Can antibiotic impregnated cement nail achieve both infection control and bony union in infected diaphyseal femoral non-unions?

BACKGROUND: Infected non-union is complex and debilitating disorder affecting orthopaedic surgeon and patient in terms of cost and time. Many methods are described in the literature for treatment of infected non-union. Local high concentration of antibiotic and mechanical stability of antibiotic cement impregnated intramedullary nail (ACIIN) proves cost and time effective. Recently it was suggested that ACIIN can achieve both union and infection control in infected non-unions with bone gap less than 4cm. The aim of our study was to investigate this hypothesis and study the outcome of antibioti

Reamed versus minimally reamed nailing: a prospectively randomised study of 100 patients with closed fractures of the tibia.

It is generally accepted that in tibial fractures the results of reamed intramedullary nailing are better than those of unreamed. However, it is not known whether the clinical effects of reaming are cumulative or if minimal reaming would induce the same beneficial effects as more extensive reaming. This international multicentre study has investigated the effects of different degrees of reaming. 100 patients with closed diaphyseal tibial fractures were prospectively randomised in two centres. Method of treatment was reamed nailing up to 12 mm inserting an 11 mm tibial nail (n: 50), and minimal

Osteogenesis imperfecta and hyperplastic callus formation in a family: a report of three cases and a review of the literature.

Osteogenesis imperfecta is one of the most common groups of inherited disorders of connective tissue. Hyperplastic callus formation in patients with osteogenesis imperfecta after fracture or surgery is a rare occurrence that has often been misdiagnosed as osteosarcoma. Previous series reported that hyperplastic callus formation is more often present in osteogenesis imperfecta male patients, with white sclerae, and a negative family history of the disorder. This is the first time that this complication has been presented in three female siblings, with a positive family history of osteogenesis i

Distal metaphyseal fractures of the tibia with minimal involvement of the ankle. Classification and treatment by locked intramedullary nailing.

We reviewed 63 patients with fractures of the distal tibial metaphysis, with or without minimally displaced extension into the ankle joint. The fractures had been caused by two distinct mechanisms, either a direct bending force or a twisting injury. This influenced the pattern of the fracture and its time to union. All fractures were managed by statically locked intramedullary nailing, with some modifications of the procedure used for diaphyseal fractures. There were few intra-operative complications. At a mean of 46 months, all but five patients had a satisfactory functional outcome. The poor

Outcomes of Metaphyseal Versus Diaphyseal Stems in Conversion Total Hip Arthroplasty After Intramedullary Nailing.

BACKGROUND: Diaphyseal-engaging stems are conventionally used in conversion total hip arthroplasty (cTHA) after prior intramedullary nailing (IMN) to bypass screw holes and previously instrumented meta-diaphyses. However, it remains unclear if cementless metaphyseal or standard cemented stems can also provide satisfactory fixation in this scenario. This study compared the early outcomes of cTHA using three femoral stem designs: metaphyseal cementless, diaphyseal cementless, and standard cemented. METHODS: A retrospective review of 80 patients who underwent cTHA after prior IMN at a single aca

Combination of radiofrequency ablation and intramedullary nailing for the treatment of femoral metastases: single-center, retrospective observational study.

BACKGROUND: Radiofrequency ablation (RFA) has gained attention as a palliative treatment for bone metastases, providing pain relief and local tumor control. While its use for axial lesions is well documented, its application in long bones remains limited due to concerns about post-ablation fractures. These risks may be mitigated through the combination of RFA with prophylactic intramedullary nailing (IMN). METHODS: Five consecutive patients with femoral metastases who underwent combined RFA and IMN, performed either as a single-stage procedure under intraoperative fluoroscopic guidance or as

Heterotopic ossification around the knee after tibial nailing and ipsilateral antegrade and retrograde femoral nailing in the treatment of floating knee injuries.

PURPOSE: Floating knee injuries are relatively uncommon injuries. We report the prevalence, location, and severity of heterotopic ossification (HO) around the knee in patients treated with antegrade tibial intramedullary nailing and ipsilateral antegrade versus retrograde femoral intramedullary nailing as well as how the severity of HO around the knee affects knee range of motion (ROM). METHODS: From 2004 to 2014, 26 floating knee injuries were included. Radiographs were reviewed to determine presence, location, and severity of HO. Post-operative knee ROM was determined. RESULTS: A significa

The efficacy of single-stage open intramedullary nailing of neglected femur fractures.

