Callosal and cortical contribution to procedural learning.

Acallosal and callosotomized subjects usually show impairments on tasks requiring bilateral interdependent motor control. However, few studies have assessed the ability of these subjects to learn a skill that requires the simultaneous contribution of each hemisphere in its acquisition. The present study examined whether acallosal and callosotomized subjects could learn a visuomotor skill that involved a motor control from either both or a single hemisphere. Eleven adult patients, six acallosal and five callosotomized, participated in this study. Seven of these patients had epileptic foci locat

An interdisciplinary approach to the diagnosis and management of a complex case of postencephalitic behavioral disorder.

Postencephalitic behavioral syndrome secondary to measles is an almost extinct condition in Western culture. The present paper describes the clinical state of a 13-year-old pubertal female who presented nine years after the original acute febrile illness. In addition, an innovative behavioral approach to treatment of intractable seizures and aggressive behavior is described.

Altered Gray Matter Myelin in Type IIb Focal Cortical Dysplasia.

BACKGROUND AND OBJECTIVES: Myelin is altered in several neurologic disorders. Published data demonstrate reduced white matter myelin content and lower oligodendrocyte cell number in postsurgical brain specimens obtained from patients with focal cortical dysplasia (FCD) and temporal lobe epilepsy; a pathogenic role of dysfunctional myelin in focal epilepsies has been proposed. Based on this evidence, our study aims to investigate the myelination status in the gray matter in postsurgical brain specimens from patients with FCDIIb. METHODS: We collected specimens from patients with a histopatholo

Cholinergic and behavior-dependent beta and gamma waves are coupled between olfactory bulb and hippocampus.

Olfactory oscillations may enhance cognitive processing through coupling with beta (β, 15-30 Hz) and gamma (γ, 30-160 Hz) activity in the hippocampus (HPC). We hypothesize that coupling between olfactory bulb (OB) and HPC oscillations is increased by cholinergic activation in control rats and is reduced in kainic-acid-treated epileptic rats, a model of temporal lobe epilepsy. OB γ2 (63-100 Hz) power was higher during walking and immobility-awake (IMM) compared to sleep, while γ1 (30-57 Hz) power was higher during grooming than other behavioral states. Muscarinic cholinergic agonist pilocarpine

Speech-related auditory salience detection in the posterior superior temporal region.

Processing auditory human speech requires both detection (early and transient) and analysis (sustained). We analyzed high gamma (70-110 Hz) activity of intracranial electroencephalography waveforms acquired during an auditory task that paired forward speech, reverse speech, and signal correlated noise. We identified widespread superior temporal sites with sustained activity responding only to forward and reverse speech regardless of paired order. More localized superior temporal auditory onset sites responded to all stimulus types when presented first in a pair and responded in recurrent fashi

Cancer frequency detected by positron emission tomography-computed tomography in limbic encephalitis.

OBJECTIVE: Paraneoplastic limbic encephalitis (LE) occurs frequently with considerable variability according to literature reports. We thus determined the cancer frequency in mixed LE subtypes sharing the diagnosis of temporal lobe epilepsy (TLE). METHODS: All patients underwent magnetic resonance imaging (MRI) of the brain, electroencephalography (EEG) recordings, neuropsychological testing, immunohistochemistry, and clinical examination together with whole body 2-fluor-2-desoxy-d-glucose (FDG)-positron emission tomography (PET)/computed tomography (CT) to detect cancer in this observatory s

[Cyberknife and benign pathologies].

Conventional radiotherapy is known to be an effective treatment approach even for "benign" pathologies. However, this kind of treatment yields a high potential for side effects. The Cyberknife, a robotic stereotactic radiotherapy device, enables to offset a large proportion of the disadvantages encountered with conventional radiotherapy essentially through the high precision of dose administration and sparing of healthy tissues. Therefore, it seems to be a treatment of choice in the approach of some benign intracranial diseases. We review published data on indications and outcome of Cyberknife

Cerebral arterial diameters during changes in blood pressure and carbon dioxide during craniotomy.

Forty-five measurements of diameters of 12 human cerebral arteries were performed during 10 craniotomies under moderate changes in mean blood pressure and end tidal CO2. The mean change in blood pressure was 30 +/- 16 mm Hg (standard deviation) and that of end tidal CO2 was 14 +/- 6 mm Hg (standard deviation). These changes were induced with nitroprusside, phenylephrine, and adjustment of ventilator rate. Measurements were made through the operating microscope focused at the highest power, with meticulous attention to constant angle and distance from the artery. The mean diameter change in the

Geniculate hemianopia: incongruous homonymous field defects in two patients with partial lesions of the lateral geniculate nucleus.

