Retinal vascular occlusion after vitrectomy with retrobulbar anesthesia-observational case series and survey of literature.

BACKGROUND: Severe postoperative loss of vision has been occasionally reported as a rare complication of retrobulbar anesthesia, and several possible causes have been proposed in the literature. In this work, our own and other investigators' experiences with these complications are surveyed with a view to identifying its pathophysiology. PATIENTS: This observational case series refers to six patients who presented during a 3-month period with occlusion of either the central artery itself (n = 3) or a branch thereof (n = 3) 2-14 days after uneventful vitreoretinal surgery following retrobulbar

Arteritic anterior ischemic optic neuropathy with positive myeloperoxidase antineutrophil cytoplasmic antibody.

BACKGROUND: Myeloperoxidase antineutrophil cytoplasmic antibody (MPO-ANCA) is related to smallvessel vasculitis. There have been some reports of optic nerve involvements with increased values of MPO-ANCA. We report two cases of anterior ischemic optic neuropathy (AION) in which ANCA-associated vasculitis was suspected to be responsible for the pathogenesis. CASES: A 66-year-old man and a 72-year-old man had ocular symptoms of AION in both eyes with positive MPO-ANCA. OBSERVATIONS: Both patients showed high erythrocyte sedimentation rate, C-reactive protein, and MPOANCA values at first. Tempo

[Chorioretinal anastomosis as unfavourable prognostic factor during photodynamic therapy].

BACKGROUND: The aim of this study was the documentation of chorioretinal anastomosis in neovascular age-related macular degeneration (AMD) and correlation with functional and angiographic results following photodynamic therapy (PDT). METHOD: A total of 100 patients presenting with neovascular AMD and indication for PDT based on the presence of predominantly classic choroidal neovascularization (CNV), underwent ophthalmoscopic and angiographic screening for chorioretinal anastomosis. Conventional PDT using verteporfin was performed according to the recommended standard procedure. The pre- and

Vitreous haemorrhage and other ocular complications of a persistent hyaloid artery.

PURPOSE: To report ocular complications of a persistent hyaloid artery. METHODS: We studied eight patients with persistent hyaloid artery. RESULTS: Seven patients showed strabismus and very low visual acuity (< or = 0.12) of one eye. Despite correction of refractive errors, cataract surgery and occlusion therapy for amblyopia, visual acuity had not improved in these cases. Four patients showed nystagmus. Four had progression of unilateral cataract. In two cases, a 24-year-old women and a 4-months-old boy, a vitreous haemorrhage had occurred due to rupture of a hyaloid artery, in the woman's

Progressive Retinal and Cerebral Arteriovenous Malformations in Wyburn-Mason Syndrome: A 5-Year Follow-Up Case Report.

This report presents a rare case of Wyburn-Mason syndrome (WMS) with documented progression of both retinal and cerebral arteriovenous malformations (AVMs) over 5 years, emphasizing the clinical significance of ocular changes that may be associated with cerebral involvement. A 6-year-old girl initially presented with unilateral blurred vision and was diagnosed with WMS due to a retinal AVMs in her left eye. Initial brain magnetic resonance imaging (MRI) showed no intracranial AVMs. At routine follow-up, she had developed headaches and nausea after 4 years, leading to diagnosis of a cerebral AV

Retinal microvascular abnormalities and age-related hearing loss: the Blue Mountains hearing study.

OBJECTIVE: Microvascular disease may contribute to hearing loss. We tested this hypothesis by examining the relation of retinal microvascular abnormalities and hearing loss. METHODS: This was a cross-sectional, population-based study. We performed retinal photography and pure-tone audiometry on 1511 older individuals (ages 54+ years) from the Blue Mountains Eye Study population. Examination of the retinal microvasculature provides a noninvasive means of assessing systemic microvascular changes associated with cardiovascular disease, especially since reliable methods of quantifying retinal mic

An angiopoietin-2 vaccine improves arteriovenous malformation pathology in hereditary hemorrhagic telangiectasia mice.