BACKGROUND: Neglected femur fractures are not rare in the developing world. Treatment options include single-stage open reduction and intramedullary nailing, or open release, skeletal traction, and then second-stage open intramedullary nailing, with bone grafting. Single-stage procedures have the potential advantage of avoiding neurovascular complications secondary to acute lengthening, but they require a second operation, with potentially increased resource use and infection risk. QUESTIONS/PURPOSES: We sought to determine the (1) likelihood of union, (2) complications and reoperations, and

Benefits and harms of locking plate osteosynthesis in intraarticular (OTA Type C) fractures of the proximal humerus: a systematic review.

INTRODUCTION: Locking plate osteosynthesis of proximal humeral fractures are widely recommended and used, even in complex intraarticular fracture patterns such as AO/OTA Type C fractures. We systematically reviewed clinical studies assessing the benefits and harms of osteosynthesis with angle stable plates in AO/OTA Type C fractures of the proximal humerus. METHODS: We conducted an iterative search in PubMed, Embase, Cochrane Library, Web of Science, Cinahl, and PEDro in all languages from 1999 to November 2010. Eligible studies should study the outcome for Type C fractures after primary oste

Anterograde femoral nailing with a reamed interlocking titanium alloy nail.

Intramedullary nailing has become the gold standard for treatment of femoral diaphyseal fractures. Between March 1995 and December 1998 we performed 40 intramedullary nailings using the ACE femoral nail (De Puy). The patients were followed for an average of 27.9 months (range: 6-54 months). The mean age was 33.2 years (range: 17-87 years); the sex distribution was 33 males and 7 females. All fractures were unilateral (right 18, left 22). Most of the fractures were caused by traffic accidents (35), the others originated from sports, work and gunshot (1). Thirty-five fractures were closed and 5

What is the Reason for the Trend Shift from Dynamic Hip Screw to Cephalomedullary Nailing for the Treatment of Intertrochanteric Fractures? A Comprehensive Retrospective Study in a Single Tertiary Referral Hospital.

BACKGROUND: The prevalence of intertrochanteric fractures is increasing with the aging population. Two surgical treatments, cephalomedullary nailing (CMN) and dynamic hip screw (DHS), have been widely utilized; however, recent trends indicate growing preference for CMN. While several studies have compared these 2 surgical methods, there has been limited comprehensive analysis with a substantial sample size from a single medical center aimed at minimizing bias. METHODS: This retrospective study was conducted at a single tertiary academic hospital, analyzing data from patients who underwent sur

Custom-Made Antibiotic Cement-Coated Nail for the Treatment of Infected Bone Defect.

BACKGROUND: Longbone infected bone defect remains a great challenge due to multiple surgeries, long-term treatment duration, and uncertain prognosis. Treatment principles include eradication/debridement, stabilization, and antibiotic administration. An antibiotic cement-coated nail has shown great prospects due to both local antibiotic elution and stabilization of bone defects. However, the current fabrication technique remains to be improved. METHODS: For the first time, we described a new method for custom-made cement-coated nail fabrication based on a 3D printing technique. A retrospective

Intraoperative Issues and Clinical and Radiographic Outcomes of Femur Fractures Treated With Flexible Nails: A Comparison of Cases Utilizing Skeletal Traction and a Traction Table to Cases Using Manual Traction Only.

INTRODUCTION: The use of the orthopaedic traction table (OTT) during elastic stable intramedullary nailing (ESIN) in the management of displaced diaphyseal femur fractures (DFFs) is still debated. In most centers, children with displaced DFF are treated using an OTT. In some other institutions, however, fracture reduction and stabilization by ESIN are performed on a radiolucent table without an OTT. The aim of this study was to evaluate the clinical and radiologic outcome of children with displaced DFF managed by ESIN with and without the use of an OTT. METHODS: Charts and radiographs were re
