Distal biceps repair using the lacertus fibrosus as a local graft.

BACKGROUND: We report a case series of 15 patients who underwent a new surgical technique that uses a transfer of the lacertus fibrosus to augment the repair of the distal biceps tendon. This technique seeks to minimize the need for and potential complications associated with autogenous or allograft tendon reconstruction in this clinical scenario. METHODS: We reviewed the clinical outcomes of patients who both underwent a lacertus transfer for biceps tendon reconstruction during a 10-year period and had at least 6 months of follow-up. Their clinical outcomes, including success of tendon repai

A systematic review of outcomes of contralateral C-7 transfer for the treatment of traumatic brachial plexus injury: an international comparison.

OBJECTIVE The effectiveness of contralateral C-7 (CC7) transfer is controversial, yet this procedure has been performed around the world to treat brachial plexus injuries. The authors performed a systematic review to study whether Asian countries reported better outcomes after CC7 transfer compared with "other" countries. METHODS A systematic literature search using PubMed, EMBASE, and 3 Chinese databases was completed. Patient outcomes of CC7 transfer to the median and musculocutaneous (MC) nerves were collected and categorized into 2 groups: Asia and "other" countries. China was included as

Partial joint denervation II: knee and ankle.

BACKGROUND: Partial joint denervation is the concept of preservation of joint function and relief of joint pain by interrupting neural pathways that transmit the pain message from the joint to the brain. Partial denervation of painful wrist, elbow, and shoulder joints was described in part I. Application of these principles to the knee and ankle is described in part II. METHODS: Cadaveric anatomical studies identified innervation of the knee and the ankle (sinus tarsi) to provide a guide to nerve blocks and surgical intervention. Patients were evaluated who had sports injuries, trauma, osteoa

[Arthroscopic surgery--current status and perspectives].

The morbidity after arthroscopic surgery is low; a hospitalization is not necessary in most patients. The possible operations of the knee joint include removal of loose bodies, resections on meniscus, plicae and synovium as well as more complicated procedures as suturing of a meniscus, total synovectomy and operations in patients with osteoarthritis. The advantage of the arthroscopic operation compared with arthrotomy is well documented in meniscal resection (shorter treatment, stay in the hospital and sick leave, reduced costs, and nevertheless excellent results) whereas the indication and cl

Biceps Tenodesis Versus Tenotomy in the Treatment of Lesions of the Long Head of the Biceps Tendon in Patients Undergoing Arthroscopic Shoulder Surgery: A Prospective Double-Blinded Randomized Controlled Trial.

BACKGROUND: The biceps tendon is a known source of shoulder pain. Few high-level studies have attempted to determine whether biceps tenotomy or tenodesis is the optimal approach in the treatment of biceps pathology. Most available literature is of lesser scientific quality and shows varying results in the comparison of tenotomy and tenodesis. PURPOSE: To compare patient-reported and objective clinical results between tenotomy and tenodesis for the treatment of lesions of the long head of the biceps brachii. STUDY DESIGN: Randomized controlled trial; Level of evidence, 1. METHODS: Patients a

Platelet-rich plasma application in the management of chronic tendinopathies.

Platelet-rich plasma (PRP) may represent a new therapeutic option for chronic tendinopathies. Platelets release various cytokines and growth factors which promote angiogenesis, tissue remodeling, and wound healing. We made an extended literature review of the use of PRP in chronic tendinopathies: epicondylitis, rotator cuff, patellar and calcaneal tendinopathies, and plantar fasciitis. Medline, Embase and Google Scholar were used (until July 31, 2012). Clinical studies on PRP and tendinopathies published in English and French language peer-reviewed journals were included. Management of intercondylar fractures of the humerus using the extensor mechanism-sparing paratricipital posterior approach.

Twenty two patients with intra-articular fractures of the distal humerus were operatively treated using the extensor mechanism-sparing paratricipital approach as described by Schildhauer et al. All fractures were AO type C (six AO type C1, 11 C2 and five C3). There were 16 males and six females with a mean age of 32.5 years. Internal fixation was achieved with bilateral plates and screws. The results were evaluated after a mean follow-up period of 30 months using Mayo Elbow Score. The results were graded as excellent in 13 patients (59.2%), good in six (27.3%), fair in two patients (9%) and po

Unstable elbow dislocations.

Seventeen patients with a posterior dislocation of the elbow and either no fracture or a minimal capsuloligamentous avulsion fracture were treated operatively for persistent redislocation after manipulative reduction. Fifteen had open reduction and ligament repair (3 with ancillary hinged external fixation), and 2 infirm patients had closed reduction and cross pinning of the elbow joint. Among the 12 patients treated with ligament repair alone, 1 redislocated (treated with a second surgery for hinged external fixation) and 4 had residual subluxation (treated with temporary casting of protected

Overuse injuries in female athletes.

The last three decades have witnessed a tremendous increase in female sports participation at all levels. However, increased sports participation of female athletes has also increased the incidence of sport-related injuries, which can be either acute trauma or overuse injuries. Overuse injuries may be defined as an imbalance caused by overly intensive training and inadequate recovery, which subsequently leads to a breakdown in tissue reparative mechanisms. This article will review the most frequent overuse injuries in female athletes in the context of anatomical, physiological, and psychologic

Clinical presentation and radiographic findings of distal biceps tendon degeneration: a potentially forgotten cause of proximal radial forearm pain.

For patients who present insidiously with proximal-radial forearm pain, the differential diagnosis should include distal biceps tendon degeneration. If radial tunnel and pronator syndromes, lateral epicondylitis, and radiocapitellar arthritis are eliminated as potential causes, tenderness over the proximal radius, supination weakness, and characteristic radiographic or magnetic resonance imaging findings may facilitate correct diagnosis of distal biceps tendon degeneration. In this article, we describe the clinical presentation and radiographic findings for 5 patients who presented with distal

The treatment of pulled elbow: a prospective randomized study.

To evaluate the effectiveness in decreasing recurrence of cast application after manual reduction of pulled elbow. Sixty-four children with pulled elbow were randomized into two treatment groups: Group A underwent manipulative reduction followed by splinting the elbow in a flexed and supinated position for 2 days; group B underwent manipulative reduction only. Both groups were examined 2, 5, and 10 days later. None of the 33 patients in group A had a pulled elbow at follow-up. Four (13%) of 31 patients in group B had a pulled elbow 2-5 days later. Immobilizing the elbow for 2 days after manipu