Quantitative perimetric studies in two patients with involvement of a lateral geniculate nucleus (caused by astrocytoma in one and a small arteriovenous malformation in the other) revealed strikingly incongruous defects in the contralateral homonymous fields of vision. The patterns of these hemianopias were analysed and correlated anatomically in terms of established patterns of retinal projections within the six cellular laminae of the geniculate nucleus. Incongruous wedge-shaped field defects recorded in the patient with the astrocytoma are unique and appear to be pathognomonic of focal geni

Long-term deep intracerebral microelectrode recordings in patients with drug-resistant epilepsy: Proposed guidelines based on 10-year experience.

PURPOSE: Human neuronal activity, recorded in vivo from microelectrodes, may offer valuable insights into physiological mechanisms underlying human cognition and pathophysiological mechanisms of brain diseases, in particular epilepsy. Continuous and long-term recordings are necessary to monitor non predictable pathological and physiological activities like seizures or sleep. Because of their high impedance, microelectrodes are more sensitive to noise than macroelectrodes. Low noise levels are crucial to detect action potentials from background noise, and to further isolate single neuron activi

A pro-convulsive carbamazepine metabolite: quinolinic acid in drug resistant epileptic human brain.

Drugs and their metabolites often produce undesirable effects. These may be due to a number of mechanisms, including biotransformation by P450 enzymes which are not exclusively expressed by hepatocytes but also by endothelial cells in brain from epileptics. The possibility thus exists that the potency of systemically administered central nervous system therapeutics can be modulated by a metabolic blood-brain barrier (BBB). Surgical brain specimens and blood samples (ex vivo) were obtained from drug-resistant epileptic subjects receiving the antiepileptic drug carbamazepine prior to temporal lo

Improvement of aggressive and antisocial behavior after resection of temporal lobe tumors.

Seizures associated with temporal lobe tumors may rarely manifest as episodic aggressive behavior. We describe 2 cases involving pediatric patients who presented with histories of unusually aggressive and antisocial behavior. Magnetic resonance imaging identified right mesial temporal lobe masses in both patients. After craniotomy for tumor removal, both patients were seizure-free and had marked reductions in their aggressive behavior. Tumors in the temporal lobe may be associated with behavioral problems, including aggression and rage attacks, which can be alleviated with surgical interventio

Effects of Small Conductance Ca2+-Activated Potassium Channel Agonists on SLEs in Varied Epilepsy Models, From Animal Slices to Pharmacoresistant Human Tissue.

To assess the impact of SK channel agonists on seizure-like events (SLEs) in various seizure models in slices of the temporal cortex obtained from pharmacoresistant patients. SLEs were triggered by applying 4-aminopyridine (100 μM) to slices of the entorhinal cortex taken from both normal and pilocarpine-treated rats. Additionally, SLEs were induced in slices of the temporal cortex obtained from individuals who had undergone epilepsy surgery. In the case of human slices, SLEs were also provoked by increasing potassium levels along with the administration of either 4-AP (100 μM) or bicuculline

Calsyntenin-1 expression and function in brain tissue of lithium-pilocarpine rat seizure models.

To present the expression of calsyntenin-1 (Clstn1) in the brain and investigate the potential mechanism of Clstn1 in lithium-pilocarpine rat seizure models. Thirty-five male SD adult rats were induced to have seizures by intraperitoneal injection of lithium chloride pilocarpine. Rats exhibiting spontaneous seizures were divided into the epilepsy (EP) group (n = 15), whereas those without seizures were divided into the control group (n = 14). Evaluate the expression of Clstn1 in the temporal lobe of two groups using Western blotting, immunohistochemistry, and immunofluorescence. Additionally,

Inhibition of the SGLT2/NHE-1/NLRP3 signaling axis attenuates neuroinflammation and oxidative stress to ameliorate seizures and cognitive impairment in epileptic mice.

OBJECTIVE: Beyond neuroinflammation, oxidative stress is a key pathomechanism in epilepsy. This study investigated a novel SGLT2/NHE-1/NLRP3 signaling axis and evaluated its role in driving oxidative stress and neuroinflammation in epilepsy. We aimed to determine whether targeted inhibition of this axis could alleviate neuronal excitability and cognitive deficits by restoring redox balance and suppressing neuroinflammation. METHODS: Network pharmacology predicted the primary anti-epileptic target of dapagliflozin. Bioinformatic analysis was performed on the GEO dataset GSE256068 from patients

Complex emotion recognition in neurological and neurodevelopmental conditions.

Social cognition deficits are prevalent but under-recognised in neurological and neurodevelopmental disorders. Complex emotion recognition (CER) - encompassing mentalising, theory of mind, and perspective-taking - is crucial for social functioning but lacks a standardised clinical assessment. We evaluated CER performance across neurological and neurodevelopmental populations using COSIMO, a novel online assessment tool with muted video clips of dyadic interactions, in six clinical cohorts compared to controls (HC, N = 449): temporal lobe epilepsy (TLE, N = 36), multiple sclerosis (MS, N = 64),
