Argatroban for preventing occlusion and restenosis after extracranial artery stenting.

BACKGROUND/AIMS: Restenosis following extracranial artery stenting is a limitation that affects long-term outcomes. Effective and satisfying pharmacological strategies in preventing restenosis have not been established. This study aimed to evaluate whether argatroban, a direct thrombin inhibitor, could reduce the risk of in-stent restenosis after extracranial artery stenting. METHODS: One hundred and fourteen patients hospitalized between August 2010 and August 2011 were enrolled. Patients were randomly assigned to argatroban (n = 58) and blank control groups (n = 56). The patients in the arg

An analysis of flow dynamics in cerebral cavernous malformation and orbital cavernous angioma using indocyanine green videoangiography.

BACKGROUND: Cerebral cavernous malformations (CCMs) are known to be vascular anomalies with low perfusion because of being angiographically occult. We attempted direct visualization of blood flow within CCMs and orbital cavernous angiomas (CAs), and analyzed flow dynamics using indocyanine green videoangiography (ICG-VAG). METHODS: This series included seven CCMs and two orbital CAs. ICG-VAG was performed to visualize blood flow of the lesions before resection. Time to peak of staining was evaluated by reviewing recorded ICG-VAG. RESULTS: In five of seven CCMs, stain was identified. CCMs wer

"Stretched loop sign" of the vertebral artery: a predictor of vertebrobasilar insufficiency in atlantoaxial dislocation.

BACKGROUND: Vertebrobasilar territory infarction is one of the rarer presentations of CVJ anomalies. A new radiologic sign due to stretching of the short third segment of VA detected on MRA/DSA may identify patients of AAD at risk of developing VBI. METHODS: Seven patients who presented with VBI were found to have a coexisting mobile (n = 6) or fixed (n = 1) AAD. None of these patients had the presence of any of the known risk factors for cerebrovascular disease. On identification of VBI on CT/MRI, DSA (n = 7) and MRA (n = 1) were performed to assess bilateral vertebral arteries. The course o

Intradural dorsal arteriovenous fistula: a case report and literature review.

BACKGROUND: Intradural dorsal arteriovenous fistula (AVF) is a rare spinal vascular malformation that primarily affects the blood-supplying arteries of the intradural dorsal. Its clinical manifestations are dominated by bilateral lower limb weakness and a lack of specificity in early symptoms, which can easily lead to underdiagnosis or misdiagnosis. Thus, the successful diagnosis of intradural dorsal AVF is difficult. CASE DESCRIPTION: A 56-year-old woman presented with lower limb weakness for 3 months without significant sensory deficits. The patient has a history of hyperthyroidism, but the

Molecular and multimodal biomarkers in Moyamoya disease: from pathogenic mechanisms to clinical translation.

Moyamoya disease (MMD) is a chronic, progressive cerebrovascular condition marked by narrowing or blockage of the terminal segments of the internal carotid arteries, resulting in ischemic and hemorrhagic strokes. Its pathogenesis involves a multifactorial interplay between genetic susceptibility, immune-inflammatory dysregulation, endothelial dysfunction, and aberrant vascular remodeling, influenced by non-genetic and environmental factors. Despite considerable research progress, clinically useful biomarkers remain limited, lacking sufficient sensitivity and specificity for predicting disease

Comparative analysis of the geometric morphology and local hemodynamics of small and medium intracranial aneurysms: a two-center retrospective study in a Chinese Han population.

BACKGROUND: Small (<4 mm) and medium (≥4 and <15 mm) intracranial aneurysms (IAs) account for the majority of lesions detected on routine cerebrovascular imaging, yet their true rupture propensity and mechanistic pathways remain controversial, particularly in Asian populations. Emerging evidence indicates that geometry-dependent hemodynamic disturbances may modulate rupture risk, but size-specific patterns have not been systematically compared. Therefore, we aimed to compare the clinical, geometric, and hemodynamic characteristics of small and medium IAs and to clarify the rupture-related mech

Advantages of computed tomography-based navigation in clipping distal anterior cerebral artery aneurysms: a retrospective cohort study.

BACKGROUND: The occurrence rate of distal anterior cerebral artery (DACA) aneurysms is relatively low, primarily due to their deep-seated location, which makes surgical clamping challenging. The objective of this study was to investigate the efficacy and safety of computed tomography (CT) navigation-assisted clipping of DACA aneurysms compared to traditional clipping without navigation. METHODS: A retrospective cohort study involving retrospective data collection was performed. The retrospective analysis was conducted on 139 patients with ruptured DACA aneurysms who underwent clipping. From J

Endovascular Treatment of Distal Lenticulostriate Artery Aneurysm by Selective Catheterization of Artery with Balloon-Blocking Technique: 2-Dimensional Video Illustration.

We report the case of a 15-year-old male patient with polyarteritis nodosa who presented with ruptured lenticulostriate artery (LSA) aneurysm and was successfully treated with endovascular N-butyl-2-cyanoacrylate (Histoacryl, B. Braun, Melsungen, Germany) acrylic glue embolization. Selective catheterization of LSA is sometimes difficult even with a low-profile microcatheter (Magic 1.2 FM, Balt Extrusion, Montmorency, France) due to acute angulation at the origin of the artery. In this 2-dimensional video illustration of the roadmap in digital subtraction angiography, reproduced after informed
