Comparison of Two Screening Strategies for Gestational Diabetes (GDM2) Trial: Design and rationale.

Gestational diabetes mellitus (GDM) affects approximately 5 to 7% of pregnancies and is associated with increased risk for fetal overgrowth, cesarean delivery, birth trauma, and pre-eclampsia. GDM is commonly diagnosed in the US using a two-step screening and confirmatory approach, whereas a one-step approach is commonly used outside the US. Recent guidelines have suggested that the one-step approach should be used to diagnose GDM, although concern that this may increase the prevalence of GDM to approximately 18%-as well as the lack of clinical trials-based evidence regarding the difference in

Maternal hemoglobin concentration and hematocrit values may affect fetus development by influencing placental angiogenesis.

OBJECTIVE: Vasculogenesis and angiogenesis are crucial for maintaining proper placental perfusion and optimal fetal development. Among other physical and chemical factors, hypoxia is known to stimulate angiogenic processes. Preplacental type of hypoxia is often associated with maternal anemia and is thought to enhance vascularization within the fetoplacental unit. The goal of this study was to establish the correlation between the local expression of vascular endothelial growth factor (VEGF) and placenta growth factor (PlGF) receptors (flt-1, flk-1) with maternal hemoglobin (Hb) concentration,…

Assessment of fetal growth using the ratio of the transverse cerebellar diameter to abdominal circumference.

OBJECTIVE: To evaluate use of the transverse cerebellar diameter to abdominal circumference (TCD/AC) ratio in predicting intrauterine growth restriction (IUGR). METHODS: A prospective observational study was conducted at a prenatal clinic in Udaipur, India, between August 2011 and December 2012. Women with a singleton pregnancy and a confirmed gestational age of 20-36 weeks were enrolled. Transverse cerebellar diameter and abdominal circumference were measured by ultrasonography on two prenatal visits at early (20-28 weeks) and late (30-36 weeks) gestation. A TCD/AC ratio above the 90th perce

Placental vitamin D receptor expression is decreased in human idiopathic fetal growth restriction.

UNLABELLED: Fetal growth restriction (FGR) affects up to 5 % of pregnancies worldwide, and trophoblast function plays a significant role on the outcome. An epidemiological study has linked vitamin D deficiency to adverse perinatal outcomes, which include decreased birth weight. The placenta as an important source of vitamin D regulates its metabolism through the vitamin D receptor (VDR), but the mechanism by which VDR regulates trophoblast function is poorly understood. Our study aimed at determining placental VDR expression in FGR and gestation-matched control (GMC) pregnancies and identifyin

An ultrasound-based fetal weight reference for twins.

OBJECTIVE: The objective of the study was to construct an ultrasound-based estimated fetal weight-for-gestational-age reference for twin fetuses, stratified by chorionicity. STUDY DESIGN: We performed a retrospective cohort study of live-born nonanomalous twins delivered longer than 34 weeks at the Royal Victoria Hospital (Montreal, Canada). Fetal weight was estimated using ultrasound biometric measurements combined using Hadlock's formula. Multilevel linear regression models were used to adjust for clustering by twin pregnancy and to account for the serial ultrasound measurements taken on ea

Reference charts for fetal biometric parameters in twin pregnancies according to chorionicity.

OBJECTIVE: The objective of this article is to determine reference values for fetal biometric parameters in twin pregnancies and to compare these values between monochorionic and dichorionic pregnancies. METHODS: A retrospective cross-sectional study was conducted among 157 monochorionic and 176 dichorionic twin pregnancies between 14 and 38 weeks of gestation. Biometric measurements included the biparietal diameter (BPD), abdominal circumference (AC), femurs length (FL) and estimated fetal weight (EFW). To evaluate the correlation between biometric parameters and gestational age, polynomial

Fetal abnormal growth associated with substance abuse.

Substance abuse remains a major concern in pregnancy. The current review summarizes the best available literature on the subject. The findings of most studies are confounded by multiple drug use and environmental and social factors that by themselves are known to adversely affect the pregnancy outcomes of interest. Overall, however, substance abuse during pregnancy was associated with negative effects on birth weight and head circumference.

Placental growth factor (PlGF): a key to optimizing fetal growth.

The needs of the uterus and the fetus for the provision of nutrients and oxygen, supplied by the blood flow, are understandably extremely high, with the circulatory system playing the most important role in this action. Abnormal vascular growth and transformation that create a high vessel resistance network have been associated with various pregnancy pathologies, including miscarriage, small for gestational age (SGA) fetuses with or without preeclampsia and intrauterine growth restriction (IUGR). Placental growth factor (PlGF) has a major role in vasculogenesis and angiogenesis in human placen

Progesterone receptor isoform expression in response to in utero growth restriction in the fetal guinea pig brain.

