Evidence for localized release of substance P within rat spinal cord evoked by physiological and electrical stimuli.

Antibodies immobilized onto the outer surface of glass microelectrodes were used to measure and localize substance P (SP) release in the spinal cords of anaesthetized rats. Utilizing a C-terminally directed antibody, significant levels of SP were not found in the lumbar spinal cord in the absence of peripheral noxious stimulation. Following noxious heating or pinch of the ipsilateral hind paw or electrical stimulation of the ipsilateral tibial nerve at C-fibre strength, significant amounts of released SP were detected. This noxious stimulus-evoked release of SP was primarily in the region of t

Differential effects of propofol and nitrous oxide on posterior tibial nerve somatosensory cortical evoked potentials during alfentanil anaesthesia.

We have studied differential effects of propofol and nitrous oxide on posterior tibial nerve somatosensory evoked potentials (PTN-SEP) during a continuous infusion of alfentanil. In study 1, 14 patients received initially 66% nitrous oxide in oxygen, which was replaced 90 min after incision by propofol 6 mg kg-1 h-1. This substitution resulted in a significant increase in mean P1N1 amplitude from 1.01 (SD 1.14) microV to 2.61 (2.17) microV (P less than 0.01), while amplitudes of later peaks were unaffected. Latencies of peaks N1, P2 and N2 increased after the substitution. In study 2, 30 patie

Intrathecal clonidine: analgesia and effect on opiate withdrawal in the rat.

Clonidine, an alpha 2 adrenergic agonist, has analgesic properties and recently has been used to suppress opiate withdrawal. These two properties theoretically make it a suitable analgesic substitute in patients tolerant to opioids. The objectives of this study were to see if intrathecal clonidine is analgesic and whether it can modify morphine withdrawal at the spinal level. Rats chronically implanted with catheters in the lumbar subarachnoid space were utilized. In analgesia experiments, intrathecal clonidine produced analgesia with the peak effect in the paw-lick test occurring at 200nM, an

Quantification of intraoperative somatosensory evoked potential.

Cortical (C) and spinal (S) somatosensory evoked potentials (SEP) were measured and quantified in 30 patients undergoing scoliosis surgery during the following periods: I, preincision; II, hypotension; III, instrumentation; IV, postinstrumentation; and V, skin closure. Paired two-tailed t tests were performed on all commonly measured SEP parameters comparing values obtained at periods I and II with each subsequent period. CSEP were obtained with Cz-Fz recording sites of the international 10-20 system while SSEP were obtained with recording electrodes at C7-Fz, following bilateral posterior tib

The Role of Erector Spinae Block in Urological Surgeries as a Promising Alternative to Conventional Analgesia: A Prospective Observational Study.

Introduction: The erector spinae plane block (ESPB) is a regional anesthesia technique that provides somatic and visceral analgesia by targeting the dorsal and ventral rami of the spinal nerves. It is performed under ultrasound guidance. ESPB is particularly beneficial in urological surgeries, which often require multimodal analgesia due to complex pain management needs and patient comorbidities. However, its efficacy in urological procedures remains insufficiently studied. Therefore, our aim was to assess the effectiveness and advantages of ESPB in urological procedures. Material and Methods:

The transversus abdominis plane block in conjunction with intrathecal morphine use after cesarean section in women with severe pre-eclampsia: a randomized controlled trial.

BACKGROUND: The transversus abdominis plane (TAP) block in conjunction with intrathecal morphine has been demonstrated to provide more superior postcesarean analgesia to intrathecal morphine alone. However, the analgesia efficacy of their conjunction has not been demonstrated in patients with severe pre-eclampsia. The study aimed to compare the postcesarean analgesia of TAP block in conjunction with intrathecal morphine versus intrathecal morphine alone in women with severe pre-eclampsia. METHODS: Pregnant women with severe pre-eclampsia undergoing planned cesarean section were randomly alloc

To Compare the Effects of Different Doses of Dexmedetomidine on Intrathecal Bupivacaine in Infraumbilical Surgeries: A Prospective, Randomized, Double-blind Clinical Study.

INTRODUCTION: Spinal anesthesia is preferred technique of choice in infraumbalical surgeries. Limitation of this technique is shorter duration of analgesia, so various adjuvants have been used with intrathecal bupivacaine such as fentanyl, clonidine, and dexmedetomidine. Dexmedetomidine is a highly selective alpha 2 adrenergic agonist. The aim of our study was to know the effect of different doses of dexmedetomidine on intrathecal bupivacaine. MATERIALS AND METHODS: The prospective, randomized, double-blind study was conducted in tertiary health care center, on ninety patients of the American

Endovenous laser with miniphlebectomy for treatment of varicose veins and effect of different levels of laser energy on recanalization. A single center experience.

