Wernicke encephalopathy, due to the depletion of thiamine, can be a complication of chronic hemodialysis. We report a 43 years old diabetic male in chronic hemodialysis, who two weeks after an infra-condyleal amputation of his left leg, was admitted to the hospital due to an episode of vomiting and abdominal pain lasting 5 days, where confusion and ocular motor signs appeared. Parenteral thiamine administration was started and the confusional state abated. Dialysis can be a predisposing factor for Wernicke encephalopathy and this diagnosis must be considered in confused patients.

AIM: To compare clinical, microbiological, echocardiographic factors and complications between patients that died during an episode of infective endocarditis and those who survived.

We report the case of a 63 years old woman with a left mammary hydatid cyst. The patient presented with a left painless mammary mass of four months evolution. Mammography showed a dense image of 9 cm phi in the left breast. Ultrasound showed a solid and heterogeneous lesion. The tumor was excised under general anesthesia. The initial macroscopic inspection showed a tumor with a well defined fibrous capsule filled with a grayish material that contained membrane debris. The pathological study confirmed the diagnosis of a hydatid cyst.

Lately, myeloprolipherative disorders are frequently reported as causes of portal vein thrombosis, probably due to the early detection of latent cases of this condition. We report two patients with portal vein thrombosis that presented with abdominal pain, nausea, vomiting and clinical consequences of portal hypertension such as variceal hemorrhage, splenomegaly and ascites. Diagnosis was made by a CAT scan in one patient and doppler ultrasound in the other. Both patients had high platelet counts and an essential thrombocytosis in the bone marrow.

In a family composed by eight brothers, five had a type I glycogen storage disease and three presented with liver tumors complicated with hemorrhage or malignant transformation, during the follow up. The periodic ultrasonographic control allowed the early diagnosis of these neoplasms. We describe the clinical picture and treatment of two patients. Metabolic alterations were corrected during the preoperative period with parenteral nutrition. Resection was successful in both cases. It is concluded that a close follow up and early treatment of these lesions is effective and avoids complications.

BACKGROUND: Adult respiratory distress syndrome is highly prevalent in intensive care units and has a high mortality. Lately, nitric oxide has been used as adjuvant therapy.
AIM: To study the effects of nitric oxide inhalation in patients with adult respiratory distress syndrome.
PATIENTS AND METHODS: Twelve patients with adult respiratory distress syndrome, were subjected to nitric oxide inhalation at a concentration of 10 parts per million, during 30 minutes.

Treatment of human hydatidosis is surgical. Lately, however, surgical indications have decreased, due to pharmacological therapy and ultrasound-guided percutaneous cyst puncture, aspiration of its contents, instillation of medications and respiration.
