Perforated appendicitis in children: benefits of early laparoscopic surgery.

INTRODUCTION: The aim of this study was to analyse the feasibility, safety and benefits of laparoscopic appendicectomy (LA) in comparison with open appendicectomy (OA) for perforated appendicitis (PA) in children. MATERIALS AND METHODS: A retrospective analysis of all consecutive cases of PA who underwent OA or LA between July 2001 and April 2004 was done. The patient demographics, duration of symptoms and operative findings were noted and the feasibility, safety and benefits of LA were analysed with respect to postoperative recovery and complications. RESULTS: One hundred and thirty-seven c

Exploratory laparoscopy for recurrent right lower quadrant pain in a pediatric population.

Recurrent or chronic abdominal pain can be a challenging problem when conventional diagnostic studies fail to identify the cause. It is esti … dominal pain greater than 1 month in duration who ultimately underwent laparoscopic exploration and appendectomy were included in the study. Patients were excluded if an identified source of pain was discovered during preoperative workup, or if postoperative follow-up was less than 2 years. Intraoperative findings were noted, and all specimens were histologica …

Uncommon content in groin hernia sac.

Groin hernia may have very unusual sac content. Vermiform appendix, acute appendicitis, ovary, fallopian tube and urinary bladder have been rarely reported. We aimed to present our experience with these unusual hernia contents. Records of 1,950 groin hernia patients were retrospectively analyzed. Vermiform appendix was found in 0.51% and acute appendicitis was found in 0.10% of groin hernia sacs. The incidence of appendix in femoral hernia was 5%, while inguinal hernia sac contained ovary and fallopian tube in 2.9% of the cases. The incidence of groin hernias containing urinary bladder was 0.3

McBurney's point: are we missing it?

A prospective study of 100 post-evacuation barium enemas was done. Films were centered at McBurney's point, with an opaque skin marker at that point. Analysis of these revealed that in only one case (1%) was the base of the appendix at McBurney's point. In 67% it was cephalic and in 32% it was caudal to this point. The limitations of McBurney's point as an anatomical landmark should be recognized. This needs to be highlighted in teaching anatomy, especially to surgical trainees. Planning and choice of surgical incisions should be based on an understanding of these anatomical variations since M

[Routine use of open laparoscopy: technique and results in 1006 consecutive cases].

Blind insertion of the Veress needle and/or the first trocar is one of the most frequent causes of laparoscopic surgery complications. Nevertheless, the closed technique is still more preferred than the open one. The Authors retrospectively analyzed 1006 consecutive laparoscopic procedures in which Hasson's technique was routinely utilized. The overall complication rate was 2.2%, but the vast majority of complications occurred during the learning curve (6% vs. 1.9%). The Authors conclude that after the first 50 cases the open technique is a quick and safe procedure.

[Does laparoscopic treatment of abdominal infections generate bacteremias? Prospective study: 75 cases].

Laparoscopy is increasingly used in conditions complicated by peritonitis. A theoretical concern is that carbon dioxide pneumoperitoneum may increase bacteraemia. In a prospective study 90 patients were treated by laparoscopic appendicectomy. 30 of them had no histological abnormality; 30 had an acute appendicitis and 30 an acute peritonitis. 75 patients were eligible for the study. The treatment protocol (surgery-antibiotherapy) was the same for the 3 groups. All patients had blood cultures before, during and after insufflation of CO2 in the peritoneum, and bacterial examination of the operat

Appendicovesical fistula.

Appendicovesical fistula is a rare complication of unrecognized appendicitis. Only 99 previous cases have been reported in the literature. We reviewed these cases and contribute an additional one with hope that increased awareness of this entity may facilitate the correct diagnosis and avoid inappropriate management.

Post-extubation negative pressure pulmonary edema following acute upper airway obstruction in a young athlete: a case report.

BACKGROUND: Negative pressure pulmonary edema (NPPE) is a non-cardiogenic hemodynamic pulmonary edema resulting from significant inspiratory effort against obstructed airways. Although rare, it is a recognized cause of postoperative respiratory distress and can mimic differential diagnoses. Patients most predisposed to this condition are muscular or athletic individuals, due to their ability to generate significant negative intrathoracic pressure. CASE PRESENTATION: An 18-year-old male patient underwent a laparoscopic appendectomy under general anesthesia with tracheal intubation. Two minutes…

Pathological examination showed MAC with high-risk features and a positive surgical margin. Incidence, risk factors and preventive strategies for post-operative recurrence of Crohn's disease: A retrospective propensity-matched cohort analysis.

BACKGROUND: Crohn's disease (CD) is characterized by recurrence following surgical resection. We assessed the post-operative recurrence rate … regression model that included age, sex, smoking, disease behavior and location, surgical approach, appendectomy history, prior anti-tubercular therapy and pre-operative prophylaxis (caliper = 0.20 SD of the logit; no replacement). Recurrence-free survival was analyzed by Kaplan-Meier curves, log-rank test and adjusted hazard ratios from multi-variable Cox reg …

Exploring the role of trained surgical care nurses in cricothyrotomy and other emergency procedures: a systematic review and meta-analysis.

BACKGROUND: There is a severe shortage of healthcare professionals, emphasized in a stark manner by the recent COVID-19 pandemic, where the mortality rate was primarily a consequence of medical professionals lacking the technical know-how for conducting specialized procedures. Therefore, this systematic review and meta-analysis aimed to evaluate the success rates of nurse-performed emergency surgeries, focusing on trauma care (e.g., cricothyrotomy), rural obstetric emergencies (e.g., caesarean section, hysterectomy), and general procedures (e.g., laparotomy, appendectomy). METHODS: A systemat…

Treatment Strategies and Outcomes in Pediatric Patients With Immune Thrombocytopenia Undergoing Surgical Procedures: A Multicenter Study.

INTRODUCTION: Children with immune thrombocytopenia (ITP) may have an increased risk of perioperative bleeding. However, current pediatric ITP guidelines do not address this management setting. We aimed to describe perioperative management and outcomes in pediatric patients with ITP by platelet count, type of surgery, and ITP-directed treatment strategies. METHODS: We conducted a retrospective analysis of patients with ITP ages 0-24 years who underwent tooth extraction, tonsillectomy and adenoidectomy, appendectomy, and/or splenectomy at six centers in the United States and Canada between 201…

A Simple Preoperative Scoring System for Risk Stratification of Complicated Appendicitis in Children.

Background/Objectives: Preoperative discrimination of complicated appendicitis in children remains challenging but crucial for timely surgical decision-making and perioperative management. We aimed to develop a simple, clinically applicable scoring system for preoperative risk stratification of complicated appendicitis in children. Methods: This retrospective single-center study included children aged 5 to <16 years who underwent emergency appendectomy between 2015 and 2024. Complicated appendicitis was defined as gangrene, perforation, or intra-abdominal abscesses on intraoperative or patholo…
