Fully implantable hearing aid in the incudostapedial joint gap.

A fully implantable hearing aid is introduced which is a combined sensor-actuator-transducer designed for insertion into the incudostapedial joint gap (ISJ). The active elements each consist of a thin titanium membrane with an applied piezoelectric single crystal. The effectiveness of the operating principle is verified in a temporal bone study. We also take a closer look at the influence of an implantation-induced increase in middle ear stiffness on the transducer's output. An assembly of the transducer with 1 mm thickness is built and inserted into six temporal bones. At this thickness, the

Selective Auditory Attention Engages Distinct Neural Strategies in Simulated Electric and Acoustic Stimulation Within and Across Ears.

OBJECTIVES: This study aimed to investigate whether electric-acoustic stimulation (EAS) and bimodal stimulation (BS) rely on similar or distinct neural mechanisms to support attention-driven speech comprehension in noisy environments. DESIGN: This cross-sectional study used electroencephalography to measure cortical multidimensional neural responses and compared them between two groups of young adult normal-hearing listeners (15 subjects per group) exposed to simulated EAS or BS listening conditions in a two-talker competing scene. The simulated stimuli were generated using a vocoder to mimic

Speech-Like Vibrotactile Stimulation Is Associated With Enhanced Cortical Activations in Single-Sided Deafness: An fMRI Study.

Audiotactile cues can enhance speech perception in noise, yet the neural mechanisms underlying vibrotactile speech processing remain poorly understood, particularly in individuals with single-sided deafness (SSD). The present study investigated how adults with acquired SSD process a complex amplitude-modulated speech-derived vibrotactile stimulus compared to simple periodic vibration, and how these responses differ from normal-hearing (NH) controls. Twenty adults with acquired SSD and 20 age- and sex-matched NH controls underwent functional magnetic resonance imaging (fMRI) during vibrotactile

Factors Associated With Hearing-Related Quality of Life in Children and Adolescents With Hearing Loss.

OBJECTIVE: As the impact of hearing loss extends beyond auditory function, patient-reported outcome measures (PROMs) measuring quality of life (QOL) are increasingly incorporated in the management and follow-up of children with hearing loss. The Hearing Environments and Reflection on Quality of Life (HEAR-QL) is a hearing-specific QOL tool, providing insight into the impact of hearing loss on daily life. This study aims to identify factors associated with hearing-related QOL in children and adolescents with hearing loss. METHOD: Clinical data of 136 subjects (84 children and 52 adolescents) a

Transcutaneous Cervical Vagus Nerve Stimulation Improves Speech Comprehension in Noise: A Crossover, Placebo-Controlled Study.

BACKGROUND: Speech comprehension in noisy environments remains a significant challenge, even among individuals with clinically normal hearing and users of hearing aids and cochlear implants. Although conventional assistive hearing devices address limitations in the auditory periphery, they do not directly enhance the brain's capacity to segregate speech from background noise. Because tonic vagus nerve stimulation (VNS) has shown potential for rapidly improving central sensory processing, this study investigated whether tonic transcutaneous cervical VNS (tcVNS) can enhance speech-in-noise intel

Education Intervention on 'Hearing Impairment - Its Assessment and Management' among Medical Practitioners in Sikkim, India.

BACKGROUND: Health care providers must be aware of the choices in aural rehabilitation methods and assessment procedures available presently to those with hearing impairment in order to facilitate early identification and intervention and ensure quality of care to those hearing impaired individuals. The referrals from the medical fraternities for the audiological services are not uniform across India. Part of reason may be that awareness about the roles of and responsibilities of audiologist's is poor among medical practitioners. In some regions of India medical practitioners may be only gross

The geometry of photopolymerized topography influences neurite pathfinding by directing growth cone morphology and migration.

Cochlear implants (CIs) provide auditory perception to those with profound sensorineural hearing loss: however, the quality of sound perceived by a CI user does not approximate natural hearing. This limitation is due in part to the large physical gap between the stimulating electrodes and their target neurons. Therefore, directing the controlled outgrowth of processes from spiral ganglion neurons (SGNs) into close proximity to the electrode array could provide significantly increased hearing function. For this objective to be properly designed and implemented, the ability and limits of SGN neu

An mHealth App (Speech Banana) for Auditory Training: App Design and Development Study.

BACKGROUND: With the growing adult population using electronic hearing devices such as cochlear implants or hearing aids, there is an increasing worldwide need for auditory training (AT) to promote optimal device use. However, financial resources and scheduling conflicts make clinical AT infeasible. OBJECTIVE: To address this gap between need and accessibility, we primarily aimed to develop a mobile health (mHealth) app called Speech Banana for AT. The app would be substantially more affordable and portable than clinical AT; would deliver a validated training model that is reflective of moder

[Subtotal petrosectomy - indications, surgical technique, experiences in Pécs].

