Motor and non-motor improvements following short-term multidisciplinary day-clinic care in Parkinson´s disease.

BACKGROUND: Inpatient as well as outpatient care does often not meet PD-patients' individual needs. INTRODUCTION: Day-clinic concepts encompassing a multidisciplinary team as well as therapy adjustments accompanying everyday demands aim at filling this gap. METHODS: This is a retrospective study on short-term effects of a 3 week multidisciplinary rehabilitation program in patients with Parkinson´s disease (PD) embedded in a specialized movement disorder day-clinic. We analyzed short-term outcome of motor and non-motor symptoms (NMS) in 143 PD-patients (mean age 65.3 ± 11.9 years; Hoehn-and-Y

"Using the same hand": The complex local perceptions of integrated one health based interventions in East Africa.

BACKGROUND: Neglected Tropical Diseases (NTDs) such as soil transmitted helminths (STH) and human rabies represent a significant burden to health in East Africa. Control and elimination remains extremely challenging, particularly in remote communities. Novel approaches, such as One Health based integrated interventions, are gaining prominence, yet there is more to be learned about the ways in which social determinants affect such programmes. METHODOLOGY: In 2015 a mixed method qualitative study was conducted in northern Tanzania to determine community perceptions towards integrated delivery o

Predictive Factors of Neutropenia in HIV-Infected Patients with Malignancy Receiving Chemotherapy or Radiotherapy.

OBJECTIVES: Rising health care costs necessitate value-based health care (VBHC) initiatives. Large hospital systems offer a platform for surgeon collaboration in developing standardized, interdisciplinary orthopedic care. This study evaluates the formation, implementation, and outcomes of the Musculoskeletal Quality Collaborative (MSKQC) across a hospital system. METHODS: Established in 2019, the MSKQC comprises physician, quality, and nursing leaders from 14 hospitals. Using the Delphi method, surgeons identified VBHC interventions, leading to protocols for antibiotic-infused bone cement (AI

Surgical Decision-Making for Temporal Lobe Epilepsy: Patient Experiences of the Informed Consent Process.

Background: Surgical resection is frequently the recommended treatment for drug-resistant temporal lobe epilepsy (TLE), yet many factors play a role in patients' perceptions of brain surgery that ultimately impact decision-making. The purpose of the current study was to explore how people with epilepsy, in their own words, experienced the overall process of consenting to surgery for drug-resistant TLE. Methods and Materials: Data was drawn from in-person, semi-structured interviews of 19 adults with drug-resistant TLE eligible to undergo epilepsy surgery. A systematic thematic analysis was per

VarioGuide® frameless neuronavigation-guided stereoelectroencephalography in adult epilepsy patients: technique, accuracy and clinical experience.

BACKGROUND: Stereoelectroencephalography (SEEG) allows the identification of deep-seated seizure foci and determination of the epileptogenic zone (EZ) in drug-resistant epilepsy (DRE) patients. We evaluated the accuracy and treatment-associated morbidity of frameless VarioGuide® (VG) neuronavigation-guided depth electrode (DE) implantations. METHODS: We retrospectively identified all consecutive adult DRE patients, who underwent VG-neuronavigation DE implantations, between March 2013 and April 2019. Clinical data were extracted from the electronic patient charts. An interdisciplinary team agr

Machine Learning for Subtyping Concussion Using a Clustering Approach.

Background: Concussion subtypes are typically organized into commonly affected symptom areas or a combination of affected systems, an approach that may be flawed by bias in conceptualization or the inherent limitations of interdisciplinary expertise. Objective: The purpose of this study was to determine whether a bottom-up, unsupervised, machine learning approach, could more accurately support concussion subtyping. Methods: Initial patient intake data as well as objective outcome measures including, the Patient-Reported Outcomes Measurement Information System (PROMIS), Dizziness Handicap Inven

Dysphagia in cervical spinal cord injury: How international literature trends can guide South African practice patterns - A scoping review.

BACKGROUND: The limited data regarding dysphagia in high-level spinal cord injuries (SCIs) stem from economically developed countries. Dysphagia is prevalent in patients with cervical SCI; however, in a South African context, speech-language pathologists (SLPs) are not seen as key when managing this population. This may result in patients not being screened or identified early, leading to possible complications. The literature could provide useful insight on how best to address this clinical gap. OBJECTIVES: The aim of my study was to conduct a scoping review on the description of dysphagia,

Parental perceptions and expectations concerning speech therapy-related cleft care - a qualitative study.

AIMS: Speech (i.e., resonance, nasal airflow and articulation) is one of the primary outcomes in individuals with a cleft of the palate with or without a cleft of the lip (CP ± L). Therefore, it is highly important to obtain information regarding parental perceptions and expectations concerning speech therapy-related cleft care. Literature investigating these parental perspectives is scarce. The present study investigated perceptions and expectations of parents of children with CP ± L concerning (outcomes of) speech therapy. METHODS: Eleven parents of nine children with CP ± L were recruited

Contextual influences on the perception of pregnant women who use drugs: Information about women's childhood trauma history reduces punitive attitudes.

Punitive attitudes and consequences (e.g., incarceration) for prenatal illicit drug use persist in the United States despite evidence that these policies are ineffective and even harmful to women and children. For instance, the threat of these consequences can deter women from seeking healthcare, prenatal care, and drug treatment. Punitive responses may persist due to pejorative public perceptions of pregnant women who use illicit drugs. Although there is evidence that contextual information about prenatal drug use (e.g., drug type) can change such perceptions, other contextual influences are

Combined Optokinetic Treatment and Vestibular Rehabilitation to Reduce Visually Induced Dizziness in a Professional Ice Hockey Player After Concussion: A Clinical Case.

Background: The appropriate detection and therapy of concussion symptoms are of great importance to avoid long-term impairment and absence from pre-concussive activities, such as sport, school or work. Post-traumatic headache and dizziness are known as risk factors of persistent symptoms after a concussion. Dizziness has even been classified as a predictor for symptom persistence. One type of dizziness, which has never been considered is visually induced dizziness (VID) often develops as a consequence of vestibular impairment. This manuscript presents the clinical case of a 25-year-old male, p

Increasing the Local Relevance of Epidemiological Research: Situated Knowledge of Cattle Disease Among Basongora Pastoralists in Uganda.

Cattle disease can have severe negative impacts on the livelihoods of the poor, but still, animal disease management and outreach often remain suboptimal in low-income settings. In a study on Basongora pastoralists in Uganda, we examined local priorities, perceptions and practices regarding cattle disease, in order to improve outreach and disease control advisory work in such contexts. We also investigated how participatory epidemiology can be better equipped for gathering situated knowledge. Empirical material obtained in focus group discussions, interviews, participatory mapping, and wealth-
