A Dual-Mode Near-Infrared Optical Probe and Monte Carlo Framework for Functional Monitoring of Rheumatoid Arthritis: Addressing Diagnostic Ambiguity and Skin Tone Robustness.

Current diagnostic modalities for rheumatoid arthritis (RA), such as Magnetic Resonance Imaging (MRI) and ultrasound (US), excel at visualizing structural pathology but are either resource-intensive or often limited to morphological assessment. In this work, we present the design and technical validation of a low-cost continuous-wave near-infrared (NIR) dual-mode optical probe for functional monitoring of joint inflammation. Unlike superficial imaging, NIR light penetrates approximately 3-5 cm and is tissue and wavelength dependent, enabling trans-illumination of the synovial volume. The syste

An in-depth snake venom proteopeptidome characterization: Benchmarking Bothrops jararaca.

UNLABELLED: A large-scale proteomic approach was devised to advance the understanding of venom composition. Bothrops jararaca venom was fractionated by OFFGEL followed by chromatography, generating peptidic and proteic fractions. The latter was submitted to trypsin digestion. Both fractions were separately analyzed by reversed-phase nanochromatography coupled to high resolution mass spectrometry. This strategy allowed deeper and joint characterizations of the peptidome and proteome (proteopeptidome) of this venom. Our results lead to the identification of 46 protein classes (with several uniqu

Sensory processing in Parkinson's and Huntington's disease: investigations with 3D H(2)(15)O-PET.

There is conjoining experimental and clinical evidence supporting a fundamental role of the basal ganglia as a sensory analyser engaged in central somatosensory control. This study was aimed at investigating the functional anatomy of sensory processing in two clinical conditions characterized by basal ganglia dysfunction, i.e. Parkinson's and Huntington's disease. Based on previously recorded data of somatosensory evoked potentials, we expected deficient sensory-evoked activation in cortical areas that receive modulatory somatosensory input via the basal ganglia. Eight Parkinson's disease pati

Factors Associated With a Discretionary Upper-Extremity Surgery.

PURPOSE: Surgery for nontraumatic upper-extremity problems is largely discretionary and preference-sensitive. Psychological and social determinants of health correlate with greater symptoms and limitations and might be associated with discretionary operative treatment. METHODS: We used routinely collected patient-reported outcome measures from patients with de Quervain tendinopathy, ganglion cyst, trapeziometacarpal arthritis, trigger digit, and carpal tunnel syndrome to study factors associated with discretionary surgery using multiple logistic regression. Patients completed a measure of the

Functional and patient-reported outcomes of the Swanson metacarpo-phalangeal arthroplasty in the rheumatoid hand.

INTRODUCTION: Replacement of the metacarpo-phalangeal joints (MCPJ) with silastic Swanson's implants can help decrease pain, stiffness and allow for improved function in rheumatoid arthritis (RA). There is a lack of patient reported outcome measure (PROM) studies assessing the efficacy of this procedure in RA. The aim of this study was to report any change in function, pain, stiffness and satisfaction following the Swanson MCPJ replacement using patient reported outcomes in a rheumatoid population. METHODS: The combined results of 64 RA patients (71 hands) with 284 Swanson MCPJ arthroplasties

Trichorhinophalangeal syndrome type I: a novel mutation and Perthes-like changes of the hip in a family with 4 cases over 3 generations.

The trichorhinophalangeal syndrome is a rare genetic syndrome with characteristic craniofacial and skeletal abnormalities including hip pathology in variable manifestation. We describe hip involvement with Perthes-like changes and a novel mutation of the TRPSI gene in a family with 4 affected individuals. This case series underlines the clinical significance of rare genetic disorders such as TRPS that among other differential diagnoses should be kept in mind when children present with Perthes-like changes of the hip joint.

Increased levels of IL-6, IL-1β, and TNF-α in Kashin-Beck disease and rats induced by T-2 toxin and selenium deficiency.

