Less Invasive Surfactant Administration: A Viewpoint.

The standard of care in treating respiratory distress syndrome in preterm infants is respiratory support with nasal continuous positive airway pressure or a combination of continuous positive airway pressure and exogenous surfactant replacement. Endotracheal intubation, the conventional method for surfactant administration, is an invasive procedure associated with procedural and mechanical ventilation complications. The INSURE (intubation, surfactant administration, and extubation soon after) technique is an accepted method aimed at reducing the short-term complications and long-term morbiditi

[EDNOS is an eating disorder of clinical relevance, on a par with anorexia and bulimia nervosa].

BACKGROUND: The category 'eating disorder 'not otherwise specified'' (EDNOS) in DSM-IV is restricted to eating disorders of clinical severity that do not completely fulfil the criteria for anorexia and bulimia nervosa. The EDNOS category is, by definition, often regarded as a a residual category and in principle designed to incorporate a small group of patients with atypical characteristics. Health insurance companies argue that the treatment of patients diagnosed with EDNOS should not be treated in mental health institutions and therefore should not get their treatment costs reimbursed by the…

Understanding antihypertensive medication use after living kidney donation through linked national registry and pharmacy claims data.

BACKGROUND: Use of antihypertensive medications (AHM) after living kidney donation is not well described. METHODS: We examined a database wherein national transplant registry data for 4,650 living kidney donors in 1987-2007 were linked to pharmacy claims from a US private health insurer (2000-2007 claims) to identify post-donation AHM fills. Cox regression with left- and right-censoring was used to estimate the frequencies and relative likelihood (adjusted hazards ratios, aHR) of post-donation AHM fills according to donor demographic traits. Medication possession ratio (MPRs), defined as (day

Evaluating dispensing patterns of Parkinson's medications from 2012 to 2024 in Australia: insights from the Pharmaceutical Benefits Scheme concessional database.

INTRODUCTION: Parkinson's disease is a neurodegenerative disorder that affects patients' lives by deteriorating their motor and non-motor functions through the loss of dopaminergic neurons. Medications remain the preferred option to manage Parkinson's disease symptoms. In Australia, the Pharmaceutical Benefits Scheme (PBS) supports access to essential medicines by subsidising their cost, making long-term treatment more affordable. AIM: This study analyses the dispensing patterns of Parkinson's medications in Australia from 2012 to 2024, focusing on utilisation trends over time and shifts in p

Section 1115 Substance Use Disorder Waivers: Opportunities and Limitations.

In 2022, 48.7 million people in the United States (17.3% of the population aged 12 or older) met the criteria for substance use disorder (SUD). Nearly 40% of people with opioid use disorder (OUD) are Medicaid recipients, making Medicaid the largest single source of OUD treatment insurance coverage. Despite this crucial importance, two major barriers to expanding access to treatment for persons with SUD are baked into the program: the institutions for mental diseases (IMD) exclusion and the Medicaid inmate exclusion. This article first provides a timeline of these two waiver reforms to illustra…

National guideline development and adaptation, Egypt.

PROBLEM: In 2018, the Egyptian government enacted a universal health coverage law mandating provision of high-quality, effective and equitable health care. Achieving this mandate would require expanding availability and use of evidence-based national clinical practice and public health guidelines. APPROACH: In 2022, Egypt's Ministry of Health and Population requested support from the World Health Organization (WHO) to establish a national guideline development and adaptation programme. To inform the process, WHO conducted a situation analysis. The findings led to a collaboration between WHO a

From regulation to real legal cases: physician-patient disputes as a lens on universal health coverage governance in China, 2015-2025.

Physician-patient disputes (PPD) have been increasingly recognized as structural indicators of health system performance, yet they remain underexplored within universal health coverage (UHC) frameworks, particularly in the Chinese context. This study employed a mixed-methods approach integrating regulatory review and legal cases analysis, revealing that the regulatory framework for PPD has evolved from fragmented arrangements toward a more integrated civil, criminal, and administrative system, encompassing three stages: ex ante prevention, in-process management, and ex post dispute resolution
