Accuracy of fingerbreadth measurements for thyromental distance estimates: a brief report.

There have been many contradicting studies as to how well preoperative airway assessments predict difficult intubation. One of these measures, thyromental distance (TMD), has often been called into question. However, there have been no published studies evaluating whether anesthesia practitioners are measuring TMD accurately, especially if they use fingerbreadths as opposed to a centimeter ruler. A convenience sample of 60 anesthesia providers were asked to participate in a brief study. The subjects were asked a series of questions, including the type of anesthesia provider they were, how ofte

Intrathecal hydromorphone for cesarean delivery: in search of improved postoperative pain management: a case report.

Currently, morphine and fentanyl are the most commonly used intrathecal opioids for the postoperative pain management of patients who underwent cesarean delivery. Unfortunately, the analgesic benefits of these 2 drugs tend to fall into different extremes based on lipid solubility. Intrathecal hydromorphone may provide more consistent analgesia because its lipid solubility falls between that of the other 2 opioids. A 22-year-old woman with a 39-week intrauterine pregnancy, gravida 2, para 1, came in for a scheduled second-time cesarean delivery. Her preoperative history included a morphine alle

Severe preeclampsia, pulmonary edema, and peripartum cardiomyopathy in a primigravida patient.

Peripartum cardiomyopathy (PPCM) is a rare form of heart failure of unknown etiology that is associated with late pregnancy and the early postpartum period. Although the complete pathogenesis of PPCM is not completely understood, the signs and symptoms are identical to those of left ventricular heart failure. The diagnosis of PPCM is made in a parturient only after other causes of heart failure are ruled out. Management of PPCM is similar to that of congestive heart failure with a few exceptions, such as avoiding the use of angiotensin-converting enzyme inhibitors during pregnancy. This report

AANA Journal course: transient neurologic symptoms and spinal anesthesia.

A universal goal of anesthesia providers is to provide the safest, most effective anesthesia and analgesia for their patients. When reports emerge showing problems or complications with an agent or technique that previously was thought safe, recommendations often are adopted in anesthesia departments to avoid or abandon the agent or technique, or alternatives are sought. Hyperbaric 5% lidocaine has been an effective and safe spinal anesthetic agent for short procedures for years. During the past decade, controversy arose over its use because it was implicated as the cause of transient neurolog

Malignant hyperthermia: a case study.

Malignant hyperthermia continues to be a life-threatening emergency that can occur without warning. With early discharge, this crisis may even occur at home. Perioperative, anesthesia, and Post-Anesthesia Care Unit (PACU) nursing staff need to be educated in the signs, symptoms, treatment, and care of a malignant hyperthermia patient. This is a US government work. There are no restrictions on its use.

Substance Misuse and Drug Diversion in Anesthesiology: Impact of a National Educational Intervention for Nurse Anesthesia Residents and Support People.

Substance misuse and drug diversion among anesthesiology professionals may begin during training and span decades of practice. Because treatment for substance use disorder (SUD) is often delayed, early recognition and structured education are essential. This national quality improvement study evaluated the effect on knowledge, perceptions, and the likelihood of reporting suspected substance misuse. A descriptive, comparative pre-post survey was conducted with resident registered nurse anesthetists and support people recruited from 32 randomly selected U.S. nurse anesthesia programs. The interv…

Anesthesia in Pediatric Radiotherapy: A Systematic Literature Review by the SIOP Europe Working Group on Pediatric Anesthesia in Radiation Therapy.

Radiotherapy (RT) is essential in pediatric cancer treatment and often requires complete immobility. In younger or noncompliant children, th … edation was most frequently reported, with favorable safety when delivered by experienced pediatric anesthetists. Complication rates varied widely and were often poorly defined. Additional concerns included long-term neurocognitive impact, vascular access, and procedural burden, especially in resource-limited settings. Evidence supports the safe use of sedatio …

The Legacy of COVID-19: Hospital Fear Led to the Delayed Diagnosis of an Ovarian Tumor with Massive Ascites and Extensive Abdominal Necrosis.

The COVID-19 pandemic has significantly impacted healthcare-seeking behaviors, leading to delayed cancer diagnoses due to hospital-related a … th peritoneal involvement, and a multidisciplinary team (surgeons, gynecologists, plastic surgeons, anesthetists, and intensive care specialists) determined the need for surgical intervention. Histopathology confirmed mucinous adenocarcinoma with pseudomyxoma peritonei (FIGO IIIB). This case underscores the critical impact of delayed oncological diagnosis and …

Controversies about the systematic preoperative pharmacological treatment before pheochromocytoma or paraganglioma surgery.

The question of systematic use of a pharmacological treatment before surgery in patients diagnosed with pheochromocytoma and paraganglioma (PPGL) remains highly controversial. While recent guidelines suggest that this should be used in all patients, some experienced teams consider it unnecessary in some cases, provided the surgery is performed in a dedicated center that has expert endocrinologists, cardiologists, surgeons, and anesthetists. This controversy is aimed at shedding light on the potential benefits and risks of such a treatment, focusing specifically on alpha blockers which are cons…

Data and meta-analysis for choosing sugammadex or neostigmine for routine reversal of rocuronium block in adult patients.

This meta-analysis was conducted to define clinical efficacy and side effects (bradycardia and post-operative nausea and vomiting [PONV]) in … sed using Cochran's Q test and the I2 statistic. Quantification of these outcomes can better inform anesthetists and health systems of the relative costs and benefits of the two reversal agents. This information also forms a basis for a comparative cost analysis in a co-submitted manuscript [1].

Using critical care physicians to deliver anesthesia and boost surgical caseload in austere environments: the Critical Care General Anesthesia Syllabus (CC GAS).

BACKGROUND: Despite an often severe lack of surgeons and surgical equipment, the rate-limiting step in surgical care for the nearly five billion people living in resource-limited areas is frequently the absence of safe anesthesia. During disaster relief and surgical missions, critical care physicians (CCPs), who are already competent in complex airway and ventilator management, can help address the need for skilled anesthetists in these settings. METHODS: We provided a descriptive analysis that CCPs were trained to provide safe general anesthesia, monitored anesthesia care (MAC), and spinal a…

Improving Patient Outcomes Through Closed-Claims Analysis: Salient Characteristics and Patterns Associated With Respiratory Events.

A retrospective, exploratory research design was used to analyze salient characteristics and patterns associated with closed claims involving Certified Registered Nurse Anesthetists (CRNAs) in which a respiratory event caused the adverse outcome. Alleged malpractice acts found in these claims occurred between 2003 and 2012. Respiratory events were the most frequent cause of adverse outcomes in the current database (34%). The respiratory adverse outcomes often resulted in mortality or significant and permanent morbidity (69%) and were largely preventable (81%). Of these respiratory outcomes, in…

Do patients with isolated Pierre Robin Sequence have worse outcomes after cleft palate repair: A systematic review.

Cleft palate repair for Pierre Robin Sequence (PRS) patients has always been a challenge for surgeons and anesthetists. The aim of this systematic review is to investigate the outcome of cleft palate repair for PRS patients compared with cleft palate-only patients. All papers published before October 2014 were searched in the databases PubMed and MEDLINE. Search terms included "Pierre Robin Sequence," "cleft palate repair," and "speech result." Additional studies were identified by hand searching the reference lists of the papers retrieved from electronic search. Two independent reviewers asse…
