quality training that improves clinicians' self-reported competencies in serious illness communication. Design: Multimethod evaluation of an educational program. Setting/Context: In 2016, three faculty at Ariadne Labs (AL) conducted three train-the-trainer courses to equip faculty trainers at each of the three institutions to teach seri

Morbidity management and disability prevention for lymphatic filariasis in Sri Lanka: Current status and future prospects.

BACKGROUND: Sri Lanka was acknowledged to have eliminated lymphatic filariasis (LF) as a public health problem in 2016, largely due to its success in Mass Drug Administration (MDA) to interrupt disease transmission. Analysis of the Strengths, Weaknesses, Opportunities and Threats (SWOT) of the national Morbidity Management and Disability Prevention (MMDP) program, the other pillar of the LF control program, was carried out with the objective of evaluating it and providing recommendations to optimize the use of available resources. METHODOLOGY: A situation analysis of the MMDP activities provi

Evidence-Based Nonpharmacologic Strategies for Comprehensive Pain Care: The Consortium Pain Task Force White Paper.

Medical pain management is in crisis; from the pervasiveness of pain to inadequate pain treatment, from the escalation of prescription opioids to an epidemic in addiction, diversion and overdose deaths. The rising costs of pain care and managing adverse effects of that care have prompted action from state and federal agencies including the DOD, VHA, NIH, FDA and CDC. There is pressure for pain medicine to shift away from reliance on opioids, ineffective procedures and surgeries toward comprehensive pain management that includes evidence-based nonpharmacologic options. This White Paper details

Reimagining Dental Education: Consensus Recommendations for Strengthening Faculty Pathways and Academic Career Success.

BACKGROUND: Since the beginning of the 21st century, national reports have warned that dental education is not recruiting and retaining enough oral health faculty, even as student bodies grow and become more diverse and the professions increase the emphasis on oral health's intersection with overall health. The faculty who prepare future clinicians and scientists are in short supply, and this shortage compromises the quality of dental education and ultimately impacts the quality of care delivered to our communities. OBJECTIVE: To develop consensus recommendations to increase and sustain denta

A scalable dissemination model for training faculty in mentor development and networking.

Faculty success in academia depends on effective mentoring and developmental networks, yet shortages in trained mentors, limited networking skills and inconsistent mentoring quality impact career advancement. While structured mentor development programs improve mentor competency and curriculum-based training improves networking self-efficacy, these programs are widely unavailable and/or underutilized. To address this gap, we evaluated scalable, evidence-based faculty training programs disseminated to academic institutions across the United States using the Project ECHO (Extension of Community

Workplace communication experiences of internationally recruited radiographers (IRR): A phenomenological study.

INTRODUCTION: Internationally recruited radiographers (IRR) continue to make valuable contributions to the UK healthcare. They make up 20-25% of the UK workforce, yet communication remains a significant challenge to their workplace integration and well-being. These issues contribute to stress, reduced job satisfaction, and poor interprofessional collaboration, which adversely affect patient care and safety. This study aimed to identify communication challenges faced by IRRs, their impact, coping strategies adopted, and the support they need to navigate communication challenges. METHODS: This

Rethinking compression: expanding decongestive therapy with innovative garment solutions.

BACKGROUND: Access to skilled lymphedema care is increasingly limited due to workforce shortages, financial constraints, and logistical barriers. Although compression bandaging remains the standard for the intensive phase and compression garments for maintenance, their complexity and reliance on trained clinicians pose challenges for many patients. This article explores the use of an elastic nighttime garment (ENG) and an adjustable wrap device (AWD) in both phases to optimize outcomes. The ENG used in this analysis is a specific low-profile, elastic, lymphatic alternating pressure profile-bas

OBJECTIVES: The National Pediatric Readiness Project (NPRP) offers an evidence-based framework for pediatric emergency readiness, yet implementation in general emergency departments (GEDs) remains variable. The Pediatric Emergency Care Coordinator (PECC) is the central driver of NPRP operationalization. The Improving Pediatric Acute Care Through Simulation (ImPACTS) collaborative formalized the Academic Medical Center (AMC) Partner role-experienced pediatric emergency clinicians providing structured NPRP implementation support to GED PECCs. We characterized AMC Partners' perceptions of barrier

Advancing mental health equity: a community-based adaptation of problem management plus (PM+) on a mobile health clinic.

