Discovery and characterization of phenoxyacetic acid derivatives as potential antiepileptic agents.

This study aimed to discover novel multifunctional anticonvulsant agents through the evaluation of a series of test compounds 5d-f, 7b, and 10c-f, which were previously identified based on their potent anti-inflammatory activity in both in vitro and in vivo models in acute and chronic seizure models. Initial screening in the pentylenetetrazol (PTZ)-induced seizure model identified compounds 7b, 5f, 5e, and 10c as the most effective, with compound 7b demonstrating complete seizure protection (100%) and zero mortality, outperforming the reference drug valproic acid (VI). These four candidates we

Ligand-gated mechanisms leading to ictogenesis in focal epileptic disorders.

We review here the neuronal mechanisms that cause seizures in focal epileptic disorders and, specifically, those involving limbic structures that are known to be implicated in human mesial temporal lobe epilepsy. In both epileptic patients and animal models, the initiation of focal seizures - which are most often characterized by a low-voltage fast onset EEG pattern - is presumably dependent on the synchronous firing of GABA-releasing interneurons that, by activating post-synaptic GABAA receptors, cause large increases in extracellular [K+] through the activation of the co-transporter KCC2. The Spatial Learning Task of Lhermitte and Signoret (1972): Normative Data in Adults Aged 18-45.

OBJECTIVE: The Spatial Learning Task of Lhermitte and Signoret is an object-location arbitrary associative learning task. The task was originally developed to evaluate adults with severe amnesia. It is currently used in populations where the memory system either is not yet fully developed or where it has been compromised (e.g. epilepsy, traumatic brain injury, electroconvulsive therapy, cerebrovascular disease and dementia). Normative data have been published for paediatric cohorts and for older adults, however no data exist for the intervening adult years. METHOD: Here, we address this gap,

Faciobrachial Dystonic Seizures as a Sign of Relapse in a Child with LGI-1 Encephalitis.

We report an interesting case of a young girl with LGI1-antibody encephalitis who presented at 7 years old with very frequent seizures and severe neurocognitive decline. She responded very well to high dose corticosteroids and intravenous immunoglobulin (IVIG) initially but relapsed after 7 months. The relapse included frequent faciobrachial dystonic seizures (FBDS) that were successfully treated with rituximab. This case report highlights a few important points about LGI1-antibody encephalitis in children to help clinicians recognize this condition early and start prompt treatment with immuno

Excitatory GABA input directly drives seizure-like rhythmic synchronization in mature hippocampal CA1 pyramidal cells.

GABA, which generally mediates inhibitory synaptic transmissions, occasionally acts as an excitatory transmitter through intense GABA(A) receptor activation even in adult animals. The excitatory effect results from alterations in the gradients of chloride, bicarbonate, and potassium ions, but its functional role still remains a mystery. Here we show that such GABAergic excitation participates in the expression of seizure-like rhythmic synchronization (afterdischarge) in the mature hippocampal CA1 region. Seizure-like afterdischarge was induced by high-frequency synaptic stimulation in the rat

Amobarbital effects on the posterior hippocampus during the intracarotid amobarbital test.

OBJECTIVE: To relate functional effects on the hippocampus during the intracarotid amobarbital test (IAT) to its direct perfusion with amobarbital. METHODS: In 17 patients with intractable temporal lobe epilepsy, 28 hemispheres were perfused with 2.3 mL of 10% solution of 200 mg amobarbital and 37 MBq 99mTc-hexamethylpropylene amine oxime (HMPAO). For evaluation of amobarbital effects, data were combined from stereo-EEG (S-EEG) recordings from intrahippocampal depth electrodes and high-resolution SPECT after intracarotid injection of HMPAO. RESULTS: Perfusion of the entire hippocampus was ob

The synaptic organization of the human temporal lobe neocortex by high-resolution transmission, focused ion beam scanning, and electron microscopic tomography.

Fine-scale transmission electron microscopy (TEM), focused ion beam scanning EM (FIB), and EM tomography have opened a new window on the synaptic organization of the normal, developing and pathologically altered brain in experimental animals. Progress in the human brain has been slower, due to technical challenges and the problem of tissue availability from donors that underwent epilepsy or tumor surgery. The present manuscript is in part an overview of the geometry of synaptic boutons in surgical biopsy samples taken from human temporal lobe neocortex ('hTLN'). Here, the number, size, and sha

Persistent neuronal firing in the medial temporal lobe supports performance and workload of visual working memory in humans.

The involvement of the medial temporal lobe (MTL) in working memory is controversially discussed. Recent findings suggest that persistent neural firing in the hippocampus during maintenance in verbal working memory is associated with workload. Here, we recorded single neuron firing in 13 epilepsy patients (7 male) while they performed a visual working memory task. The number of colored squares in the stimulus set determined the workload of the trial. Performance was almost perfect for low workload (1 and 2 squares) and dropped at high workload (4 and 6 squares), suggesting that high workload e

Can antibiotics cause a psychosis?: Case report and review of the literature.

