Platelets and white blood cell subpopulations among patients with myocardial infarction and unstable angina.

Platelet (PLT)-endothelial cell and PLT-leukocyte interactions at lesion-prone sites might trigger a local inflammatory response early in the genesis of atherosclerosis and contribute to plaque destabilization leading to acute coronary syndromes (ACS). The aim of this study was to assess the PLT count, mean PLT volume (MPV), PLT mass, white blood cell (WBC; including eosinophils) and plasma interleukin (IL)-5, in patients with ACS and controls. PLT count, MPV, PLT mass, WBC and eosinophil percentage were determined in 167 consecutive patients with ACS (86 with acute myocardial infarction, AMI,

Unstable angina and non-ST-segment myocardial infarction: an evidence-based approach to management.

Unstable angina (UA) and non-ST-segment elevation myocardial infarction (NSTEMI) represent two common, closely related acute coronary syndromes with potentially high morbidity and mortality. Integration of information from the history, physical exam, electrocardiogram, and cardiac biomarkers is used to formulate both the diagnosis of UA/NSTEMI and the overall assessment of patient prognosis and risk. Early diagnosis and risk stratification of patients with UA/NSTEMI enable the physician to initiate timely, appropriate treatment. (There is strong clinical evidence supporting the tailoring of sp

Decreased soluble cell adhesion molecules after tirofiban infusion in patients with unstable angina pectoris.

AIM: The inflammatory response, initiated by neutrophil and monocyte adhesion to endothelial cells, is important in the pathogenesis of acute coronary syndromes. Platelets play an important role in inflammatory process by interacting with monocytes and neutrophils. In this study, we investigated the effect of tirofiban on the levels of cell adhesion molecules (soluble intercellular adhesion molecule-1, sICAM-1, and vascular cell adhesion molecule-1, sVCAM-1) in patients with unstable angina pectoris (AP). METHODS: Thirty-five patients with unstable AP (Group I), ten patients with stable AP (Gr

Treatment of unstable angina pectoris/non-ST-segment elevation myocardial infarction in elderly patients.

Elderly patients with unstable angina pectoris/non-ST-segment elevation myocardial infarction should be hospitalized. Precipitating factors should be identified and corrected. Electrocardiogram monitoring is important. Aspirin should be given as soon as possible and continued indefinitely. Clopidogrel should given for up to 9 months in patients in whom an early noninterventional approach is planned or in whom a percutaneous coronary intervention (PCI) is planned. Clopidogrel should be withheld for 5-7 days in patients in whom elective coronary artery bypass graft surgery (CABGS) is planned. A

Stromal cell-derived factor-1alpha in unstable angina: potential antiinflammatory and matrix-stabilizing effects.

BACKGROUND: Chemokines play a pathogenic role in atherogenesis and plaque destabilization by activating and directing leukocytes into the atherosclerotic plaque. However, stromal cell-derived factor (SDF)-1 was recently found to have antiinflammatory effects, and we hypothesized that this chemokine could play a beneficial role in coronary artery disease. METHODS AND RESULTS: Plasma levels of SDF-1alpha were significantly decreased in patients with stable (n=30) and unstable angina (n=30) compared with healthy control subjects (n=20), particularly in those with unstable disease. By flow cytome

[Lectin-induced platelet aggregation inpatients with unstable angina pectoris treated with anti-aggregation agents].

Lectin-induced platelet aggregation was studied in patients with unstable angina pectoris treated with tiklid and aspirin. ADP, PHA-P, WGA, Con A served as the aggregation inductors. In unstable angina pectoris lectin-induced platelet aggregation was enhanced. Changes in this aggregation in response to treatment with antiaggregants with different mechanisms of action are demonstrated.

Dobutamine stress echocardiography and exercise electrocardiography for risk stratification in medically treated unstable angina.

UNLABELLED: Previous reports have demonstrated the superiority of exercise echocardiography over exercise electro-cardiography (ex-ECG) for risk stratification in patients with medically stabilized unstable angina (UA). We sought to analyze the prognostic value of dobutamine stress echocardiography (DSE) compared with ex-ECG for risk stratification in patients with UA. METHODS: Ninety-two patients with medically treated UA were studied (mean age 65 +/- 11 years, 24 women, 42% of patients had electrocardiographic abnormalities on admission). Dobutamine stress echocardiography and treadmill ex-

New therapies for unstable angina and non-Q-wave myocardial infarction: recent clinical trials.

