Perioperative analgesic effect of preemptive ultrasound-guided rectus sheath block and transversus abdominis plane block with dexmedetomidine versus dexamethasone for laparoscopic surgery in paediatrics: A randomised trial.

BACKGROUND: We aimed to assess the impact of the rectus sheath and transversus abdominis plane blocks on postoperative pain alleviation and recovery in children undergoing laparoscopic appendicectomies. METHODS: Ninety patients got a transversus abdominis plane block and rectus sheath block and were randomly divided into three groups. Each patient had taken 2.5ml of 0.5% bupivacaine (1.25mg/kg). Group 1: 0.3mg/kg of dexamethasone was added to the bupivacaine. Group 2: 1μg/kg of dexmedetomidine was added to the bupivacaine. Group 3: received only bupivacaine. We recorded analgesic usage overal

Carcinoid tumours of the appendix: an analysis of emergency appendicectomies over a 24-year period and outcomes of laparoscopic versus open resection.

BACKGROUND: Carcinoid tumours of the appendix are the most common primary malignant lesion of the appendix. However, the overall incidence remains low; found in as few as 0.3-0.9% appendicectomy specimens. Almost all appendiceal carcinoids are found incidentally during surgery for suspected appendicitis. METHODS: A retrospective review of all appendiceal carcinoids was performed in six hospitals from January 1990 until December 2013. Demographic data, operative technique, histopathological characteristics, clinic reviews and need for further treatment were recorded and compared with literatur

Serous Tubal Intraepithelial Carcinoma-Like and Pagetoid Tubal Metastasis of an Ovarian Large Cell Neuroendocrine Carcinoma: Peculiar Metastatic Growth Patterns of a Rare Tumor.

A large cell neuroendocrine carcinoma of the ovary is presented because of tubal metastases with peculiar growth patterns: pagetoid and serous tubal intraepithelial carcinoma-like. The possibility that a tubal intraepithelial carcinoma could represent a metastasis should be considered by pathologists.

The Unacceptable Morbidity of Negative Laparoscopic Appendicectomy.

BACKGROUND: One of the most common acute conditions managed by general surgeons is acute appendicitis. Laparoscopic appendicectomy (LA) is the surgical technique used by many surgeons. The aims of this study were to define our unit's negative appendicectomy rate and compare the outcomes associated with removal of a normal appendix with those for acute appendicitis in patients having LA. METHODS: A single-centre retrospective case note review of patients undergoing LA for suspected acute appendicitis was performed. Patients were divided into positive and negative appendicectomy groups based on

Risk of new-onset type II diabetes after appendicectomy.

BACKGROUND: Altered immune function after appendicectomy has been associated with autoimmune disease, even though the mechanisms are not clearly elucidated. This study aimed to investigate whether the frequency of new-onset type II diabetes was increased after appendicectomy in a case-control study. METHODS: This was a retrospective cohort study from the Taiwan Longitudinal Health Insurance Database 2000. The relative risk was compared with that in the general population using population-based data. Each patient was tracked for a 3-year interval to identify those who developed type II diabete

[A letal case of gastro-intestinal basidiobolomycosis].

We report here the case of a 55-year-old man from Mali, who presented with abdominal pain. Radiological exploration revealed an ileo-colonic mass surrounding the appendix. A biopsy was taken and on histology, transmural granulomatous inflammation of numerous eosinophils, lymphocytes, plasmocytes and giant cells was seen. Tuberculosis was suspected clinically and an antibiotic treatment was initiated. Two months later, the patient died of septic complications. Basidiobolus ranarum was identified by PCR. Pathogens were retrospectively highlighted on biopsies. These elements were between 10 and 1

Appendiceal mucocele and pseudomyxoma peritonei; the clinical boundaries of a subtle disease.

PATIENT: Male, 70 • Male, 84. FINAL DIAGNOSIS: Appendiceal mucocele and pseudomyxoma peritonei. SYMPTOMS: -. MEDICATION: -. CLINICAL PROCEDURE: -. SPECIALTY: Surgery. OBJECTIVE: Rare disease. BACKGROUND: Mucocele of the appendix is an uncommon cystic lesion characterized by distension of the appendiceal lumen with mucus. Most commonly, it is the result of epithelial proliferation, but it can also be caused by inflammation or obstruction of the appendix. When an underlying mucinous cystadenocarcinoma exists, spontaneous or iatrogenic rupture of the mucocele can lead to mucinous intraperi

Serrated lesions of the appendix: do they differ from their colorectal counterparts?

