[Perioperative factors affecting the length of hospital stay after cesarean section of pregnant women with heart disease].

Objective: To explore the related factors influencing the length of hospital stay(LOS) of pregnant women with heart disease (PWHD) after cesarean section. Methods: A total of 306 patients with PWHD who underwent cesarean section from January 2012 to March 2019 were collected. Among them, 203 patients had not undergone heart surgery (uncorrected group) and 103 patients who had undergone heart surgery (corrected group) during the same period. Demographic, perioperative and postoperative data were recorded. Predictors associated with postoperative LOS were determined using univariate and multivar

[Obstetric anaesthesia in two patients with cerebral cavernomas: about two cases].

Prevalence of cerebral cavernous malformations is comprised in between 0.4 to 0.8% of the people. Anesthetic management of pregnant women suffering from this disease is often difficult. We have witnessed two cases of delivery concerning two patients suffering from a cerebral cavernous malformation, one with vaginal delivery under peridural analgesia and the other one with cesarean delivery under rachianesthesia. We recommend the realization of a magnetic resonance imaging the year before delivery. Without medullar cavernous malformations, perimedullar analgesia seems to be the preferred method

Construction and Development of an Enhanced Recovery After Surgery Program for the Surgical Management of Patients With Spinal Metastasis: A Modified Delphi Study.

OBJECTIVE: As an effective treatment for spinal metastasis (SM), ERAS protocol can significantly reduce the length of hospital stay and complications in patients. Establishing an ERAS program for perioperative care after SM surgery is a clinical problem that needs to be addressed urgently. We aimed to develop an Enhanced Recovery After Surgery (ERAS) program and Surgical Safety Checklist (SSC) that conferred clinical benefit to patients with SM and made it relatively easy to manage the condition. We believe that our findings could help establish and promote the continuous improvement of additi

An Assessment of Open Fracture Management in Hospitals in Malawi Before and Immediately After Implementing Open Fracture Guidelines.

BACKGROUND: Open fractures, a common consequence of road traffic collisions, are associated with a high risk of complications. The introduction of standard guidelines has been shown to improve patient care and reduce the risk of complications in several countries. In September 2021, the Malawi Orthopaedic Association/Arbeitsgemeinschaft für Osteosynthesefragen Alliance (MOA/AOA) guidelines and standards for open fracture management were introduced in Malawi. This study aimed to assess the management of open fractures in hospitals in Malawi, before and after implementing a training course on th

Clinical reasoning in pragmatic trial randomization: a qualitative interview study.

BACKGROUND: Pragmatic trials, because they study widely used treatments in settings of routine practice, require intensive participation from clinicians who determine whether patients can be enrolled. Clinicians are often conflicted between their therapeutic obligation to patients and their willingness to enroll them in trials in which treatments are randomly determined and thus potentially suboptimal. Refusal to enroll eligible patients can hinder trial completion and damage generalizability. In order to help evaluate and mitigate clinician refusal, this qualitative study examined how clinici

A prospective randomized study of preemptive analgesia for postoperative pain in the patients undergoing posterior lumbar interbody fusion: continuous subcutaneous morphine, continuous epidural morphine, and diclofenac sodium.

STUDY DESIGN: A prospective randomized clinical trial in preemptive analgesia for postoperative pain was conducted. OBJECTIVE: To compare the efficacy of three preemptive analgesics combined with local anesthesia: continuous subcutaneous morphine (SC), continuous epidural morphine (ED), and diclofenac sodium (DS). SUMMARY OF BACKGROUND DATA: Systemic opioids are known to be effective methods of postoperative pain control. The use of epidural morphine for postoperative analgesia has been a standard treatment in spinal surgery. Only a few studies in the literature have investigated the efficac

Comparison of modified thoracolumbar interfascial plane and anterior quadratus lumborum blocks for postoperative analgesia and recovery after lumbar transpedicular fixation surgery: a prospective randomized clinical study.

BACKGROUND: Effective postoperative analgesia after lumbar transpedicular fixation (TPF) surgery remains challenging because of extensive tissue trauma and high nociceptive burden. Ultrasound-guided fascial plane blocks have emerged as important components of multimodal analgesia in spinal surgery. This study aimed to compare the analgesic efficacy, opioid consumption, technical performance, and quality of recovery associated with the modified thoracolumbar interfascial plane (mTLIP) block versus the anterior quadratus lumborum block (QLB) in patients undergoing two- or three-level lumbar TPF

Prospective evaluation of cerebrospinal fluid concentrations of β-Endorphin as a predictor of opioid use after scheduled cesarean delivery.

BACKGROUND: Prior laboratory work indicates that lower endogenous opioid function is associated with greater analgesic and subjective responses to opioid analgesics. We evaluated whether preoperative cerebrospinal fluid (CSF) concentrations of the analgesic endogenous opioid β-Endorphin (BE) were associated with extent of opioid use after cesarean delivery (CD). METHODS: We enrolled 136 pregnant women without opioid use or chronic pain who were undergoing CD under spinal or combined spinal-epidural anesthesia. Preoperatively, participants completed validated pain measures and biospecimens were

Personalized Perioperative Opioid Strategies in Children: Focus on Methadone, Pharmacogenomics and Prevention of Persistent Postoperative Opioid Use.

Persistent postoperative opioid use (PPOU) is an emerging challenge in pediatric perioperative care, with rates as high as 4.7% in opioid-naive adolescents. Despite advances in multimodal analgesia, current protocols often fail to prevent long-term opioid exposure, particularly after high-risk surgeries such as spinal fusions. While multiple strategies exist to reduce PPOU in children, including regional anesthesia and non-opioid analgesics, this review specifically focuses on methadone and pharmacogenomic-guided opioid prescribing as promising approaches. Methadone, a long-acting opioid with

Improving Safety and Efficacy in the Surgical Management of Low-tone Neuromuscular Scoliosis: Integrated Approach With a 2-attending Surgeon Operative Team and Modified Anesthesia Protocol.

BACKGROUND: Substantially increased operative time and amount of bleeding may complicate the course of surgical treatment in neuromuscular scoliosis. A well-organized team approach is required to reduce morbidity. The aim of this study is to review our early, short-term surgical outcomes with our new integrated approach that includes a 2-attending surgeon team and modifications in the anesthesia protocol in low-tone neuromuscular scoliosis and compare with a matched cohort of our historic patients. METHODS: We retrospectively reviewed our patients with (1) neuromuscular scoliosis with collaps

Herpes Simplex Virus Vector-Mediated Gene Delivery of Poreless TRPV1 Channels Reduces Bladder Overactivity and Nociception in Rats.

Increased afferent excitability has been proposed as an important pathophysiology of interstitial cystitis/bladder pain syndrome (IC/BPS) and overactive bladder (OAB). In this study, we investigated whether herpes simplex virus (HSV) vectors encoding poreless TRPV1, in which the segment in C terminus of TRPV1 receptor is deleted, suppress bladder overactivity and pain behavior using a rat model of chemical cystitis. Replication-defective HSV vectors encoding poreless TRPV1 were injected into the bladder wall of adult female Sprague-Dawley rats. Additionally, recombinant HSV virus (vHG) vectors
