Marginal validity of syndromic management for reproductive tract infections among pregnant women in Jamaica.

We assessed the validity of a syndromic case management approach for reproductive tract infections (RTIs) among 371 pregnant women attending antenatal care facilities in Kingston, Jamaica, using an algorithm previously validated in high-risk Jamaican women. For our antenatal attenders, the algorithm had low sensitivities for all RTIs (66.7% for cervicitis, 35.4% for trichomoniasis, 11.1% for bacterial vaginosis (BV) and 24% for candidiasis). Specificities for BV (88.9%) and candidiasis (81.1%) were higher than for cervicitis (62.8%) and trichomoniasis (68.5%). The positive predictive values we

Automated prognosis of renal function decline in ADPKD patients using deep learning.

An accurate prognosis of renal function decline in Autosomal Dominant Polycystic Kidney Disease (ADPKD) is crucial for early intervention. Current biomarkers used are height-adjusted total kidney volume (HtTKV), estimated glomerular filtration rate (eGFR), and patient age. However, manually measuring kidney volume is time-consuming and subject to observer variability. Additionally, incorporating automatically generated features from kidney MRI images, along with conventional biomarkers, can enhance prognostic improvement. To address these issues, we developed two deep-learning algorithms. Firs

Predicting Bradycardia in Preterm Infants Using Point Process Analysis of Heart Rate.

OBJECTIVE: Episodes of bradycardia are common and recur sporadically in preterm infants, posing a threat to the developing brain and other vital organs. We hypothesize that bradycardias are a result of transient temporal destabilization of the cardiac autonomic control system and that fluctuations in the heart rate signal might contain information that precedes bradycardia. We investigate infant heart rate fluctuations with a novel application of point process theory. METHODS: In ten preterm infants, we estimate instantaneous linear measures of the heart rate signal, use these measures to ext

Host lung gene expression patterns predict infectious etiology in a mouse model of pneumonia.

BACKGROUND: Lower respiratory tract infections continue to exact unacceptable worldwide mortality, often because the infecting pathogen cannot be identified. The respiratory epithelia provide protection from pneumonias through organism-specific generation of antimicrobial products, offering potential insight into the identity of infecting pathogens. This study assesses the capacity of the host gene expression response to infection to predict the presence and identity of lower respiratory pathogens without reliance on culture data. METHODS: Mice were inhalationally challenged with S. pneumonia

Long-term survival prediction in older women with stage I-II breast cancer using decision tree-based machine learning.

INTRODUCTION: Predicting survival in older women with early breast cancer can guide personalized care and improve outcomes. Aging is an individualized process that influences tumor characteristics and survival, with cardiovascular disease (CVD) being the leading non-cancer cause of death due to cardiovascular risk factors. This study aimed to develop and validate machine learning (ML) models to predict all-cause, breast cancer-related, and CVD-related mortality in older women with stage I-II, hormone receptor-positive breast cancer at the U.S. population level. To address the heterogeneity ass