Thrombosis after the rupture of an atherosclerotic plaque often precipitates the acute coronary syndromes of unstable angina and myocardial infarction. The combination of aspirin and heparin has been shown to reduce the occurrence of both symptomatic and asymptomatic ("silent") ischemia, myocardial infarction, and death in patients with these syndromes. However, heparin and aspirin each have significant limitations as antithrombotic drugs. Additionally, coagulation abnormalities may persist for several months after an acute ischemic event, and long-term anticoagulation may be beneficial. Becau

Antithrombin-III plasma activity during and after prolonged use of heparin in unstable angina.

Heparin effectively prevents the complications of unstable angina but disease reactivation has been documented following its discontinuation. To investigate whether this could be related to antithrombin-III depletion, 50 patients with unstable angina had serial determinations of activated partial thromboplastin time and of the plasma levels of heparin, antithrombin-III activity and of the thrombin-antithrombin-III complex before, during and, in a subgroup of 8 patients, 4 hours after heparin discontinuation. Heparin was administered intravenously at therapeutic doses for a mean of 7.6 +/- 4.1

Mechanisms contributing to precipitation of unstable angina and acute myocardial infarction: implications regarding therapy.

Clinical and autopsy studies indicate that most patients who present with unstable angina or an acute myocardial infarction (AMI) have significant underlying coronary atherosclerosis. This review discusses 4 mechanisms that may contribute to the precipitation of these acute ischemic clinical syndromes: progression of atherosclerosis, acute coronary thrombosis, coronary artery spasm, and platelet aggregation. Numerous clinical trials using thrombolytic agents early during AMI have shown the incidence of thrombosis to be at least 60%. Other studies suggest that platelet aggregation frequently co

Ceramide-based machine learning models for the diagnosis of unstable angina: a prospective multicenter study.

BACKGROUND: Coronary artery disease is a critical public health issue. While traditional biomarkers like cardiac troponin show high sensitivity, their utility in unstable angina (UA) patients is limited. Recent research has identified novel biomarkers, including ceramides, which may correlate with atherosclerotic plaque stability and acute coronary syndrome (ACS) risk. This study aimed to develop a machine learning-based model to evaluate ceramide's diagnostic efficacy in patients with UA. METHODS: We conducted a prospective study from April 2021 to August 2022 at two medical centers, compari

OUFR Versus FFR for Functional Assessment of Coronary Artery Stenosis in Patients With Unstable Angina.

BACKGROUND: Hybrid intravascular ultrasound-optical coherence tomography (IVUS-OCT) imaging can integrate both advantages. Optical ultrasonic flow ratio (OUFR) was recently developed for functional assessment. OBJECTIVES: This study aimed to verify the diagnostic performance of OUFR using fractional flow reserve (FFR) as the reference standard. METHODS: From February 2023 to June 2023, 80 patients (126 vessels) with unstable angina pectoris were assessed with FFR and either hybrid IVUS-OCT or stand-alone IVUS imaging before percutaneous coronary intervention in a prospective series. Correlation between hyperuricemia and coronary in-stent restenosis in patients with unstable angina: A retrospective study.

This study investigates the correlation between hyperuricemia and the development of in-stent restenosis (ISR) following percutaneous coronary intervention (PCI) in patients with unstable angina. We analyzed clinical data from 128 patients diagnosed with unstable angina who underwent PCI. Patients were categorized into 2 groups: hyperuricemia group (34 cases) and non-hyperuricemia group (94 cases), based on the presence or absence of hyperuricemia. A multivariate Cox regression model was used to determine the correlation between hyperuricemia and the development of ISR. The proportion of patie

The comparison of IL-17 levels in patients with unstable angina before and after medical treatment.

BACKGROUND: Interleukin (IL)-17 plays a central role in orchestrating the cytokine cascade and accelerates atherosclerosis and plaque vulnerability in animal models. However, epidemiological data evaluating the role of IL-17 levels in unstable angina are lacking. The aim of our study is to evaluate and compare the IL-17 levels in unstable angina (UA) cases before and after treatment. METHODS: This cross-sectional study was performed from July to October 2018 in Ali Iben-abitaleb heart center, Zahedan, Iran. 48 patients with UA in the age range of below 50 years entered the study.