BACKGROUND/AIMS: The aim of this study is, therefore, to classify appendiceal serrated polyps in a large case series with respect to the recent World Health Organization classification using diagnostic criteria provided for colorectal serrated polyps. MATERIALS AND METHODS: A total of 960 appendix specimens diagnosed between 2005 and 2010 were reviewed retrospectively, and cases presenting with a polyp with serrated morphology were classified with reference to the recent World Health Organization criteria. Histologic criteria comprised architectural features of the crypts, including serration

[Right lower quadrant abdominal pain--the usual suspects? Diagnosis and therapy of a symptomatic mesenteric cyst].

HISTORY AND CLINICAL FINDINGS: A 17-year-old male sought medical attention for right lower abdominal pain. At clinical examination an abdominal mass in the right lower abdominal quadrant was accompanied by pain and tenderness in this very region. The febrile patient (temperature axillary: 37.5 °C, rectal: 38.6 °C) was in reduced general health. INVESTIGATIONS: Except for an elevated C-reactive protein (CRP: 14.3 mg/dl, normal: < 0.5) laboratory tests were inconspicuous. Abdominal ultrasound showed a hypoechogenic/anechogenic septated mass measuring 7.2 × 10.4 × 15 cm as well as small amounts

Hyperbilirubinaemia a predictive factor for complicated acute appendicitis: a study in a tertiary care hospital.

UNLABELLED: Pott's puffy tumor, a rare complication of frontal sinusitis or trauma, presents with subperiosteal abscess and osteomyelitis of the frontal bone. Early diagnosis with imaging studies and prompt treatment with antibiotics and percutaneous drainage of the abscess requires in most cases for a successful outcome. We report a case of a 12-year-old boy who initially presented with left sided retro-orbital/frontal area headache associated with high grade temperature and painful localized small forehead swelling. Symptoms developed few days after he had a dental extraction of left upper m

Negative appendicectomy and perforation rates in patients undergoing laparoscopic surgery for suspected appendicitis.

BACKGROUND: Despite widespread use of imaging technologies including ultrasonography and computed tomography, rates of negative appendicectomy and perforated appendicitis remain high. This trend analysis examined whether rates of negative appendicectomy and perforated appendicitis have decreased over time, and sought to evaluate clinical predictors associated with negative appendicectomy and perforated appendicitis. METHODS: This analysis was based on the prospective database of the Swiss Association of Laparoscopic and Thoracoscopic Surgery (SALTS). All patients aged 12 years and over underg

Subhepatic appendicitis with subdiaphragmatic abscess in a pediatric patient without intestinal malrotation: case report.

BACKGROUND: Acute subhepatic appendicitis in children is an uncommon presentation. It is usually associated with intestinal malrotation. When these conditions are met, accurate diagnosis and early management decisions are delayed. CLINICAL CASE: We present the case of a 10-year-old male who had diarrhea without mucus or blood for 5 days. He was treated with antibiotics. Afterwards, he presented with vomiting, abdominal pain, and fever. Physical examination of the abdomen demonstrated a soft and depressible mass and pain in the lower right abdomen. Abdominal ultrasound and tomography reported

[Laparoscopic coincidental finding in a case of incomplete ileus: adenocarcinoma of the small intestine as first manifestation of Crohn's disease].

The case-study reminds of adenocarcinoma of the small intestine as a rare complication of Crohn's disease. A few more than 100 of these cases have been published. Epidemiological studies concerning small bowel carcinoma showed consumption of sugar and carbohydrates as pathogenetic factors, other conditions like ileostoma, ileumconduit, Crohn's disease and coeliac disease have been identified to some extent. An adenoma-carcinoma sequence as in large intestine carcinoma has been discussed. Immunohistochemical and oncogenetic findings failed to demonstrate any result of practical clinical value.
