Tibiotalocalcaneal Arthrodesis With the Hindfoot Arthrodesis Nail: A Prospective Consecutive Series From a Single Institution.

Tibiotalocalcaneal arthrodesis (TTCA) is a salvage procedure. We report a series of 20 patients who underwent TTCA using an intramedullary nail. Of the 20 patients, 7 (35%) had diabetes mellitus. The patient experiences and outcomes were analyzed. Their mean age was 61.1 (range 39 to 78) years. The minimum follow-up period was 13 (mean 28, range 13 to 49) months. Surgical indications included diabetic Charcot arthropathy in 7 (35%), hindfoot osteoarthritis in 10 (50%), and severe equinovarus deformity in 3 (15%). A calcaneal spiral blade was used in 2 patients (10%). Significant improvements (

Does Mobility of the Elderly with Hip Fractures Improve at One Year Following Surgery? A 5-year Prospective Survey.

BACKGROUND: Hip fracture and its consequences are one of the major causes of morbidity and disability in the elderly. The purpose of this study was to collect epidemiologic data and assess the daily activities and walking ability of patients 3 months and 1 year following surgery because of a hip fracture. MATERIAL AND METHODS: We prospectively obtained data of all consecutive patients more than 50 years of age with any type of hip fracture from 2008 to 2013. After excluding pathologic fractures, hip fracture dislocations, non-surgically treated cases, and dead patients, functional status of t

Closed Reduction and Percutaneous Pinning of Displaced Pediatric Lateral Condyle Fractures of the Humerus: A Cohort Study.

BACKGROUND: In treating pediatric lateral condyle fractures (LCFs) of the humerus, closed reduction and percutaneous pinning (CRPP) is an attractive alternative to open reduction and internal fixation (ORIF) because of the potential decrease in tissue injury, shorter surgical times, and faster functional recovery. However, there is limited information available regarding its outcome. METHODS: The data on 191 pediatric LCFs [163 fractures (85%) treated with ORIF (group 1) and 28 fractures (15%) treated with CRPP (group 2)], who were followed for over 12 weeks, was included in the present analy

Causes and treatments of lag screw's cut out after intramedullary nailing osteosinthesis for trochanteric fractures.

Background. Superior cut-out of a lag screw remains a serious complication in the treatment of trochanteric or subtrochanteric fractures and it is related to many factors: the type of fracture, osteoporosis and the stability of fracture reduction. Little is known about the outcome after revision surgery for complications of the gamma nail. We assessed the outcome in patients who had revision surgery because of lag screw's cut out after gamma nailing for a trochanteric fracture.Material and Method. We present a study of 20 consecutive patients who underwent treatment after 20 cut-out of the lag

TRIGEN INTERTAN intramedullary nail versus sliding hip screw: a prospective, randomized multicenter study on pain, function, and complications in 684 patients with an intertrochanteric or subtrochanteric fracture and one year of follow-up.

BACKGROUND: Both intramedullary nails and sliding hip screws are used with good results in the treatment of intertrochanteric and subtrochanteric fractures. The aim of our study was to assess whether use of the TRIGEN INTERTAN nail, as compared with a sliding hip screw, resulted in less postoperative pain, improved functional mobility, and reduced surgical complication rates for patients with an intertrochanteric or subtrochanteric fracture. METHODS: In a prospective, randomized multicenter study, 684 elderly patients were treated with the INTERTAN nail or with a sliding hip screw with or wit

Sequential treatment with teriparatide and strontium ranelate in a postmenopausal woman with atypical femoral fractures after long-term bisphosphonate administration.

OBJECTIVE: Despite the existence of numerous case series, no evidenced-based medical management for atypical fractures associated with bisphosphonate (BP) treatment has been established. DESIGN: We report the outcome of teriparatide (TRP) administration followed by strontium ranelate (SR) in a woman with a complete and an incomplete contralateral atypical fracture of the femoral diaphysis (AFF) associated with BP treatment. The spontaneous complete AFF was managed with intramedullary nailing, discontinuation of BP and initiation TRP. RESULTS: Eleven months later, she suffered a contralateral

Fractures in large-segment allografts.

