Economic and environmental impact of reducing unnecessary coagulation screen testing on an acute medical unit: a quality improvement project.

BACKGROUND: Inappropriate coagulation testing contributes to inefficiency, cost, and environmental harm. Baseline audits on our acute medical unit (AMU) showed that one-third of coagulation screen requests lacked a clear clinical indication. AIM: To reduce unnecessary coagulation screens on acute medical services [AMU and Medical Same Day Emergency Care (mSDEC)] by 90% within 12 months, aligned with National Health Service (NHS) Net Zero ambitions and our Trust's Green Plan. METHODS: Using the Model for Improvement, we conducted three Plan-Do-Study-Act cycles. Our analogue and digital interv

Global short-term mortality risk and burden associated with tropical cyclones from 1980 to 2019: a multi-country time-series study.

BACKGROUND: The global spatiotemporal pattern of mortality risk and burden attributable to tropical cyclones is unclear. We aimed to evaluate the global short-term mortality risk and burden associated with tropical cyclones from 1980 to 2019. METHODS: The wind speed associated with cyclones from 1980 to 2019 was estimated globally through a parametric wind field model at a grid resolution of 0·5° × 0·5°. A total of 341 locations with daily mortality and temperature data from 14 countries that experienced at least one tropical cyclone day (a day with maximum sustained wind speed associated wit

Genomic differentiation and conservation relevance of isolated marginal Primula farinosa populations in Central-Eastern Europe.

INTRODUCTION: Quaternary climatic oscillations produced heterogeneous range dynamics in European cold-adapted plants. Primula farinosa L. has been discussed in the context of the interglacial expansion model for moisture-demanding cold-adapted species. However, it remains unclear whether isolated low-elevation populations in Central European calcareous fens and wet meadows represent long-term relicts or recently established populations. METHODS: We used RADseq-derived SNPs from four sampling localities in the Balaton Region of Hungary and one low-elevation comparative locality at Lipce in the

Work motivation profiles and work engagement among nursing assistants in Chinese aged care institutions: a cross-sectional latent profile analysis.

BACKGROUND: Geriatric nursing assistants are central to long-term care, but average-score analyses may conceal differences in how autonomous and controlled forms of work motivation combine within individuals. OBJECTIVE: To identify latent work motivation profiles among geriatric nursing assistants, compare the primary outcome of work engagement across profiles, and examine factors associated with profile membership. METHODS: A cross-sectional survey was conducted between May and August 2025 in nine aged care institutions in Xi'an and Weinan, Shaanxi Province, China. Eligible participants wer

Prevalence and risk factors for udder cleft dermatitis in UK dairy herds.

Udder cleft dermatitis (UCD) is an endemic disease in dairy cows, initially presenting as skin lesions, which can lead to ulceration and deep, persistent infection. Early lesions may go undetected due to their location deep in the intermammary cleft. There is little existing evidence on the prevalence of UCD in dairy cattle in the United Kingdom (UK), but studies in other European countries with similar climates and dairy production systems have found prevalence to be highly variable between herds. The aim of this study was to investigate the prevalence, severity, and location of UCD lesions i

Conceptual thermal constraints on the growth of the first tree on a terraformed Mars.

The environmental conditions on present‑day Mars are far outside the range tolerated by known complex terrestrial life. Conceptual climate studies have suggested that, in hypothetical terraforming scenarios, artificially enhancing the greenhouse effect could restore Mars to more habitable surface conditions. Early colonizing terrestrial life on a warming Mars would plausibly consist of lichens and high‑alpine or high‑arctic plants. Here, we consider a later, more demanding step and investigate the thermal conditions under which the first tree could, in principle, grow on the Martian surface. B

Spatiotemporal Heterogeneity and the Impact of PM1 and Nitrogen Oxides on Chronic Kidney Disease Mortality in 39 European Countries (1990-2022): A Multi-Model Analysis.

BACKGROUND: Chronic kidney disease (CKD) has become a significant public health burden in Europe, with marked spatiotemporal heterogeneity. Nitrogen oxides (NOx) and fine particulate matter (PM) are considered important environmental risk factors; however, the association of submicron particulate matter (PM1) with CKD mortality and the potential non-linear exposure-response patterns of these pollutants remain insufficiently characterized. METHODS: In this ecological study, national-level aggregated data from 39 European countries from 1990 to 2022 were analyzed. Environmental exposure was ass

Mapping the Contributing Factors to Missed Nursing Care in Hospital Settings During a Global Health Crisis: A Systematic Scoping Review.

BACKGROUND: Little foreknowledge and preparation exist for health-related crises, and they do not match the magnitude of the problem. During the COVID-19 pandemic, nursing care in some countries faced more challenges. One of these challenges was missed nursing care. This scoping review aims to identify and map the factors influencing missed nursing care in hospital settings during the COVID-19 pandemic in studies conducted in developed and developing countries. METHODS: A scoping review was conducted according to methodology recommended by the Joanna Briggs Institute (JBI). We searched five d

Maternal exposure to ambient temperature and risk of preterm birth in Chengdu, China, from 2017 to 2020: a cohort study.

BACKGROUND: Due to climate change, the frequency and intensity of heat waves and other extreme weather events are rapidly increasing. Compared to the general population, pregnant women and fetuses are increasingly vulnerable to the effects of extreme temperatures and are associated with the occurrence of adverse birth outcomes, including preterm birth (PTB). However, its risk of preterm birth is currently uncertain. The objective of the research is to examine the effect of ambient temperature on PTB in pregnant women. METHODS: This study included 6,850 pregnant women from the Tongji-Shuangliu

Genomic epidemiology of Histoplasma in Africa.

