Morbidity and mortality in children with obstructive sleep apnoea: a controlled national study.

BACKGROUND: Little is known about the diagnostic patterns of obstructive sleep apnoea (OSA) in children. A study was undertaken to evaluate morbidity and mortality in childhood OSA. METHODS: 2998 patients aged 0-19 years with a diagnosis of OSA were identified from the Danish National Patient Registry. For each patient we randomly selected four citizens matched for age, sex and socioeconomic status, thus providing 11 974 controls. RESULTS: Patients with OSA had greater morbidity at least 3 years before their diagnosis. The most common contacts with the health system arose from infections (OR

Isolated Medial Ankle Tenosynovitis of the Flexor Digitorum and Posterior Tibialis Tendons: A Case Report of the Role of Early Musculoskeletal Ultrasound in Identifying Anti-citrullinated Protein Antibody (ACPA)-Positive At-Risk Rheumatoid Arthritis.

Tenosynovitis is a recognized feature of rheumatoid arthritis (RA), yet it typically presents alongside symmetric polyarticular synovitis. Isolated focal tenosynovitis as the primary and initial manifestation of RA is uncommon and frequently leads to diagnostic delays. We present the case of a 62-year-old female who presented with localized right ankle pain in the absence of systemic joint involvement or elevated inflammatory markers. While the rheumatoid factor was negative and both the erythrocyte sedimentation rate and C-reactive protein levels were within normal limits, musculoskeletal ult

Recent advances in immunoregulatory biomaterials for tendon healing: From immune remodeling to functional regeneration.

Tendon injury is a prevalent musculoskeletal disorder characterized by delayed healing, functional impairment, and diminished quality of life. Tendon repair is a dynamic process driven by immune regulation of tissue remodeling. This review summarizes the immunological mechanisms underlying tendon healing. In the early inflammatory phase, neutrophils and M1 macrophages trigger tissue degradation through TNF-α and IL-1β mediated NF-κB and MAPK pathways. As healing progresses, M2 macrophages and regulatory T cells promote inflammation resolution and extracellular matrix reconstruction via IL-10 and

Polyunsaturated fatty acids in the prevention, treatment, and survival of breast cancer.

Breast cancer is one of the leading causes of death among women worldwide, and its occurrence is shaped by both nonmodifiable factors (such as age and genetics) and modifiable factors, among which diet plays a central role. This review addresses the role of polyunsaturated fatty acids (PUFAs) in the prevention, development, and treatment of breast cancer, highlighting their capacity to modulate inflammatory processes and impact mammographic density. Evidence indicates a negative association between n-3 PUFAs intake and breast cancer risk, whereas the role of n-6 PUFAs in disease development re

Exercise, Exerkines, and Sarcopenia.

Sarcopenia is a progressive age-related musculoskeletal disorder characterized by loss of skeletal muscle mass, strength, and physical function. Closely associated with aging and physical inactivity, it significantly compromises mobility, independence, and quality of life in older adults. Exercise is widely recognized as an effective non-pharmacological intervention for managing sarcopenia. However, the underlying molecular mechanisms remain incompletely elucidated. Among the emerging mediators, exerkines, physical exercise-induced signaling molecules, are secreted by multiple tissues, includi

Advanced Mechano-Electrical Coupling Biomaterials for Orthopedics: From Fundamental Mechanisms to Clinical Translation.

Musculoskeletal tissues are continuously subjected to mechanical stimulation during physiological activity, accompanied by the generation of endogenous bioelectrical signals. Such mechano-electrical coupling plays a pivotal role in tissue development, homeostasis, and repair. However, injury or pathological conditions often disrupt or weaken the local bioelectrical microenvironment, thereby limiting regenerative outcomes. Driven by the need to reconstruct the electrophysiological microenvironment, mechano-electrical coupling strategies based on piezoelectric, triboelectric, magnetoelectric, an

Enthesitis on Chip - A Model for Studying Acute and Chronic Inflammation of the Enthesis and its Pharmacological Treatment.

Enthesitis, the inflammation of the enthesis, which is the point of attachment of tendons and ligaments to bones, is a common musculoskeletal disease. The inflammation often originates from the fibrocartilage region of the enthesis as a consequence of mechanical overuse or -load and consequently tissue damage. During enthesitis, waves of inflammatory cytokines propagate in(to) the fibrocartilage, resulting in detrimental, heterotopic bone formation. Understanding of human enthesitis and its treatment options is limited, also because of lacking in vitro model systems that can closely mimic the

Amniotic membrane and/or umbilical cord tissue for treatment of facet joint syndrome: a narrative review.

Musculoskeletal spine disorders, especially low back pain, induce enormous amounts of stress and financial burden on individuals and healthcare systems throughout the world. Disorders of the facet joints in the lumbar spine are the most predominant cause of back pain, resulting in facet joint syndrome (FJS). Conventional treatments for FJS are short-lived and have limitations and side effects. Thus, safer and more effective alternatives that can reduce pain and improve patient-reported outcomes are needed. Recently, the utilization of biologics, including the ones derived from perinatal tissue

Intervertebral Disc-on-a-Chip as Advanced In Vitro Model for Mechanobiology Research and Drug Testing: A Review and Perspective.

Discogenic back pain is one of the most diffused musculoskeletal pathologies and a hurdle to a good quality of life for millions of people. Existing therapeutic options are exclusively directed at reducing symptoms, not at targeting the underlying, still poorly understood, degenerative processes. Common intervertebral disc (IVD) disease models still do not fully replicate the course of degenerative IVD disease. Advanced disease models that incorporate mechanical loading are needed to investigate pathological causes and processes, as well as to identify therapeutic targets. Organs-on-chip (OoC)

Metformin as a Treatment Strategy for Sjögren's Syndrome.

Sjögren's syndrome (SS), a chronic inflammatory disease involving the salivary and lacrimal glands, presents symptoms of sicca as well as systemic manifestations such as fatigue and musculoskeletal pain. Only a few treatments have been successful in management of SS; thus treatment of the disease is challenging. Metformin is the first-line agent for type 2 diabetes and has anti-inflammatory potential. Its immunomodulatory capacity is exerted via activation of 5' adenosine monophosphate-activated protein kinase (AMPK). Metformin inhibits mitochondrial respiratory chain complex I which leads to

Functional Impairments and Rehabilitation Outcomes of Patients With Immunotherapy-Induced Acute Inflammatory Demyelinating Polyradiculoneuropathy, Myasthenia Gravis, and Myositis.

Immunotherapy has led to a higher survival rate among different oncological disease groups but also associated with adverse-related events in multiple organ systems. Immunotherapy-related musculoskeletal weakness often results in a loss of cancer survivors' physical function, ultimately impacting their independence and quality of life. This is a retrospective study of 24 cancer patients who were treated with immunotherapy either alone or in conjunction with other oncological treatments. Twelve subjects (50%) were found to have acute inflammatory demyelinating polyradiculopathy/Guillain-Barré s

Effects of sunlight exposure and vitamin D supplementation on HIV patients.

Unlike many vitamins derived predominantly from food sources, vitamin D is produced endogenously in the skin upon exposure to sunlight. Ethnicity, skin pigmentation, socioeconomic status, geographic location, climate and sunscreen; all of these factors contribute to the amount of insolation for any given individual. Insufficient insolation creates the prerequisites for vitamin D deficiency. This is particularly true in HIV-infected individuals, who are highly vulnerable to vitamin D insufficiency/deficiency, as it plays a huge role in the musculoskeletal and cardiovascular systems. Antiretrovi