It is relatively unknown that some of the possible side effects of antibiotic treatment can be transient psychotic episodes and other encephalopathies such as seizure disorders. Since these are rare events, there have not been many studies about them, nor is there a clear understanding of the underlying basis for these symptoms. This phenomenon was recently brought to our attention by a patient whose case history is discussed here. For the past few years, he has been diagnosed at different times with schizophrenia, schizoaffective disorder, bipolar disorder with psychosis, temporal lobe epilep

Neurological symptoms as the result of enlarged dimensions and non-typical course of inferior superficial temporal vein.

Neurological symptoms as the result of non-typical course of superficial cerebral veins are described in available literature very rarely. The case described below indicates that in some circumstances the compression symptoms derived from the cerebral cortex may be incredibly more serious than their anatomical reasons. In our observation a young woman was described complaining of paroxysmal numbness of the left upper limb with paraesthesia of the left side of the face, the left eye and left half of the tongue. The patient said that in childhood she used to have paroxysmal itching of the left h

Evaluation of long-term quality of life in traumatic brain injury patients early treated with biperiden - a randomized controlled trial.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of disability, with post-traumatic epilepsy (PTE) being a major complication. Biperiden has shown experimental potential in preventing epileptogenesis, however its impact on outcomes, such as quality of life (QoL), remains unknown. This study evaluated long-term QoL in TBI survivors treated with biperiden and identified clinical and socioeconomic predictors of their QoL. METHODS: We analyzed QoL data from a randomized, double-blind, placebo-controlled trial involving 48 adults with moderate to severe TBI, assessed on average 32 month

Does white matter structure relate to hemispheric language lateralization? A systematic review.

The relationship between functional language lateralization and diffusion MRI-based white matter metrics remains a subject of considerable interest and complexity. This systematic review aims to synthesize existing diffusion MRI studies examining white matter correlates of functional language dominance. Twenty-five studies were identified through searches of Web of Science, Scopus and Ovid MEDLINE (search period: inception to 16th May 2023), involving adults with epilepsy, tumours or healthy controls. The results suggest that while the arcuate fasciculus and corpus callosum are commonly associ

Identification of Houge type of X-linked syndromic mental retardation caused by CNKSR2 truncated variants.

BACKGROUND: Houge type of X-linked syndromic mental retardation (MRXSHG) is a type of X-linked condition which is mainly manifested as delayed development, mental retardation, epilepsy that begins at an early age, and delayed language acquisition. MRXSHG is a serious disorder with CNKSR2 variant and at least 34 variants have been identified in MRXSHG patients. However, the genotype-phenotype correlation and variants characteristics of CNKSR2 need further investigation and improvement. METHODS: Two Chinese MRXSHG families were recruited, and their genetic causes were investigated using whole-e

Biofluid-Permeable and Erosion-Resistant Wireless Neural-Electronic Interfaces for Neurohomeostasis Modulation.

Neural-electronic interfaces through delivering electroceuticals to lesions and modulating pathological endogenous electrical environments offer exciting opportunities to treat drug-refractory neurological disorders. Such an interface should ideally be compatible with the neural tissue and aggressive biofluid environment. Unfortunately, no interface specifically designed for the biofluid environments is available so far; instead, simply stacking an encapsulation layer on silicon-based substrates makes them susceptible to biofluid leakage, device malfunction, and foreign-body reactions. Here, w

Acetazolamide as an effective treatment for pilomotor seizures in autoimmune encephalitis.

Pilomotor seizures are strongly associated with autoimmune encephalitis (AE), particularly anti-LGI1 encephalitis. The carbonic anhydrase inhibitor acetazolamide may have special efficacy for treating AE-associated pilomotor seizures. Six patients with AE (five anti-LGI1, one seronegative) and temporal lobe pilomotor seizures (five with seizures inducible by hyperventilation) were treated with acetazolamide, administered in a cycling (2-days-ON, 4-days-OFF) regimen to offset tolerance. Seizures were assessed during epilepsy monitoring unit (EMU) recordings in four inpatients (one of whom also

Alleviation of hippocampal necroptosis and neuroinflammation by NecroX-7 treatment after acute seizures.

Temporal lobe epilepsy (TLE) is one of the most common neurological disorders, but still one-third of patients cannot be properly treated by current medication. Thus, we investigated the therapeutic effects of a novel small molecule, NecroX-7, in TLE using both a low [Mg2+]o-induced epileptiform activity model and a mouse model of pilocarpine-induced status epilepticus (SE). NecroX-7 post-treatment enhanced the viability of primary hippocampal neurons exposed to low [Mg2+]o compared to controls in an MTT assay. Application of NecroX-7 after pilocarpine-induced SE also reduced the number of deg