The objective of this study is to investigate the possible role of inflammatory mediators such as IL-6, IL-1β, and TNF-α in Kashin-Beck disease (KBD) children and rats fed with T-2 toxin under a selenium-deficient nutrition status in order to determine possible mechanism underlying KBD. Sprague-Dawley rats were administered a selenium-deficient diet for 4 weeks prior to their exposure to T-2 toxin for 4 weeks. The morphology of joint cartilages of KBD children and rats was examined by light microscopy, and the expression of proteoglycans was determined by histochemical staining. The serum leve

Percutaneous screw and axial Kirschner wire fixation for acute transscaphoid perilunate fracture dislocation.

PURPOSE: In this retrospective, nonrandomized study, we evaluated the clinical outcomes of percutaneous screw and K-wire fixation in the treatment of acute transscaphoid perilunate fracture dislocation. METHODS: Between 2004 and 2009, we studied 24 patients treated by fixation with a 2.0-mm (0.078-in) axial K-wire across the capitolunate joint and a percutaneous screw (3.0-mm AO cannulated screws were used in 13 patients, and Herbert-Whipple screws were used in 11 patients) within 7 days of the injury. We performed a comparative radiographic assessment of the scapholunate angle, radiolunate a

Concomitant Distal Radius Fracture, Scaphoid Fracture, and Pisiform Dislocation with Ulnar Nerve Neuropraxia: A Case Report with Biomechanical Analysis.

INTRODUCTION: Distal radius fractures commonly result from falls onto an outstretched hand, making them a frequent presentation in orthopedic practice. However, pisiform dislocation associated with a distal radius fracture is uncommon, due to the pisiform's protected anatomical position and the distinct biomechanical forces typically required for its displacement. When this combined injury pattern is further complicated by ulnar nerve neuropraxia, it becomes rare and may be easily overlooked. The clinical subtlety of pisiform dislocation can lead to diagnostic delay, potentially resulting in i

Provider assessment of the temporomandibular joint in Juvenile idiopathic arthritis: a retrospective analysis from the CARRA database.

BACKGROUND: Temporomandibular joint (TMJ) involvement is an often underrecognized complication of juvenile idiopathic arthritis (JIA) that can cause decreased mandibular growth, altered facial morphology, and orofacial pain. It is estimated that the TMJ is affected in 30-45% of children with JIA. Standardized physical examination and imaging evaluations are important in accurately assessing active TMJ arthritis and sequalae. Little is known about the rate at which providers evaluate TMJ involvement in their clinical practice. METHODS: Data were obtained from the Childhood Arthritis and Rheuma

Grip strength measurements at two different wrist extension positions in chronic lateral epicondylitis-comparison of involved vs. uninvolved side in athletes and non athletes: a case-control study.

BACKGROUND: Lateral epicondylitis is a common sports injury of the elbow caused due to altered muscle activation during repetitive wrist extension in many athletic and non-athletic endeavours. The amount of muscle activity and timing of contraction eventually is directly dependent upon joint position during the activity. The purpose of our study was to compare the grip strength in athletes with lateral epicondylalgia in two different wrist extension positions and compare them between involved and uninvolved sides of athletes and non-athletes. METHODS: An assessor-blinded case-control study of

Intraoperative radial nerve injury during coronary artery surgery--report of two cases.

BACKGROUND: Peripheral nerve injury and brachial plexopathy are known, though rare complications of coronary artery surgery. The ulnar nerve is most frequently affected, whereas radial nerve lesions are much less common accounting for only 3% of such intraoperative injuries. CASE PRESENTATIONS: Two 52- and 50-year-old men underwent coronary artery surgery. On the first postoperative day they both complained of wrist drop on the left. Neurological examination revealed a paresis of the wrist and finger extensor muscles (0/5), and the brachioradialis (4/5) with hypoaesthesia on the radial aspect

Functional assessment aided by motion laboratory studies.

The physician needs to integrate the results of the assessment of overall patterns of functional use (Table 1), static contractures, dynamic deformities of motor imbalance for multiple levels of involvement (shoulder, elbow, wrist, and hand), and sensory deficiencies. This information is combined with a general assessment of the child's mentation, motivation, and generalized medical condition. Certain patients benefit most from surgical intervention in cerebral palsy. Patients with spastic deformities or flaccid control of specific movements can be helped significantly. In patients with flacci