Hereditary hemorrhagic telangiectasia (HHT) is a genetic vascular disorder that causes systemic arteriovenous malformations (AVMs) associated with severe complications. Angiopoietin-2 (ANG2) has been identified as a consistently upregulated secreted protein across various HHT models, and neutralizing ANG2 reduces AVMs in mice. Therefore, ANG2 has emerged as a potential target for HHT treatment. Here, we report the development of a peptide vaccine (ANG2-P3:cross-reacting material 197 [CRM197]) that selectively targets ANG2 over ANG1 and tested its effectiveness in decreasing retinal AVMs in neo

Joint position paper on anterior ischemic optic neuropathy (AION). Vascular Risk Group and Systemic Autoimmune Diseases Group (GEAS) of the Spanish Society of Internal Medicine (SEMI) together with the Spanish Retina and Vitreous Society (SERV).

Anterior ischemic optic neuropathy (AION) is one of the leading causes of sudden, painless, monocular vision loss in individuals over 50 years of age. Two forms can be distinguished, with clearly different clinical, diagnostic, and therapeutic implications: the arteritic form, most commonly associated with giant cell arteritis (GCA), which constitutes a medical and ophthalmological emergency; and the non-arteritic form, which is far more frequent and for which no specific effective treatment is currently available. Despite its clinical relevance and the frequent coexistence of vascular risk fa

Retrospective treatment comparisons in patients with Susac syndrome.

INTRODUCTION: Susac syndrome is a rare disease affecting arterioles in the brain, retina and inner ear. There is uncertainty about a clinical benefit of adding intravenous immunoglobulins or immunosuppressants to corticosteroid maintenance treatment. METHODS: This restrospective study identified adult patients from case records who met crite-ria for probable or definite Susac syndrome. Patients with a sole manifestation of arterial wall hyperintensity (AWH) on retinal fluorescein angiography (RFA) were also included. All clinically symptomatic disease activity and clinically asymptomatic AWH

Relevance of obesity-related organ damage and metabolic syndrome classification in cardiovascular and renal risk stratification in patients with essential hypertension.

INTRODUCTION: Hypertension is a relevant cardiovascular comorbidity. Adipose tissue represents a metabolically active tissue involved in the regulation of blood pressure and metabolic alterations. In recent decades, several classifications for the metabolic syndrome (MS) have been proposed. Recently, a new syndrome called the "Cardiovascular-kidney-metabolic" (CKM) syndrome was identified, to determine patients at high cardiovascular and metabolic risk. The aim of the study was to compare different classifications in a large population of hypertensive patients. MATERIALS AND METHODS: Between

Differences in Systemic Pulse Waveform Between Individuals With Glaucoma, Glaucoma Suspects, and Healthy Controls.

PURPOSE: It has been hypothesized that compromised ocular circulation in glaucoma may be concomitant of systemic changes. The purpose of this study is to test whether systemic blood flow pulse waveform patterns differ between individuals with glaucoma (GL), glaucoma suspects (GLS), and normal healthy controls (HC). METHODS: The study included 35 bilateral GL, 67 bilateral GLS, 29 individuals with unilateral GL who were considered GLS in the other eye, and 44 healthy controls. Systemic pulsatile blood pressure waveforms were recorded using a finger cuff. A continuous 200 Hz plethysmography rec

Choriocapillaris flow deficit is associated with disease duration in type 2 diabetic patients without retinopathy: a cross-sectional study.

BACKGROUND: Diabetes mellitus (DM) causes microvascular damage due to long-term hyperglycemia, even before the onset of retinal changes. We aimed to investigate the association between optical coherence tomography angiography (OCTA) metrics and disease duration in type 2 diabetic patients without retinopathy. METHODS: Eighty-two eyes of 82 type 2 diabetic patients without diabetic retinopathy (DR) were included. Choriocapillaris flow deficit (CC FD%), vessel density (VD), vessel length density (VLD) in the superficial (SVP) and deep vascular plexus (DVP) were calculated and compared between d

Inhibition of bradykinin in SARS-CoV-2 infection: a randomised, double-blind trial of icatibant compared with placebo (ICASARS).

UNLABELLED: SARS-CoV-2 binds to ACE2 receptors and enters cells. The symptoms are cough, breathlessness, loss of taste/smell and X-ray evidence of infiltrates on chest imaging initially caused by oedema, and subsequently by a lymphocytic pneumonitis. Coagulopathy, thrombosis and hypotension occur. Worse disease occurs with age, obesity, ischaemic heart disease, hypertension and diabetes. These features may be due to abnormal activation of the contact system. This triggers coagulation and the kallikrein-kinin system, leading to accumulation of bradykinin and its derivatives, which act on recepto