Mental health inequities persist in underserved communities due to systemic barriers to care. Healthy Roads, a mental wellness support program developed by the mobile clinic The Family Van, adapted the World Health Organization's Problem Management Plus (PM+) curriculum to address these disparities with support from Partners In Health. PM+ is a brief, evidence-based psychological intervention designed for trained non-specialists to deliver in low-resource settings. This community case study describes the adaptation, implementation, and early lessons learned from delivering PM+ with trained com

Promise and pragmatism of AI in global-scale digital pathology: pan-cancer approaches for clinical practice.

Artificial intelligence (AI) tools for digital pathology have crossed the threshold from pilot project to clinical deployment, with regulatory approvals, commercial products and prospective trials now accumulating across multiple tumour types. However, global-scale implementation demands careful evaluation of both opportunities and challenges. Traditional histopathology faces mounting pressures including interobserver variability and escalating workloads. At the same time, molecular diagnostics have become increasingly complex, while core workflows have remained fundamentally unchanged for dec

Barriers Perceived By Palliative Care Fellowships in Training Physicians From Diverse Specialties.

CONTEXT: Hospice and Palliative Medicine (HPM) fellowships may train fellows from thirteen medical specialties, but 90% of recent graduates were from Internal Medicine, Family Medicine, or Pediatrics. Fellows from the other specialties may have distinct barriers to successful training. Little is currently known about fellowship programs' perceived readiness to train this more diverse group of palliative care specialists. OBJECTIVES: This study aims to better understand barriers to training HPM fellows from non-traditional specialties, specifically from the program director's perspective. MET

Situational analysis of health systems for ear and hearing care in the World Health Organization (WHO) Eastern Mediterranean Region: A systematic review and evidence synthesis to inform national policies and strategies.

In the Eastern Mediterranean Region (EMR), 78.1 million people experience hearing loss of any degree, with 22.1 million having disabling hearing loss, projected to reach 51.7 million by 2050. Unless global action is taken, the worldwide burden could reach over 700 million people with disabling hearing loss by 2050. This systematic review presents the first comprehensive health systems analysis of ear and hearing care (EHC) in the region. Following PRISMA guidelines, we analysed 146 articles through the WHO health systems framework to identify systemic barriers to effective EHC integration. Our

Disability prevalence, disclosure, and accommodation use in pediatric residency: results from a pilot study.

INTRODUCTION: Disability inclusion in residency is essential to building a diverse and equitable physician workforce. Recent Accreditation Council for Graduate Medical Education requirements mandate that programs maintain formal disability policies and provide reasonable accommodations. However, residents with disabilities continue to face barriers, including stigma, complex documentation processes, and limited institutional infrastructure, challenges that differ from medical school settings. Specialty-specific data are necessary to identify gaps in inclusion, especially given that national da

Predictor and subgroup analysis of somatic symptoms and emotional exhaustion among university students in Germany during the SARS-CoV-2 pandemic: a longitudinal analysis.

INTRODUCTION: The mental and physical health of university students is not just of interest to the students themselves, but to our whole society. While most research has focused on medical students, other disciplines remain underexplored. Therefore, this study aims (1) to assess the prevalence and development of emotional exhaustion and somatic symptoms from 2020 to 2021 in both cross-sectional and longitudinal samples of university students, (2) to examine subgroup differences based on demographics and study fields, and (3) to investigate predictors of symptom changes. METHODS: Data were col
