Conservative Treatment of Ankle Osteoarthritis.

Despite the disabling nature of ankle osteoarthritis (OA), there is poor scientific evidence for a conservative treatment compared to the hip and knee OA. In this regard, most of the treatment options in use are not based on clinical studies of the ankle, and they are extracted from evidence obtained from clinical studies of other lower limb joints. However, this does not seem to be a good idea, since the aetiology of ankle OA is quite different from that of the hip or knee. Nonpharmacological and pharmacological treatments such as nonsteroidal anti-inflammatory drugs, hyaluronic acid, cortico…

[Lower leg, ankle and foot].

CLINICAL/METHODICAL ISSUE: Bipedal locomotion means high mechanical stress on the lower extremities and susceptibility to fractures. On the one hand, these can be the result of a one-time traumatic stress in the context of high speed traumas or falls, but can also result from repetitive microtraumas with subsequent stress fractures. STANDARD RADIOLOGICAL METHODS: For all fracture entities and localizations, X‑ray diagnostics is the basic modality. METHODICAL INNOVATIONS: For optimal surgical reconstruction of intra-articular fractures or in complex fracture configurations, computed tomograph

Total Ankle Replacement in Hemophilia.

INTRODUCTION: Severe ankle hemophilic arthropathy can be a calamitous sign of severe hemophilia with important inferences for activities of daily living. AIMS: To summarize the contemporary, accessible information on Total Ankle Replacement (TAR) for ankle hemophilic arthropathy. METHODS: A search of Cochrane Library and PubMed (MEDLINE) regarding the role of TAR in ankle hemophilic arthropathy. RESULTS: The insufficient information regarding the results of TAR for hemophilic arthropathy is confined to scanty case series and case reports. An evaluation of the accessible literature reveals e…

Anterocentral Portal in Ankle Arthroscopy.

BACKGROUND: The anterocentral portal is not a standard portal in anterior ankle arthroscopy due to its proximity to the anterior neurovascular bundle. However, it provides certain advantages, including a wide field of vision, and portal changes become redundant. The purpose of this study was to evaluate the neurovascular complications after anterior ankle arthroscopy using the anterocentral portal. METHODS: We retrospectively identified patients who had undergone anterior ankle arthroscopy with an anterocentral portal at our institution from 2013 to 2018. Medical record data were reviewed and…

[Medial foot and ankle instability].

The deltoid or medial collateral ligament consisting of superficial and deep components together with the spring ligament is the primary stabilizer of the ankle joint. Injuries of these anatomical structures are more frequent than assumed but are nevertheless often overlooked. Inadequate treatment can lead to chronic pain, instability, hindfoot deformities and ankle arthritis. Patient history and clinical assessment can help to identify injuries of the deltoid ligament. Magnetic resonance imaging (MRI) is the diagnostic method of choice. Arthroscopy of the ankle joint can be a valuable tool in…

Prevention of Lateral Ankle Sprains.

CONTEXT: Given the frequency of ankle sprains, especially in the athletic population, prevention is a primary task of athletic trainers and other sports health care professionals. OBJECTIVE: To discuss the current evidence as it relates to prophylactic programs for the prevention of ankle sprains and to provide critical interpretation of the evidence supporting and refuting the implementation of preventive programs. CONCLUSIONS: External prophylactic supports and preventive exercise programs are effective for reducing the risk of ankle sprains in both uninjured and previously injured populat…

[Ankle sprain in youth athlete].

Lateral ankle sprain is the most frequent musculoskeletal injury in the young athlete. Myths, dogma and common belief are regularly encountered when discussing this injury, for which the scientific literature does not prevail yet. In the youth, the growing skeleton further influences the diagnosis and therapeutic processes, as well as the healing potential. For the athlete, a fast recovery and return to sports without recurrence are a priority. In this specific context, an integrated management of the ankle sprain in the young athlete must be based on an adequate diagnosis, a sound knowledge o…

Ankle Osteoarthritis - Arthroplasty or Arthrodesis?

Osteoarthritis of the talocrural joint accounts for only 4% of cases of degenerative disease of the musculo-skeletal system. Unlike other joints, idiopathic OA of the ankle joint is identified in only 7% of patients. Until the end of the 1960's, arthrodesis was the treatment of choice in advanced OA of ankle joint. Absolute indi-ca-tions for arthrodesis include irreversible loss of joint anatomy, neurological conditions, advanced osteoporosis and chronic inflammation. Currently, the surgical treatment of ankle joint OA relies on third-generation endo-prostheses of the ankle. Arthroplasty is in…

Foot and Ankle Osteoid Osteomas.

Foot and ankle osteoid osteomas (OOs) are often cancellous or subperiosteal and rarely present with a periosteal reaction. Additionally, the large number of disorders included in the differential diagnosis and the nonspecific findings on radiographs complicate the diagnosis. We performed a manual search of the senior surgeon's hospitals' operating room records for the terms "benign bone tumor," "foot," "ankle," and "osteoid osteoma" from January 2003 until December 2014. Of 87 surgically treated patients with lower extremity OOs, 9 patients (11%) with foot or ankle OOs were included. The mean …

Ankle Spanning External Fixator Application.