Varicose veins, associated with great saphenous vein (GSV) incompetence, are traditionally treated with conventional surgery. In recent years, minimally invasive alternatives to surgical treatment such as the endovenous laser ablation (EVLA) and radiofrequency (RF) ablation have been developed with promising results. Residual varicose veins following EVLA, regress untouched, or phlebectomy or foam sclerotherapy can be concomitantly performed. The aim of the present study was to investigate the safety and efficacy of EVLA with different levels of laser energy in patients with varicose veins sec

Feasibility of radiosurgery for patients with spinal tumors treated in lateral decubitus position: A case series from Memorial Sloan Kettering Cancer Center.

INTRODUCTION: Often in clinical practice radiation oncologists encounter patients who require treatment to the spine commonly in the setting of metastatic disease. These metastases usually cause pain, immobility, or neurologic deficits mandating expedited therapy to alleviate the suffering of our patients. Spine radiosurgery techniques have been used extensively for palliation purposes; however, given the patients' deteriorating condition or pain and inability to tolerate anesthesia the radiation oncologist is often left with the conundrum of how to best set up his or her patient in preparatio

Low-Dose Subarachnoid Anesthesia Combined with PENG and FLCN Blocks Reduces Hypotensive Episodes Without Compromising Anesthetic Depth and Duration in Hip Fracture Surgery: A Retrospective Observational Study.

Background and Objectives: In the context of hip fracture surgeries, episodes of hypotension are common, and have been associated in various studies with increased complications and mortality. The latest clinical guidelines recommend close hemodynamic management. Our research team hypothesized that the use of peripheral nerve blocks in this surgery could help adjust the doses of subarachnoid anesthesia for these procedures, thereby limiting the hypotensive episodes, without compromising an adequate depth and duration of intraoperative anesthesia. Materials and Methods: A retrospective study of

Correlation of preoperative fibrinogen/albumin ratio with morbidity following advanced-age hip fractures: an observational study.

PURPOSE: Given the occurrence of comorbidities in geriatric patients, the postoperative period of hip fractures may progress with high morbidity and mortality. Recently, several inflammatory markers have been used to evaluate the treatment course. Herein, we prospectively followed-up and examined the relationship between preoperative fibrinogen/albumin ratio(FAR) and morbidity/mortality in elderly patients with hip fracture. METHODS: Patients aged ≥ 85 years who underwent hip fracture surgery under unilateral spinal anesthesia were included in this prospective observational study. The patient

Accidental dural puncture, postdural puncture headache, intrathecal catheters, and epidural blood patch: revisiting the old nemesis.

One of the controversial management options for accidental dural puncture in pregnant patients is the conversion of labor epidural analgesia to continuous spinal analgesia by threading the epidural catheter intrathecally. No clear consensus exists on how to best prevent severe headache from occurring after accidental dural puncture. To investigate whether the intrathecal placement of an epidural catheter following accidental dural puncture impacts the incidence of postdural puncture headache (PDPH) and the subsequent need for an epidural blood patch in parturients. A retrospective chart review

Secondary spread of caudal block as assessed by ultrasonography.

BACKGROUND: Redistribution and secondary spread after the initial injection of local anaesthetics (LAs) are important factors that contribute to the final spread of caudal block in children. However, to date, these phenomena have yet not been studied in detail. Thus, the aim of this observational study was to define patterns of secondary spread and redistribution of a caudal block by means of real-time ultrasonography scanning and cutaneous testing. METHODS: Ultrasound assessment of LA spread within the caudal-epidural space and epidural pressure was followed during 15 min after initial injec

[Spinal anaesthesia in day case surgery--Old technique--new trends].

Day case surgery is becoming more and more important. In order to perform these services cost-efficiently it is of primary importance to ensure that procedures can be scheduled with the largest possible patient satisfaction. Up to now spinal anaesthesia was of little importance in day case surgery due to prolonged nerve block and negative side effects especially when using long acting local anaesthetics. Since prilocaine and 2-chloroprocaine, two short acting local anaesthetics with a known low incidence of side effects, were recently introduced into clinical practice a reevaluation of spinal
