We report 4 episodes of spontaneous bacterial pleuritis observed in 3 patients with liver cirrhosis complicated by ascites and pleural effusion. This infection mimics spontaneous bacterial peritonitis. Three episodes were successfully treated. Proposed pathogenesis, diagnostic methods and therapy are discussed.

Recently, the association between anti-phospholipid antibodies (false positive VDRL, lupus anticoagulant or anti-cardiolipin antibody) and diverse clinical manifestations has been termed antiphospholipid syndrome. We report 6 female patients with "primary" antiphospholipid syndrome, not related to connective tissue disorders. Their age ranged from 23 to 66 years and they were followed from 1 to 27 years (mean 9.2). Five adult patients presenting with clinical and laboratory manifestations of an acute hepatitis in the course of a hitherto undiagnosed infectious mononucleosis (IM) are reviewed. A 51 year old man with acquired immune deficiency syndrome for 2 years developed a chronic leukemia/T cell lymphoma. Anti HTLV-1 antibodies were confirmed by Western Blot. In the last months he developed hypercalcemia and leukocytosis of 130,000. Necropsy confirmed the diagnosis of Leukemia/T cell lymphoma without cutaneous involvement.

We evaluated the role of oxygen free radicals in the induction of acute stress gastric ulcer in rats. After 12 hr of immobility, ulcers of up to 4 mm were observed in the gastric mucosa. Pretreatment with allopurinol, a xanthine oxidase inhibitor, produced a significant reduction in the number and size of lesions (p < 0.0001). Measurement of cortisol (F) and cortisone (E) are important to study cortisol metabolism during gestation or in hypertensive patients. We developed a method for simultaneous measurement of F and E based on plasma extraction by dichloromethane, separation by high resolution chromatography and detection at 254 nm. The IgG antibody response specific to Helicobacter pylori was evaluated through ELISA in a group of 92 gastric patients colonized by this bacteria. 74 had gastritis and 19 gastroduodenal ulcer. We performed 2 dimensional echocardiograms to investigate the presence of mitral valve prolapse (MVP) in 21 patients with Graves disease and active hyperthyroidism. 22 healthy subjects were used as a control group. The incidence of MVP was 24% (n = 5) in hyperthyroid patients compared to 5% (n = 1) in normals (NS by Fisher's exact test). Age, T3 and T4 levels were similar in hyperthyroid patients with and without MVP, but the clinical course was longer in the former. A study in a larger group of patients may help establish the increased incidence of MVP in hyperthyroidism.

Thyroid disfunction in the aged is often misdiagnosed either due to scanty symptoms, masking by other ailments or because function tests can be altered by extrathyroid causes such as chronic diseases, drugs or undernutrition. We surveyed 93 patients from 60 to 104 years old (73 females) living in geriatric homes. Most received at least 2 drugs for control of hypertension, coronary artery disease, diabetes, parkinsonism or psycho-organic deterioration. No clinical evidence of thyroid disfunction was found in 75 patients. T3 was 73.6 +/- 25.5 ng/dl, T4 7.3 +/- 1.8 micrograms/dl, TSH 2.8 +/- 0.9

Approximately 15% of patients with Wolff Parkinson White's syndrome who undergo electrophysiologic study have more than one accessory pathway. We describe the electrophysiologic testing and findings in 2 patients with multiple pathways. We report 2 patients with epidermoid cyst of the testis. Clinical, histological and echographic aspects are discussed. A radical orchyectomy was performed in one patient, a partial one in the other. A successful outcome followed in each case.

A 54 year old female was operated on for ampullary carcinoma diagnosed by retrograde cholangiography. Histologic study of the gallbladder resected at operation revealed another carcinoma at that location. Pancreato-duodenectomy plus a double segmental resection of liver was performed. The patient remains in good condition 11 months after operation.

A 38 year-old woman had an anterolateral wall myocardial infarction and acute left ventricular failure 11 days after a cesarean section. Coronary angiography performed 14 days later because of recurrent angina disclosed dissection of the left main coronary artery. She subsequently underwent aorto coronary bypass surgery uneventfully. Spontaneous coronary artery dissection is a rare entity that mainly affects women in peripartum periods and presents a high mortality rate.

Two patients developed severe hyponatremia attributed to inappropriate antidiuretic hormone secretion during a crisis of acute porphyria, and were treated with intravenous hypertonic sodium solution. One of them developed clinical and radiological findings of pontine and extrapontine myelinolysis, which may constitute a risk in the treatment of this complication.

Spontaneous intramural hematoma of the esophagus (SIHE) is a rare condition usually affecting middle-aged or elderly women. It presents as acute substernal or epigastric pain, typically accompanied by dysphagia or hematemesis. SIHE is not usually associated with vomiting, and is therefore clearly distinguishable from hematogenic esophageal disorders, such as the Mallory Weiss lesion and the Boerhaave syndrome.