Sixteen of thirty-five large-segment allografts that had been implanted after resection for neoplastic conditions, and had been followed for a minimum of thirty-six months, were found to have fractured at a mean of twenty-six months after the implantation. Thirteen of the fractures were treated operatively, and we found a lack of vascularization and soft-tissue attachments to the graft at the fracture site. For seven fractured grafts, there were radiographic and clinical signs of union with the host bone. Eight of the sixteen grafts that had fractured were salvaged with one or more autogenous

Intraoperative Burden of Flexible Intramedullary Nailing and Spica Casting for Femur Fractures in Young Children.

BACKGROUND: Spica casting (Spica) remains the widely accepted treatment of closed femoral shaft fractures in young children aged 6 months to 5 years. In some centers, there has been a recent trend towards surgical fixation of these fractures with flexible intramedullary nails (FINs). Despite numerous studies evaluating both Spica and FIN treatment of femoral shaft fractures, there remains a paucity of data regarding patient burden during the intraoperative period. The purpose of this study was to compare the intraoperative burden, defined as anesthetic exposure, fluoroscopic duration, and radi

The road to the Pearl-String Technique: lessons learned from early IMT modifications using RIA autograft and cancellous allograft granules.

BACKGROUND: The induced membrane technique (IMT) is widely used for reconstruction of large bone defects; however, reported outcomes vary considerably among technical modifications. One potential mechanism impairing consolidation is graft sedimentation, particularly when low-density cancellous autograft harvested using the reamer-irrigator-aspirator (RIA) is applied. This study evaluates a standardized modification of the IMT and explores mechanical factors potentially affecting graft consolidation. METHODS: In this retrospective single-center study, 28 patients were treated following eradica

Patellar Malalignment Combined With Deformed Valgus Knee Treated by Distal Femoral Varus Osteotomy and Retrograde Intramedullary Nailing.

OBJECTIVES: Patellar malalignment (PM) is common and a deformed valgus knee is an uncommon contributing factor. A deformed valgus knee can magnify the traction forces laterally during knee extension, consequently leading to PM. Treatment of combined disorders without correction of the deformed valgus knee is often less effective. The objective of this study was to assess the possibility of using the retrograde intramedullary nailing technique with some modifications for the treatment of both disorders. METHODS: From January 2011 to December 2020, 36 consecutive adult patients with 36 combined

Aneurysmal bone cyst healing response with intramedullary lengthening nail.

We report the treatment process of a pediatric patient with deformity and shortening in the arm after a recurrent aggressive aneurysmal bone cyst (ABC) in the proximal humerus. The patient was treated with curettage of the lesion and lengthening on an intramedullary nail following an osteotomy just distal to the ABC. The period of lengthening was approximately 50 days. At the end of the treatment the lengthening goal was achieved without any neurovascular complication. There was a minimal loss in shoulder hyperabduction due to the deformity of the humeral head.

Bifocal Lengthening with Monorail External Frame for Severe Shortening of the Femur: A Case Report.

AIM AND BACKGROUND: Femoral lengthening using a monorail external frame often involves creating two simultaneous distraction sites. This approach allows for a better distribution of the osteogenesis process and can reduce the overall treatment duration, particularly in cases of severe shortening. However, the literature on bifocal femoral lengthening is limited, with reported cases rarely exceeding 10 cm of lengthening. This case report aims to describe the treatment and outcome of a patient who underwent bifocal lengthening with a monorail external frame for severe femoral shortening. CASE D

Restoration of Ankle Stability After Complete Traumatic Loss of the Distal Fibula Using Peroneus Brevis Tendon Reconstruction Without Osseous Reconstruction.

The distal fibula plays a key role in ankle stability by forming the ankle mortise and serving as the attachment site for the lateral ligament complex. Traumatic loss of the distal fibula is extremely rare and is associated with ankle instability, valgus deformity, and eventual arthritis if not addressed. We report the case of a 34-year-old female who sustained a high-energy open injury with complete loss of the distal fibula, disruption of the lateral ligament complex, and a comminuted distal tibial fracture following a road traffic accident. Initial management included emergency debridement,

Surgical treatment of metastatic bone disease of the distal extremities.

Metastatic bone disease of the distal extremities, also known as acrometastasis, is very rare. Thus, there is very limited information regarding the clinical manifestations and methods of surgical treatment. The current available literature shows that acrometastases are often encountered in the context of advanced disease and are thus associated with poor patient survival. As metastatic bone disease is generally uncurable, the goal of surgical treatment is to provide the patient with good function with as few complications as possible. In this article, we discuss the clinical manifestation of
