Iron fortification: country level experiences and lessons learned.

Iron fortification has been used to enhance iron intake in many developed countries for more than 50 years, but only in the last decade has this strategy been applied on a large scale to other parts of the world. Iron fortification of rice is being instituted in the Philippines. Initially, the rice will be produced in government-controlled rice mills and sold at low cost mainly to low income families. Efforts to improve the technology (using coating or extrusion techniques) are currently underway to reduce cost and minimize losses during storage and washing. Effectiveness and feasibility studi

Treatment of visceral leishmaniasis: a review of current treatment practices.

Visceral leishmaniasis (VL) is a systemic protozoan infection that infects a million people living in subtropical and tropical areas. Once established, the clinical course of untreated disease leads to death. Recent, large-scale epidemics in east Africa and India and the emergence of a new epidemic in patients infected with HIV makes VL a priority for the World Health Organization. Pentavalent antimonials have been the mainstay of the treatment for > 60years. The progressive appearance of antimonial resistance, the developments of lipid formulations of amphotericin B and a new oral administere

Access to essential drugs in poor countries: a lost battle?

Drugs offer a simple, cost-effective solution to many health problems, provided they are available, affordable, and properly used. However, effective treatment is lacking in poor countries for many diseases, including African trypanosomiasis, Shigella dysentery, leishmaniasis, tuberculosis, and bacterial meningitis. Treatment may be precluded because no effective drug exists, it is too expensive, or it has been withdrawn from the market. Moreover, research and development in tropical diseases have come to a near standstill. This article focuses on the problems of access to quality drugs for th

Gas gangrene in a patient with severe haemophilia A.

The haemophilia patient tends to live a more protected life than his normal counterpart, this is particularly so in underdeveloped and developing countries where due to poor health infrastructure, financial constraints and nonavailability of factor concentrates, patients quickly learn that they need to live a protected life. Under such circumstances, gas gangrene seems to be a very unusual infection for this group of patients. We report here a 25-year-old male with severe haemophilia who developed gas gangrene due to inadequate medical management following a road traffic accident. Subsequently…

Cholera reveals helminthiasis problem.

Cholera has now lasted for about 6 months in our hilly region in Mwanza, Tanzania. The heavy rains coupled with the poor sanitary conditions and sewage disposal system have worsened the situation. The pit latrines used by the majority of low-income people are either not adequately and properly used for excreta disposal or they overflow. At our regional referral and consultancy hospital we were, for a period, receiving cholera cases which should have been managed in isolation at other centers. (This situation has now been rectified.) Some of the children with cholera either vomited or passed As

Transmission of Helicobacter pylori.

The epidemiology of Helicobacter pylori has been the attention of a great deal of study. The age and geographical distribution of infection is rapidly changing with improvements in living conditions, particularly in the childhood period. More than one mode and route of transmission is possible varying with local custom and practices. Infection may be acquired from environmental sources in certain parts of the developing world, but in most circumstances it is probably acquired interpersonally. Interpersonal transmission may be by either fecal-oral or oral-oral routes, depending on circumstances

Japanese encephalitis in India.

Japanese encephalitis (JE), caused by a mosquito-borne virus was first recognised in India in 1955 and since then many major out-breaks from different parts of the country have been reported, predominantly in rural areas. Children are mainly affected, with morbidity rate estimated at 0.30 to 1.5 per 100,000 population. Case fatality rate has ranged from 10% to 60%, and up to 50% of those who recover may be left with neurological deficits. Reported incidence has generally been higher in males than in females, but subclinical infections have occurred equally in both sexes. A large number of subc

Aetiology of severe vitamin A deficiency in children.

BACKGROUND: Severe forms of vitamin A deficiency or keratomalacia are common in young children. Keratomalacia is thought to be associated with malnutrition due to poor weaning practices and manifests at 3 to 4 years of age. As survival rates for infants have increased, keratomalacia is being seen in children less than 6 months of age. Hence, keratomalacia shows two peaks--one in early infancy and the other in the toddler or pre-school age groups. However, the reasons for its occurrence at these ages may be different. METHODS: Records of children admitted to the Nutrition Rehabilitation Centre

Vaccination in pulses: a strategy for global eradication of measles and polio?

Recent American successes against poliomyelitis and measles have been attributed to repeated 'pulse' vaccination campaigns. Whilst logistic and economic constraints will be crucial, a deeper epidemiological understanding of the mechanism, strengths and weaknesses of pulse vaccination will optimize the chances of success elsewhere in the world.
