[Radiological features of the rectal manifestations of myelofibrosis(author's transl)].

In addition to the well known changes in bone structure, myelofibrosis can result in extramedullary tumour formation; in the gastr-intestinal tract, proliferation of reticulum can lead to fibrotic changes. This results in appearances similar to linitis plastica or Crohn's disease. In the discussion, the authors refer to tumours in other organs which have been described in the literature and which can be demonstrated radiologically.

The Risk Factors for Complications Following Intestinal Stoma Reversal.

Background and objective  The restoration of bowel continuity after temporary intestinal stoma formation is a routine general surgical procedure. However, stoma reversal is associated with significant postoperative morbidity and mortality. Surgeons may face challenges such as dense adhesions, iatrogenic bowel injury, or even procedure abandonment, as well as postoperative complications such as surgical site infection (SSI), anastomotic leak, intra-abdominal sepsis, and death. This study aimed to identify common complications and their risk factors to facilitate strategies for optimizing surgic

[Reoperation rate and related factors in patients with Crohn's disease].

Objective: To analyze the cumulative reoperation rate of postoperative Crohn's disease (CD) patients and investigate the operation reasons a … bstruction accounted for the highest proportion in the primary operation (26.9%, 43/160), while the intestinal fistula was the most common reason for reoperation (42.5%, 17/40). After the primary surgery, the cumulative reoperation rates at 1, 3, 5 and 10 years were 5.9% (9 cases), 12.3% (17 cases), 21.8% (25 cases) and 37.6% (34 cases), respectively. The ten years …

Retroperitoneal urinoma presenting as a posterior knee sinus following emphysematous pyelitis.

A woman in her late 40s presented with abdominal sepsis secondary to emphysematous pyelitis complicated by a retroperitoneal urinoma. Despite initial antimicrobial therapy, percutaneous drainage and ureteric stenting, she developed persistent thigh pain and a posterior knee sinus. Repeat cross-sectional imaging demonstrated caudal spread of infected retroperitoneal fluid and gas along the iliopsoas and fascial planes into the thigh. Subsequent imaging identified a fistulous communication involving the colon. The exact sequence of fistula formation could not be determined, and causation between

Appendico-Sigmoid Fistula as a Rare Complication of Chronic or Recurrent Appendicitis: A Case Report.

BACKGROUND Appendico-colic fistula is a rare condition characterized by an abnormal communication between the appendix and colon, most commonly involving the sigmoid colon (ie, appendico-sigmoid fistula) because of their close anatomical proximity. It is usually associated with chronic or recurrent appendicitis, diverticular disease, inflammatory bowel disease, or malignancy. Preoperative diagnosis is challenging due to nonspecific clinical and radiologic findings. CASE REPORT A 46-year-old woman presented with a 3-month history of lower abdominal pain, which had worsened over the preceding 15

Spatial immune profiling of Crohn's disease fistula carcinomas-defining a distinct cancer subtype.

BACKGROUND AND AIMS: Fistula formation is a common and debilitating complication in Crohn's disease (CD). CD-associated fistula carcinomas (Fi-Cas), though rare, pose diagnostic and prognostic challenges. This study aims to identify disease-defining immune cell subsets in CD-associated Fi-Cas. METHODS: The study included tissue samples from 10 CD patients with Fi-Cas, 7 with CD-associated fistulas, and 6 with sporadic colorectal cancer (CRC). The main tumor (MT), infiltration front, and non-involved areas were analyzed in tumor samples. A 36-marker panel was employed to define the immune land

Indicators for early surgery in patients with intra-abdominal fistulizing Crohn's disease.

BACKGROUND: The management of intra-abdominal fistulizing Crohn's disease involves surgical resection and biologic therapy. The criteria for choosing one therapy over the other are debated. OBJECTIVES: Identify factors influencing the choice of early surgical intervention over biologic therapy. DESIGN: Retrospective. SETTING: Single center, tertiary training and research hospital. PATIENTS AND METHODS: We analyzed adult patients with Crohn's disease who had intra-abdominal fistulas and were followed for three years. Baseline data were collected from medical records, and imaging studies ass

Catastrophic Abdominal Wall After Repair of Enterocutaneous Fistula: A Case Study.

BACKGROUND: Repair of an enterocutaneous fistula (ECF) is challenging, particularly when complications occur. This case describes the use of negative pressure wound therapy (NPWT) and microadhesive dressings with polyabsorbent fibers and an acrylic core, with and without lipidocolloid and nano-oligosaccharide factors, in the management of a patient with a large abdominal wound and ECF. CASE: An 84-year-old woman underwent abdominoperineal resection with colostomy, hysterectomy, and subsequent chemotherapy and radiotherapy for colorectal cancer. She experienced complications, ultimately result

Spontaneous renoalimentary fistula as a complication of upper tract urothelial carcinoma: unknown complication of a rare disease.

Fistulae of the upper urinary tract with the alimentary tract are rare. Most cases of renoalimentary fistulae are secondary to penetrating trauma, which may be iatrogenic or due to locally invasive benign infective processes or complicated nephrolithiasis, or following surgical procedures. Spontaneous renoalimentary fistulae developing secondary to locally advanced malignancies, namely renal cell carcinoma, are very rare, and unknown due to upper tract transitional cell carcinoma. We present a case where a 60-year-old man, presented with clinical symptoms suggestive of upper gastrointestinal t

Meta-analysis of invagination and duct-to-mucosa pancreaticojejunostomy after pancreaticoduodenectomy: An update.

OBJECTIVE: Duct-to-mucosa and invagination are two commonly used techniques of pancreaticojejunostomy (PJ) after pancreaticoduodenectomy. Previously, we conducted a systematic review comparing the safety and efficacy of the two PJ techniques. Here, we added new evidence and updated our previous conclusion. METHODS: We systematically searched multiple databases and included randomized controlled trials (RCTs) comparing duct-to-mucosa and invagination techniques of PJ. The quality of evidence was assessed using Jadad score, and reporting bias was evaluated using funnel plots. Meta-analysis was

Incisional Hernia Classification Predicts Wound Complications Two Years after Repair.

Classification of ventral hernias (VHs) into categories that impact surgical outcome is not well defined. The European Hernia Society (EHS) classification divides ventral incisional hernias by midline or lateral location. This study aimed to determine whether EHS classification is associated with wound complications after VH repair, indicated by surgical site occurrences (SSOs). A retrospective cohort study of patients who underwent VH repair at a tertiary referral center between July 1, 2005 and May 30, 2012, was performed. EHS classification, comorbidities, and operative details were determi

The use of negative-pressure wound therapy to manage enteroatmospheric fistulae in two patients with large abdominal wounds.

Enteric fistulae are a relatively common complication of bowel surgery or in surgery where the bowel has been exposed. Fistulae can present a significant threat to patients' well-being. Changes in surgical techniques and in particular the rise in damage control surgery for emergency patients have led to an increase in open abdominal wounds. The presence of an enteroatmospheric fistula on the surface of a wound can cause a number of distressing symptoms/issues for the patient whilst providing a significant challenge for the clinician. The loss of fluid, proteins and electrolytes will place the