Intra-uterine growth restriction (IUGR) is a significant in utero complication that can have profound effects on brain development including reduced myelination and deficits that can continue into adulthood. Progesterone increases oligodendrocyte proliferation and myelin expression, an action that may depend on the expression of progesterone receptor (PR) isoforms A (PRA) and B (PRB). The objective of this study was to determine the effect of IUGR on PR isoform expression in the brain of male and female fetuses and whether effects were associated with a reduction in myelination. We used a guin

[Developmental origins of health and disease in adults: role of maternal environment].

Many epidemiological studies indicate that environmental conditions during embryonic and fetal development can have an impact on health at adulthood. Animal studies clearly demonstrate that maternal, and even paternal undernutrition or nutritional excess durably modify some epigenetic marks in their offspring, affecting gene expression and physiological adaptations to the environment. It is crucial to better define the effects of early environment on adult phenotype and epigenetic marks in humans and to develop, with the help of animal models, new preventive strategies and treatments.

Iron prophylaxis in pregnancy--general or individual and in which dose?

Iron is mandatory for normal fetal development, including the brain. Iron deficiency may have deleterious effects for intelligence and behavioral development. It is important to prevent iron deficiency in the fetus by preventing iron deficiency in the pregnant woman. Iron deficiency anemia during pregnancy is a risk factor for preterm delivery and low birth weight. In the Western countries there is no consensus on iron prophylaxis to pregnant women. An adequate iron balance during pregnancy implies body iron reserves of >or=500 mg at conception. The physiologic iron requirements in the second …

Circadian rhythm of maternal blood pressure and fetal growth.

This study aimed at examining any relation between the circadian variation in blood pressure (BP) in human pregnancy and fetal growth. A prospective study included 52 pregnant women monitored during the third trimester of pregnancy. There were 33 uncomplicated pregnancies with normal fetal growth (Group 1) and 19 pregnancies complicated by intrauterine growth retardation (IUGR), confirmed at birth (Group 2). Ten women (five in each group) had pregnancy-induced hypertension. All women were hospitalized and followed a similar daily routine. BP was recorded with an automatic wearable device. Meas

Placental transport of nutrients and its implications for fetal growth.

Placental growth during early and mid-pregnancy has a powerful, constraining influence on fetal growth during late pregnancy. Studies involving surgical and environmental reduction of placental size in sheep have shown an associated reduction in capacity to transport oxygen, glucose and amino acids. Oxygen transport is limited by placental blood flow but transport of glucose and amino acids is determined by the abundance and activity of specific transport proteins. Glucose transporters include the GLUT1 and GLUT3 isoforms previously identified in brain and other tissues; systems for active tra

Tracheal occlusion reverses the high impedance to flow in the fetal pulmonary circulation and normalizes its physiological response to oxygen at full term.

PURPOSE: The authors hypothesized that in utero tracheal occlusion would reverse the high impedance to pulmonary blood flow associated with congenital diaphragmatic hernia (CDH) and normalize the fetal physiological response to oxygen at term. METHODS: Three experimental groups were studied. Six fetal lambs (CDH group) underwent creation of a left CDH at 80 days' gestation, an additional six fetal lambs underwent left CDH creation at 80 days' gestation followed by fetal tracheal occlusion performed at 108 days' gestation (CDH + TO group), and four control fetal lambs (control group) underwent

Sonographic diagnosis of intrauterine growth restriction.

Estimation of fetal weight in utero using multiple ultrasonic parameters remains the mainstay in screening for IUGR. The use of various fetal morphometric ratios and/or measurements of other fetal parameters may provide additional useful information. Serial evaluation to assess interval growth may be necessary to clarify the diagnosis. The use of Doppler ultrasound, especially the evaluation of the umbilical artery and middle cerebral artery velocity flow, is an important adjunct for both the diagnosis of IUGR caused by uteroplacental insufficiency and its continued management.

Balloon valvuloplasty during pregnancy.

Although rheumatic diseases are reported to be almost eradicated in the developed countries, they still continue to contribute significantly to maternal mortality in the developing world. Surgical therapies need to be considered for those patients who do not respond satisfactorily to medication. However, valve replacement or valvulotomy during pregnancy carries significant risks for both the mother and the fetus. Two patients with severe mitral stenosis refractory to medical therapy are presented. Both patients were subjected to percutaneous balloon valvuloplasty during pregnancy without any c

Comparative developmental and maternal neurotoxicity following acute gestational exposure to chlorpyrifos in rats.

Chlorpyrifos (CPF), an organophosphorus (OP) insecticide, exerts toxicity through inhibition of acetylcholinesterase (AChE). In the present study, pregnant Sprague-Dawley rats were given CPF (200 mg/kg, sc) as a single dose on gestation d 12 (GD12) and then sacrificed on either GD16, GD20, or postnatal d 3 (PND3) for measurement of maternal and developmental indicators of toxicity. While most CPF-treated rats exhibited no overt signs, a subset (4/28) showed moderate to severe signs of "cholinergic" toxicity at 2-3 d after treatment, and these rats were omitted from further studies.