Histoplasmosis, the disease caused by thermally dimorphic fungi in the genus Histoplasma, is usually associated with pulmonary involvement in healthy individuals and a disseminated syndrome in immunocompromised patients. Among African patients, lung disease is less commonly reported than skin, lymph node, or bone disease. Because different species or strains may be associated with different disease presentations and outcomes, understanding genetic and phenotypic variation in the genus Histoplasma is important for accurate diagnosis and treatment. We sequenced the genomes of 36 Histoplasma isol

Insecticide-treated net use and elimination of malaria in Sub-Saharan African countries: assessing the Global Technical Strategy using an evolutionary game approach.

BACKGROUND: Malaria continues to be a major public health challenge in Sub-Saharan Africa (SSA), where the majority of the countries have not met the World Health Assembly's endorsed Global Technical Strategy (GTS) milestones in 2020 for malaria reduction. Insecticide-treated net (ITN) usage is a well-established and effective intervention, often outperforming other measures such as indoor residual spraying (IRS). However, multiple survey studies have reported improper use of ITNs across various SSA countries. This misuse likely poses an important barrier to the intervention's success, althoug

Exploring individual responses to welfare issues in growing-finishing pig feeding behaviour.

The feeding behaviour of individual growing-finishing pigs can be continuously monitored using sensors such as electronic feeding stations (EFSs), and this could be further used to monitor pig welfare. To make accurate conclusions about individual pig welfare, however, it is important to know whether deviations in feeding behaviour in response to welfare issues are shown only on average or by each individual pig. Therefore, this study aimed (1) to quantify the individual variation in feeding behaviour changes in response to a range of welfare issues, and (2) to explain this individual variatio

Screening and Treatment of Posttraumatic Stress Disorder in Wildfire Evacuees: A Cost-Utility Analysis.

UNLABELLED: Background. Global climate change is resulting in dramatic increases in wildfires. Individuals exposed to wildfires experience a high burden of posttraumatic stress disorder (PTSD), and the cost-effectiveness of the treatment options to address PTSD from wildfires has not been studied. The objective of this study was to conduct a cost-utility analysis comparing screening followed by treatment with paroxetine or trauma-focused cognitive behavioral therapy (TF-CBT) versus no screening in Canadian adult wildfire evacuees. Methods. Using a Markov model, quality-adjusted life-years (QAL

Dengue virus importation risks in Africa: a modelling study.

BACKGROUND: Dengue is a significant global public health concern that poses a threat in Africa. Particularly, African countries are at risk of viral introductions through air travel connectivity with areas of South America and Asia in which explosive dengue outbreaks frequently occur. Limited reporting and diagnostic capacity hinder a comprehensive assessment of continent-wide transmission dynamics and deployment of surveillance strategies in Africa. In this study, we aimed to identify African airports at high risk of receiving passengers with dengue from Asia, Latin America, and other African

Hysteresis effects of different levels of storm flooding on susceptible enteric infectious diseases in a central city of China.

BACKGROUND: Recently, attention has focused on the impact of global climate change on infectious diseases. Storm flooding is an extreme weather phenomenon that not only impacts the health of the environment but also worsens the spread of pathogens. This poses a significant challenge to public health security. However, there is still a lack of research on how different levels of storm flooding affect susceptible enteric infectious diseases over time. METHODS: Data on enteric infectious diseases, storm flooding events, and meteorology were collected for Changsha, Hunan Province, between 2016 an

The Determinants of the Low COVID-19 Transmission and Mortality Rates in Africa: A Cross-Country Analysis.

Background: More than 1 year after the beginning of the international spread of coronavirus 2019 (COVID-19), the reasons explaining its apparently lower reported burden in Africa are still to be fully elucidated. Few studies previously investigated the potential reasons explaining this epidemiological observation using data at the level of a few African countries. However, an updated analysis considering the various epidemiological waves and variables across an array of categories, with a focus on African countries might help to better understand the COVID-19 pandemic on the continent. Thus, w

Hospitalizations for heat-stress illness varies between rural and urban areas: an analysis of Illinois data, 1987-2014.

BACKGROUND: The disease burden due to heat-stress illness (HSI), which can result in significant morbidity and mortality, is expected to increase as the climate continues to warm. In the United States (U.S.) much of what is known about HSI epidemiology is from analyses of urban heat waves. There is limited research addressing whether HSI hospitalization risk varies between urban and rural areas, nor is much known about additional diagnoses of patients hospitalized for HSI. METHODS: Hospitalizations in Illinois for HSI (ICD-9-CM codes 992.x or E900) in the months of May through September from

Spatial evaluation and modeling of Dengue seroprevalence and vector density in Rio de Janeiro, Brazil.

BACKGROUND: Rio de Janeiro, Brazil, experienced a severe dengue fever epidemic in 2008. This was the worst epidemic ever, characterized by a sharp increase in case-fatality rate, mainly among younger individuals. A combination of factors, such as climate, mosquito abundance, buildup of the susceptible population, or viral evolution, could explain the severity of this epidemic. The main objective of this study is to model the spatial patterns of dengue seroprevalence in three neighborhoods with different socioeconomic profiles in Rio de Janeiro. As blood sampling coincided with the peak of deng
