Relationship of tonsil size on an airway blockage maneuver in children during sedation.

A previous report suggested that airway compromise without self-correction may occur in pediatric dental patients sedated with chloral hydrate (CH) and nitrous oxide (N2O) and may be interpreted as "deep" sedation. The purpose of this institutionally approved study was to determine 1) the association between the size of the tonsils and 2) the degree of expired carbon dioxide (CO2) and oxygen saturation (SaO2) changes to simulated airway obstruction using the Moore head-tilt maneuver for 30 sec or less. Thirty healthy children (ASA I), aged 22-40 months, were evaluated for tonsil size and sedat

The laryngeal mask airway for awake diagnostic bronchoscopy. A retrospective study of 200 consecutive patients.

The laryngeal mask airway (LMA) provides a view of the larynx and moving vocal cords without loss of airway control and can be used in flexible fibreoptic bronchoscopy for both anaesthetized and awake patients. In this retrospective review of 200 consecutive patients over a 30 month period, bronchoscopy was successful via the LMA in all but one patient using a technique of topical anaesthesia and sedation. The LMA directs the fibrescope to the glottis, allows respiratory function to be monitored and oxygen to be given. Complication rates were similar to those reported for transnasal awake bron

Chronic pressure-overload hypertrophy attenuates vortex formation time in patients with severe aortic stenosis and preserved left ventricular systolic function undergoing aortic valve replacement.

OBJECTIVE: Transmitral blood flow produces a vortex ring that enhances the hydraulic efficiency of early left ventricular (LV) filling. The effect of pressure-overload hypertrophy on the duration of LV vortex ring formation (vortex formation time [VFT]) is unknown. The current investigation tested the hypothesis that chronic LV pressure-overload hypertrophy produced by severe aortic stenosis (AS) reduces VFT in patients with preserved LV systolic function undergoing aortic valve replacement. DESIGN: Observational study. SETTING: Veterans Affairs Medical Center. PARTICIPANTS: After the Insti

Ketamine for analgosedative therapy in intensive care treatment of head-injured patients.

Ketamine was supposed to be contra-indicated in head injured patients although it possesses numerous advantages over other commonly used analgosedative drugs. Referring to these potential advantages and the lack of definitive data about its effect upon ICP, CPP or neurological development, we conducted a prospective study in which moderate or severely head injured patients (n = 35) were prospectively allocated to receive treatment either with a combination of ketamine or midazolam or fentanyl and midazolam. The initial dose was 6.5 mg/kg/day midazolam, 65 mg/kg/day ketamine or 65 micrograms/kg

[Total intravenous anesthesia. On the way to standard practice in pediatrics].

Since venous cannulation in children has become easier and extensive experience has been gained with total intravenous anaesthesia (TIVA) in adults, the interest in TIVA for children has recently increased. An intensified sensitivity of the operating room atmosphere to contamination with volatile anaesthetic agents is another important reason to choose intravenous techniques for paediatric anaesthesia. One of the most interesting agents for TIVA in paediatric anaesthesia is propofol. The pharmacokinetic and pharmacodynamic data for modern intravenous drugs is poor. Because the interpatient var

Iatrogenic Horner syndrome following internal jugular vein catheterization in an infant: a case report and literature review.

INTRODUCTION: Horner syndrome (HS) is a rare but recognized complication of internal jugular vein (IJV) catheterization in children, resulting from injury to the cervical sympathetic pathway. Neonates and infants are particularly vulnerable due to the compact anatomy of the neck. PRESENTATION OF THE CASE: We report a 38-day-old female infant who developed ipsilateral HS following open surgical placement of a Broviac catheter via the right IJV. The catheter was inserted under local anesthesia with sedation for prolonged intravenous therapy. Within 24 hours postoperatively, the patient presente

Perioperative management after transcatheter aortic valve replacement and postoperative delirium assessment: a nationwide Italian survey.

BACKGROUND: Patients undergoing Transcatheter Aortic Valve Replacement (TAVR) are at risk for delirium. In Italy, the perioperative management of TAVR lacks standardization: this survey aims to investigate TAVR management across the country, focusing on delirium. METHODS: A multiple-choice survey was conducted anonymously among 40 high-volume cardiac surgery centers in Italy: questions addressed logistics, surgical procedures, anesthetic management and delirium assessment. RESULTS: The response rate was 68%. Low-volume centers were excluded, resulting in a final sample of 22 hospitals, inclu

Distal nerve blocks at the elbow for traumatic fingertip semi-amputation repair - A case report.

Regional anesthesia has been proven useful in hand surgery after trauma, most commonly via brachial plexus blocks (BPB), with widely established benefits. Distal nerve blocks at the elbow are used much less frequently. A 15-year-old boy was admitted because of traumatic amputation of the 4th digit on his right hand. Difficult intubation was suspected. History was indicative of obstructive sleep apnea, and nil per os status was confirmed, with no apparent risk factors for regurgitation. Ultrasound-guided blocks of the median, radial, and ulnar nerves at the elbow were performed with 2% Lidocain

Diagnostic yield of thoracoscopic pleural biopsy in undiagnosed exudative pleural effusion and comparative analysis of pleural fluid adenosine deaminase levels.

Undiagnosed exudative pleural effusion remains a frequent clinical challenge, particularly in high-burden settings where tuberculosis and malignancy predominate. Pleural fluid adenosine deaminase (ADA) is useful for suggesting tuberculous pleural effusion (TPE), but a substantial "gray zone" overlaps with malignant pleural effusion (MPE). This study evaluated the diagnostic yield of medical thoracoscopy with comparative analysis of pleural fluid ADA levels. This prospective, single-center, exploratory observational study was conducted over 1.5 years in a tertiary care hospital of north India.

Experimental implementation of the peripheral nerve block clinical registry: an observational study.

BACKGROUND AND AIMS: Peripheral nerve block (PNB) is commonly used, but there is a lack of data on its effectiveness and safety in the clinic. Therefore, anesthesiologists have limited insight into how they are faring in terms of both safety and efficacy. Additionally, No PNB registry is recorded in IRAN. Herein, we reveal how a hospital might use information gathered in a hospital registry of PNB outcomes to guide future quality enhancement efforts. METHOD: This was an observational, prospective, and unicenter study of all peripheral nerve blocks done in the operating room since December 22,

Simultaneous Use of Dual Bronchoscopes for Targeted Biopsy of Peripheral Lung Lesions: The Kissing Probe Technique.

Background: Peripheral pulmonary lesions (PPLs) are increasingly detected due to widespread use of cross-sectional imaging and lung cancer screening. While cone-beam CT and robotic bronchoscopy have improved diagnostic accuracy, they remain resource-intensive and inaccessible in many settings. A novel technique employing simultaneous use of two bronchoscopes referred to as the "Kissing Probe Technique" was developed to provide real-time lesion localization and precise sampling using standard equipment. Methods: This retrospective, single-centre study included 43 patients with radiologically co

Effectiveness of prophylactic push-dose vasopressors for preventing arterial hypotension during elective orotracheal intubation: A systematic review and meta-analysis.

BACKGROUND: Orotracheal intubation is frequently associated with arterial hypotension, particularly during induction with agents such as propofol. The use of prophylactic push-dose vasopressors (Push-Dose Pressor - PDP) has been proposed to mitigate this drop in blood pressure. However, the effectiveness of PDPs remains uncertain. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) assessing the use of PDPs (epinephrine, ephedrine, phenylephrine, or metaraminol) during sedation for tracheal intubation in adult normotensive patients.
