Internal obturator muscle transposition for treatment of perineal hernia in dogs: 34 cases (1998-2012).

OBJECTIVE: To evaluate the outcome of dogs with perineal hernia treated with transposition of the internal obturator muscle. DESIGN: Retrospective case series. ANIMALS: 34 dogs. PROCEDURES: Medical records of dogs with perineal hernia surgically treated from 1998 to 2012 were reviewed. Diagnostic methods and surgical techniques were recorded. Dogs were assigned preoperative and postoperative clinical sign scores. Complication and recurrence rates were evaluated over time. Risk factors were determined. RESULTS: Median follow-up time was 345 days (range, 22 to 1,423 days). Complications were

How useful are manometric tests of anorectal function in the management of defecation disorders?

OBJECTIVES: The clinical usefulness of assessing anorectal physiology has not been systematically examined. Our aims were to evaluate whethe … ired rectal sensation; 30 (43%) improved after biofeedback therapy. Among 56 patients referred with fecal incontinence, 55 (98%) had manometric abnormalities: 30 (53%) had a low squeeze sphincter pressure, 20 (36%) had impaired rectal sensation, and 28 (50%) had pudendal neuropathy. Thiry-four (60%) patients were referred for biofeedback therapy and 11 (20%) for sur …

Endoscopic Nd:YAG laser therapy for villous adenomas of the colon and rectum.

We reviewed 34 villous tumors of the colon and rectum treated endoscopically with neodymium:YAG laser therapy from 1983 to the present. Twenty-three tumors were benign, and 11 contained carcinoma in situ. Invasive carcinomas were excluded. Treatment locations included cecum (6), descending colon (1), sigmoid colon (2), and rectum (25). Fourteen males and 20 females with a mean age of 70 years (31-93) completed an average of 3.3 total treatments per patient under no sedation (9), intravenous demerol (24), or general anesthesia (1). Treated tumors ranged between 2-12 cm in greatest dimension, an

Functional constipation in children and young adults with Prader-Willi syndrome.

OBJECTIVES: Prader-Willi Syndrome (PWS) is characterized by hyperphagia, endocrinopathies, and gastrointestinal abnormalities. Clinical concerns about constipation and fecal incontinence (FI) are common, but no studies to date have clear data on functional defecation disorders in children with PWS. This study aims to describe the prevalence of functional constipation (FC) in children with PWS seen at Seattle Children's Hospital between 2019 and 2023. METHODS: Parental perception of stooling was collected with a survey designed to correlate with the Rome IV criteria for FC. A retrospective cha…

Case Report: Segmental spinal cord hypoplasia in a Labrador Retriever puppy.

This case report describes the clinical, diagnostic imaging, and histopathologic findings of spinal cord hypoplasia in a 13-week-old, intact male Labrador Retriever. The dog was first presented at 6 weeks of age with chronic and progressive gait abnormalities and fecal and urinary incontinence. On clinical examination, the puppy was bright, alert, and responsive but smaller than its siblings and showed poor muscle mass. No abnormalities were found in the general examination. Neurologic examination revealed mild kyphosis of the lumbar spine, ambulatory spastic paraparesis, proprioceptive pelvic

Effects of Deep Core Activation Method Using Wearable Cyborg HAL in a Supine Position for Ameliorating Defecatory Function: A Pilot Study.

Fecal evacuation disorder and incontinence are prevalent problems among patients with spinal cord injury (SCI) which greatly affect their quality of life. A major cause of these issues is well associated with weakness in their deep core muscular activity. Current programs for amelioration in their deep core muscles are either difficult to apply to patients with SCI or highly invasive. We have previously proposed a minimally invasive method using the wearable cyborg Hybrid Assistive Limb (HAL) for use by patients with SCI. In our proposed method, long-term repetition of supine HAL-assisted hip

No significant differences were found in the reported burden for children or parents. Fecal incontinence was lower in the enema group (33% vs. 47%) but not statistically different. Longitudinally extensive transverse myelitis after pembrolizumab and lenvatinib therapy for a rare subtype of renal cell carcinoma: A case report.

Immune checkpoint inhibitors (ICIs) have revolutionized oncology, shifting the approach from directly targeting tumor cells to enhancing the immune response of the host towards tumor cells by blocking inhibitory receptors or ligands. The present report describes the case of a 26-year-old female patient diagnosed with a rare subtype of renal cell carcinoma (RCC), transcription factor E3-rearranged (t)RCC, who presented with progressing motor weakness of the lower extremities, along with urinary and fecal incontinence, which had begun 3 days prior to admission. A diagnosis of longitudinally exte…

BACKGROUND: Sacral nerve stimulators (SNSs) are a widely accepted, efficacious surgical option to treat patients who have failed conservative management for overactive bladder, nonobstructive urinary retention, fecal incontinence, or pelvic pain. As with all implanted devices, there are associated risks for surgical site and implant infections. There are currently no clear published data or guidelines regarding treating such infections. Examination of the Anorectal Angle and Pubococcygeal Line in Adults with Anal Disorders and Defecation Irregularities Using Magnetic Resonance Imaging.

OBJECTIVE: This study explores the role of pelvic floor (PF) muscles in individuals with anal area diseases and defecation irregularities. MATERIALS AND METHODS: Anorectal angle (ARA), pubococcygeal line (PCL), and the distance between ARA and PCL were measured retrospectively in 392 images from patients over 18 years old who underwent lower abdomen magnetic resonance (MR) defecography (sagittal, T2 sequence). The patients presented with complaints of fecal incontinence, chronic constipation, anal fissures, hemorrhoids, anal abscesses, and anal or perianal fistulas. Measurements were taken du…

Low anterior resection syndrome.

Low anterior resection syndrome (LARS) is the distressful defecatory functional problem after sphincter-saving surgery for rectal cancer. Although the symptoms of fecal urgency, frequency, and incontinence may develop in most of the patients after surgery, there is no definitive treatments for LARS. Multifactorial etiologies and various risk factors have been identified, but the reduction of storage capacity in the rectum is one of the main reasons for LARS. Anal sphincter muscle or nerve damage during rectoanal resection or anastomosis construction, and intersphincteric resection for low-lyin
