Avoiding the Tragedy of the Commons in Health Care: Policy Options for Covering High-Cost Cures.

In this article, Mattke and his colleagues discuss the risk that strategic behavior by health insurers could unravel the market for curative therapies for chronic diseases. Because the cost of these cures is front-loaded but the benefits accrue over time, insurers might attempt to delay treatment or avoid patients who require it, in the hope that they might change insurers. The authors discuss policy options to remedy this potential free-rider problem through alignment of incentives at the patient level, coordination among payers, and government intervention. They present a framework to analyz

Fragile X Syndrome: A Rare Case Report with Unusual Oral Features.

Fragile X syndrome (FXS) is a rare variant of special health-care need demonstrating delayed developmental milestones and associated with intellectual and emotional disabilities ranging from learning problem to mental retardation. The syndrome is usually not diagnosed until 8-9 years of age since the clinical manifestations of the syndrome are greatly attenuated in childhood. The physical characteristics such as facial features, hyperactivity, attention deficit, autistic behavior, and macroorchidism are quite evident in younger age group. The most typical orofacial characteristics associated w

Workplace violence: impact, causes, and prevention.

OBJECTIVES: Using a variety of sources, the author explores the complex reasons for aggression and violence in workplace settings, as well as suggesting means of prevention and intervention. METHODS: Literature Review. RESULTS: Studies indicate workplace violence affects more than half of U.S. organizations, yet nearly 70 percent have no programs or policies to deal with this problem. Research indicates that aggressive behavior of a psychological nature often precedes physical violence in the workforce, yet employers regularly ignore warning signs even when reported by employees. Costs to U.

Non-verbal vocalizations, dementia and social interaction.

In later stages of Alzheimer's disease many people will engage in noise-making (screaming and other kinds of sounds), often experienced as interruptive by others. A problem with the noise-making is the difficulty in understanding the meaning of the noise. This study addresses two questions: to what extent is noise-making responsive to the ongoing interaction and is noise-making regarded as meaningless behavior by other participants? The analysis of selective examples shows that noises may be fitted into the conversational interaction to a certain degree and in some instances is also responsive

The effects of obstructive sleep apnea syndrome due to adenotonsillar hypertrophy on the cardiovascular system in children.

Obstructive sleep apnea syndrome (OSAS) due to adenotonsillar hypertrophy (ATH) is a common and important problem in children. OSAS can lead to significant cardiopulmonary complications, poor growth and problems with learning and behavior. Many studies in the literature show that OSAS due to ATH causes pulmonary hypertension, ventricular hypertrophy and systemic hypertension in the pediatric population. In this review, we discuss the effects of ATH on cardiac function. It is well known that as a child grows, the nasopharyngeal passage becomes enlarged, helping to improve OSAS. Based on this, w

Rapid training of a community job skill to nonvocal adults with autism: an extension of intensive teaching.

We evaluated an intensive program in a simulated format for rapidly teaching a job skill to nonvocal adults with autism. Following baseline probes with a new work task of assembling mailing boxes at a publishing company, 3 supported workers individually received repeated teaching sessions at a simulated work site. All workers met criterion with 1 day of simulation teaching, with subsequent criterion level performance upon returning to the job (1 worker required booster trials). Intensive teaching did not occasion problem behavior nor unhappiness indices. Improved work performance also occurred…

Optimal gynecologic and obstetric care for lesbians.

Lesbians, like other marginalized groups of women, underutilize health care services. Lesbians also present later for health care than heterosexual women. Lack of awareness of the health issues of lesbians by some health care professionals has produced lesbians' abstention from health services. After defining lesbianism, I discuss how homophobia is a public health problem. Health issues of lesbians and practical recommendations for providing optimal gynecologic and obstetric care are presented on the subjects of adolescence, sexual identity, behavior and practice, sexually transmitted diseases

Sundown syndrome in severely demented patients with probable Alzheimer's disease.

A retrospective review of 71 patients with probable Alzheimer's disease was analyzed with respect to nursing evaluations of sundowning status (recurring confusion or agitation in the late afternoon or early evening). The prevalence of sundowning (including probable sundowners) was 24%. Sundowners and non-sundowners differed with regard to number of sedatives received daily, particularly chloral hydrate, and the number of days on the inpatient unit. There were no differences between sundowners and non-sundowners with respect to other types of medications, medical diagnoses, current age, age of

Social influence, marriage, and the heart: cardiovascular consequences of interpersonal control in husbands and wives.

Human and experimental animal research suggests that social stress in general--and chronic, effortful attempts to exert social control in particular--may contribute to cardiovascular disease. We examined the effects of exerting social influence or control on cardiovascular responses in married couples. Compared to husbands discussing a problem with their wives, husbands attempting to influence or persuade their wives displayed larger increases in systolic blood pressure (SBP) before and during the discussion. Furthermore, these physiological effects were accompanied by increases in anger and a

Accuracy of weightbearing estimation by stroke versus healthy subjects.