A 2-stage approach to treatment of tibia pilon fractures was reported in the 1990s and popularized over the ensuing decade to help avoid catastrophic soft-tissue complications experienced with one-stage open reduction and internal fixation. This approach can be expanded to the treatment of other high-energy injuries of the hind foot, with variation in external fixator application as needed. Key factors to maximize the utility and decrease complications of the external fixation stage include the following: (1) anatomic reduction of the talus under the long axis of the tibia in the coronal and s

Repair of Bimalleolar Ankle Fracture.

PURPOSE: The incidence of ankle fractures is rapidly increasing in geriatric populations. Of the 4 fracture patterns described by the Lauge-Hansen classification system, supination-external rotation (SER) accounts for most ankle fractures. This video demonstrates surgical repair of a SER type 4 ankle fracture in a geriatric patient. METHODS: SER type 4 ankle fractures are considered unstable and are generally treated with surgical fixation. After placement of plate and screws, intraoperative stress tests can be used to assess for syndesmotic widening. If necessary, the syndesmosis can be redu…

Ankle fusion in hemophilic patients.

INTRODUCTION: Hemophilic arthropathy is painful and disabling. We report a retrospective study of ankle fusion with intra- and peri-operative clotting factor perfusion. The objective was to assess the efficacy of maintaining perioperative clotting factor rates close to 100%, and report long-term results. The study hypothesis was that results would be good, without early hemorrhagic complications. MATERIAL AND METHOD: Between 2000 and 2013, 12 ankle fusions were performed in 9 patients, with a mean age of 39years (range, 19-58years). Anti-hemophilic factor perfusion was controlled by the refer…

Doctor, I sprained my ankle.

Ankle sprains constitute the majority of ankle injuries, and result in pain, limited mobility/exercise and loss of school/work days. Ankle sprains involve at least one of the ankle ligaments and range from a micro tear to complete tear of the ligament or group of ligaments. The most common mechanism of ankle sprains is inversion stress of a plantar-flexed foot, while the most frequently injured ligament is the anterior talofibular ligament. The attending clinician needs to stratify the risk of fracture through history-taking and physical examination, manage the pain, assess long‑term complicat…

Maisonneuve-hyperplantarflexion variant ankle fracture.

Maisonneuve fractures are rare ankle injuries, accounting for up to 7% of all ankle fractures. They consist of a proximal third fibula fracture, syndesmotic disruption, and medial ankle injury (either a deltoid ligament disruption or a medial malleolus fracture), and are often successfully managed with nonoperative treatment of the proximal fibula fracture and open reduction and internal fixation (ORIF) of the medial ankle injury and syndesmotic disruption. The hyperplantarflexion variant ankle fracture comprises approximately 7% of all ankle fractures and features dual posterior tibial lip fr…

Ankle surgery: focus on arthroscopy.

BACKGROUND: The ankle joint can be affected by several diseases, with clinical presentation varying from mild pain or swelling to inability, becoming in some cases a serious problem in daily life activities. Arthroscopy is a widely performed procedure in orthopedic surgery, due to the low invasivity compared to the more traditional open field surgery. The ankle joint presents anatomical specificities, like small space and tangential view that make arthroscopy more difficult. METHODS: From 2000 more than 600 ankle arthroscopies were performed at our institution. The treated pathologies were mo…

Update on acute ankle sprains.

Ankle sprains are a common problem seen by primary care physicians, especially among teenagers and young adults. Most ankle sprains are inversion injuries to the lateral ankle ligaments, although high sprains representing damage to the tibiofibular syndesmosis are becoming increasingly recognized. Physicians should apply the Ottawa ankle rules to determine whether radiography is needed. According to the Ottawa criteria, radiography is indicated if there is pain in the malleolar or midfoot zone, and either bone tenderness over an area of potential fracture (i.e., lateral malleolus, medial malle…

Guided growth for ankle valgus.

BACKGROUND: Ankle valgus may be insidious and common in a variety of congenital conditions including clubfoot, neuromuscular disorders and others or acquired after fracture, osteotomies, or other manipulations of the lower extremity. This can cause hindfoot pronaton, resulting in lateral impingement and excessive shoe wear. Orthoses do not change the natural history. Medial hemiepiphysiodesis of the tibia is an accepted method of correcting this problem. Difficulties with transmalleolor screw removal prompted us to adopt the tension band method. Our purpose was to outline the technique of usin…

Leg, ankle, and foot injuries.

The emergency provider (EP) must be aware of the anatomy of the leg, ankle, and foot. The varied presentation of common injuries must be recognized as well as the unique presentations of uncommon injuries. The astute EP must rely on a focused history and a precise examination to avoid the pitfalls and missed injuries from an over-reliance on radiographic studies. In some cases, emergent orthopedic consultation is required. Potential complications associated with these injuries must be anticipated and avoided if possible.