Introduction: Subtotal petrosectomy (STP) has been known for decades and it is considered being on the borderline between ear and skull-base surgery. Due to its excessive radicalism and adverse effects on hearing, it has been a forgotten surgical technique until recently. Over the last decade, due to its many benefits and the ability to combine it with modern hearing rehabilitation methods, STP has reappeared, providing a definitive solution to a number of problematic middl-ear disorders. Aim: In our retrospective clinical study, we wanted to highlight the effectiveness of STP, analyzing the e

The Impact of Single-Sided Deafness upon Music Appreciation.

BACKGROUND: Many of the world's population have hearing loss in one ear; current statistics indicate that up to 10% of the population may be affected. Although the detrimental impact of bilateral hearing loss, hearing aids, and cochlear implants upon music appreciation is well recognized, studies on the influence of single-sided deafness (SSD) are sparse. PURPOSE: We sought to investigate whether a single-sided hearing loss can cause problems with music appreciation, despite normal hearing in the other ear. RESEARCH DESIGN: A tailored questionnaire was used to investigate music appreciation

The masking level difference in chinchilla auditory cortex. Effects of inner hair cell loss.

The purpose of the present study was to investigate responses from the unanesthetized chinchilla auditory cortex (AC) to conditions producing a masking level difference (MLD) in perceptual studies, both before and after inner hair cell (IHC) loss caused by carboplatin. Tungsten electrodes were chronically implanted in the right AC (active) and anterior cranium (common) in six adult chinchillas. Following a recovery period, AC responses were obtained from the unanesthetized animal placed in a passive restraint. Toneburst input/output functions were obtained. Tonebursts (500 Hz) ranged from 0 to

Round window stimulation with an interface coupler demonstrates proof of concept.

HYPOTHESIS: To mechanically stimulate the round window (RW) membrane, an actuator with an interface coupler (IC) has the potential to improve sound transmission to the cochlea as compared to the most used RW stimulation device implanted today. If a proof-of-concept IC prototype shows promise as compared the most common method for RW stimulation, there is potential that future design development of an IC will be worthwhile. BACKGROUND: A variety of hearing pathologies resulting in mixed and conductive hearing loss can be addressed by mechanically stimulating the RW to transmit sound to the coc

Enhanced viral-mediated cochlear gene delivery in adult mice by combining canal fenestration with round window membrane inoculation.

Cochlear gene therapy holds promise for the treatment of genetic deafness. Assessing its impact in adult murine models of hearing loss, however, has been hampered by technical challenges that have made it difficult to establish a robust method to deliver transgenes to the mature murine inner ear. Here in we demonstrate the feasibility of a combined round window membrane injection and semi-circular canal fenestration technique in the adult cochlea. Injection of both AAV2/9 and AAV2/Anc80L65 via this approach in P15-16 and P56-60 mice permits robust eGFP transduction of virtually all inner hair

Salicylate induces tinnitus through activation of cochlear NMDA receptors.

Salicylate, the active component of aspirin, is known to induce tinnitus. However, the site and the mechanism of generation of tinnitus induced by salicylate remains unclear. Here, we developed a behavioral procedure to measure tinnitus in rats. The behavioral model was based on an active avoidance paradigm in which rats had to display a motor task (i.e., to jump on a climbing pole when hearing a sound). Giving salicylate led to a decrease in the percentage of correct responses (score) and a drastic increase in the number of false positive responses (i.e., animals execute the motor task during

Towards Policy Integration of Vestibular Management in Pediatric Hearing Health Care: Global Service Delivery Gaps with Relevance to Low-Income and Middle-Income Countries.

BACKGROUND: Vestibular dysfunction (VD) is a common yet often overlooked comorbidity in children with hearing loss, particularly those with sensorineural hearing loss (SNHL), congenital anomalies, or cochlear implants. These challenges are global, but they are especially critical in low- and middle-income countries (LMICs), including South Africa, where diagnostic resources and service integration are limited. Motor development, gaze stability, school performance, and quality of life (QoL) are affected by VD, yet it is seldom included in pediatric hearing health-care frameworks or routine clin

Registration of preoperative temporal bone CT-scan to otoendoscopic video for augmented-reality based on convolutional neural networks.

PURPOSE: Patient-to-image registration is a preliminary step required in surgical navigation based on preoperative images. Human intervention and fiducial markers hamper this task as they are time-consuming and introduce potential errors. We aimed to develop a fully automatic 2D registration system for augmented reality in ear surgery. METHODS: CT-scans and corresponding oto-endoscopic videos were collected from 41 patients (58 ears) undergoing ear examination (vestibular schwannoma before surgery, profound hearing loss requiring cochlear implant, suspicion of perilymphatic fistula, contralat