This study was performed to describe and compare the accuracy of weightbearing at three target levels (25, 50, and 75% of body weight) of 14 ambulatory stroke subjects and 14 matched healthy subjects. Weightbearing through a designated lower extremity (stroke subjects-paretic, healthy subjects-randomly selected) was measured with digital scales. No significant difference was found in magnitude of weightbearing (%) between the stroke and healthy groups. Although making significantly greater errors in weightbearing than healthy subjects, stroke subjects did not consistently weightbear under targ

Parental attitudes toward adolescent sexuality: transcultural perspectives.

The problem of teenage pregnancy continues to impact private and public resources, affecting all socioeconomic and cultural groups. A key factor for nurse practitioners to consider when planning sex education programs is the differing parental attitudes toward teenage sexuality. These attitudes are especially important to keep in mind when dealing with parents from minority cultural groups, as these groups are often highly influential in determining the nature of adolescent sexual behavior and attitudes toward reproduction. A study of Cuban and Haitian child-rearing practices clearly demonstra

Writing Abilities in Compulsive Prisoners.

Research has found links between academic failure and criminal offending and suggest that many incarcerated young people have experienced significant behavioral and learning problems in school, which could result in criminal outcomes and poor academic performance. The objective of this study was to analyse writing disorders in impulsive and compulsive prisoners. The sample was composed of 194 male prisoners, of which 81 had been diagnosed with Antisocial Personality Disorder and 113 with Obsessive Compulsive Personality Disorder. Male participants were recruited at the Granada Prison Center. T

Internet addiction: reappraisal of an increasingly inadequate concept.

This article re-examines the popular concept of Internet addiction, discusses the key problems associated with it, and proposes possible alternatives. The concept of Internet addiction is inadequate for several reasons. Addiction may be a correct designation only for the minority of individuals who meet the general criteria for addiction, and it needs to be better demarcated from various patterns of excessive or abnormal use. Addiction to the Internet as a medium does not exist, although the Internet as a medium may play an important role in making some behaviors addictive. The Internet can no

Does self-neglect occur among older adults with dementia when unsupervised in assisted living? An exploratory, observational study.

The phenomenon of older adults with dementia who develop behavioral expressions when they are unsupervised in assisted living residences is understudied. This qualitative study aimed to bridge this gap in the literature by focusing on 12 residents in various stages of dementia. Grounded Theory was followed to guide data collection and analysis. Data were collected in two special care units of an assisted living residence for 10 months. Participant observation was the primary data collection strategy. Semistructured interviews with staff and managers and review of clinical records augmented the

Health and safety at work in the transport industry (TRANS-18): factorial structure, reliability and validity.

In this article, we study the psychometric properties of a short scale (TRANS-18) which was designed to detect safe behaviors (personal and vehicle-related) and psychophysiological disorders. 244 drivers participated in the study, including drivers of freight transport vehicles (regular, dangerous and special), cranes, and passenger transport (regular transport and chartered coaches), ambulances and taxis. After carrying out an exploratory factor analysis of the scale, the findings show a structure comprised of three factors related to psychophysiological disorders, and to both personal and ve

Perceived threats of female infidelity, male proprietariness, and violence in college dating couples.

According to evolutionary theory, men faced the adaptive problem of keeping their partners sexually faithful. In the current study, men's perception that their partners are interested in other men was hypothesized to be associated with more controlling behaviors in romantic relationships. Of the two dimensions of mating tactics, controlling and attentive behaviors, physically aggressive men were expected to be especially controlling, but not attentive. Using structural equation modeling with 116 college dating couples, women who reported greater interest in other men were perceived by their pa

Issues regarding insurance and other third-party reimbursement for behavioral management procedures.

Issues related to reimbursement for time and effort expended in providing behavior management services for children often result in confusion and frustration for the parent and practitioner. Multiple medical and dental insurance plans within a state or region frequently lack a common set of definitions that are fundamental in interpreting and applying contract language. Ambiguities and inconsistencies in state and federal programs and regulatory structures, as well as budget concerns, only exacerbate the problem. This paper reviews issues of third-party payment associated with dental care for

Physical and sexual abuse: risk factors for substance use among young Hungarian women.

The authors used a multistage stratified sampling method to conduct a cross-sectional survey of a nationally representative sample of 3,615 Hungarian women aged 15 to 24 years to acquire epidemiologic data on physical and sexual abuse and analyze the relationship between abuse and health-risk behaviors. Just over 30% of the young women reported having experienced some type of abuse in their lifetimes. All maladaptive coping strategies, especially sedative use, were more prevalent among those who had experienced physical abuse. The prevalence of smoking, drinking alcohol, and experimenting with
