Symptomatic anal anomalies in chromosome 22q11 deletion syndrome: a report of three patients.

Deletion of chromosome 22q11 is a common genetic condition with varying clinical presentation ranging from neonatal fatality to patients whose presentation to medical services will be prompted after a few years by speech delay or mild developmental concerns. While most published data relating to patients with 22q11 deletions has focused on the "classical" presentation of the condition with cardiac manifestations, hypocalcaemia and velopharyngeal insufficiency, a much wider range of clinical presentations can characterise this syndrome. Anal anomalies, comprising imperforate anus and symptomati

Long-term management of chronic constipation in children.

A retrospective clinical study was conducted to determine the success of a strict regimen employing the prolonged use of daily enemas in 203 children with chronic constipation. This study confirmed that the use of long-term daily enemas did eliminate constipation as well as the primary complaint of fecal soiling. The majority of children treated had an excellent to good result (85.8%) over an extended period of time. Patients with a past medical history of imperforate anus or Hirschsprung's disease required longer treatment periods (32.6 and 20.1 months, respectively) than children with other

Urorectal septum malformation sequence. Report of six cases and embryological analysis.

We encountered six female infants with a specific pattern of developmental abnormalities of the urogenital and lower intestinal tracts. The anomalies included ambiguous genitalia, lack of perineal openings, and müllerian and urinary tract anomalies. Each patient had normal female chromosomes and normal adrenal gland function. We believe that this combination of anomalies represents a recognizable and specific sequence that is due to a failure of migration to and/or fusion of the urorectal septum with the cloacal membrane. This, in turn, we postulate, leads to persistence of the cloaca and cloa

The history of colostomy in childhood.

The idea of performing a colostomy in a child was first brought up by Littré in 1710. It lasted, however, 50 years until the first successful operation was published. Especially in the 19th century various ways to approach the colon have been practiced with frequently disappointing results (lumbar, inguinal approach). Only at the end of the century, colostomy was used as a guide for a pull-through operation. With improved surgical and anaesthesiological techniques, colostomy has gained more importance as a temporary measure in order to postpone definitive surgery. Various new techniques have b

Multidisciplinary management of a neonate with oropharyngeal teratoma and bilateral cleft lip and palate: a rare congenital presentation.

This case report describes a rare presentation of a male infant with a large congenital oropharyngeal teratoma, bilateral cleft lip and palate, and a low-type imperforate anus. The infant experienced airway obstruction at birth, necessitating immediate neonatal intensive care unit admission and surgical intervention. Imaging revealed craniofacial teratoma involvement, contributing to palatal fusion failure during embryonic development. Two major surgeries were performed: teratoma excision and anoplasty. Postoperatively, the infant faced respiratory distress, Klebsiella sepsis, and a urinary tr

Pallister-Hall syndrome with orofacial narrowing and tethered cord: a case report.

BACKGROUND: Pallister-Hall syndrome is a rare, autosomal dominant, genetic disorder characterized by different congenital abnormalities: hypothalamic hamartoblastoma, bifid or shortened epiglottis, polydactyly, renal anomalies, and imperforate anus. CASE PRESENTATION: In this case report, we describe the case of a 13-year-old Lebanese-Armenian boy born with Pallister-Hall syndrome showing newly associated manifestations (orofacial narrowing and tethered cord), and currently showing a spontaneous puberty with normal growth pattern following management with growth hormones. CONCLUSIONS: This c

Discordant Anomalies with Combined Features of Pentalogy of Cantrell and OEIS Complex: A Case Report in Monochorionic Twins.

INTRODUCTION: Ventral body wall defects have various manifestations. Among others, pentalogy of Cantrell (PC) and omphalocele exstrophy imperforate anus spinal abnormalities (OEIS) complex are defects that involve upper and lower anterior midline of body wall, respectively. Although both entities are in a spectrum of ventral body wall defects, the combination of PC and OEIS complex has not been described. CASE REPORT: In this report, we describe an unusual case of congenital ventral body wall defect with combined features of PC and OEIS complex, which discordantly occurred in monochorionic mo

Colostomy--the Port Harcourt experience.

Over the 7 years period from January 1986 to December 1992, 36 colostomies were performed at the University of Port Harcourt Teaching Hospital (UPTH). The indications for the procedure were classified under congenital, traumatic, neoplastic and miscellaneous groups. Imperforate anus and Hirschsprungs disease, under the congenital group, formed the commonest indication (19 cases); traumatic conditions followed with 9 cases, carcinoma of the rectum comprised 5 cases while there were 3 cases in the miscellaneous group--made up of 2 sigmoid volvulus and one of stercoral perforation of the sigmoid

Characteristics and treatment of congenital perineal groove in male patients.

BACKGROUND: Congenital perineal groove (CPG) of male patients has rarely been reported before. The purpose of this study was to review our cases and describe their characteristics and treatment. METHODS: Four male patients diagnosed with CPG were included in this study. Medical records were retrospectively reviewed. Type of CPG and anal position index (API) of the patients were recorded. Follow-up was through outpatient visits. RESULTS: Their age ranged from 4 years and 2 months to 10 years and 9 months. Among the four patients, two complained of intermittent CPG mucosal hemorrhage and the o

Augmented-pressure distal colostogram: the most important diagnostic tool for planning definitive surgical repair of anorectal malformations in boys.

There is little current literature on the augmented-pressure distal colostogram, the single most important diagnostic study performed in boys with imperforate anus prior to definitive repair. Accurate understanding of the anatomy of the anorectal malformation including an associated fistulous communication between the rectum and the urogenital tract is essential for optimal surgical management. Specifically, the position of the rectal pouch and recto-urinary fistula relative to posterior sagittal structures of the perineum, especially the sacral spine, dictates the operative approach. VATER association: is it recognised by rheumatologists?

The authors describe an adult patient with history of chronic low back pain and recurrent prostatitis, marked limitation of lumbar spine motion and a radiograph demonstrating fused lumbar vertebrae, which suggest a diagnosis of spondylarthropathy. However, the absence of radiographic evidence of sacroilitis, the nature of the vertebral defects and a history of imperforate anus pointed towards the diagnosis of VATER association, rather than a spondylarthropathy. Although most patients with VATER association are diagnosed during infancy, the musculoskeletal anomalies can be overlooked while the

A case of a closed partial cloacal septation defect with a patent urachus.

A boy with a closed partial cloacal septation defect with a patent urachus is reported. He had an intact abdominal wall, a patent urachus, a colovesical fistula, intact genitalia and urethra, imperforate anus, and a lipomyelocystocoele. Patients with similar constellation of findings have been reported as cloacal exstrophy variants. What distinguishes this case from the other reported variants is the intact abdominal wall with the patent urachus, the small and normally formed phallus and urethra, and the presence of a lipomyelocystocoele. We discuss the possible embryologic mechanism responsib

Urethral abnormalities in male neonates with VATER association.

In the past 3 years, eight of 20 male neonates with stigmata of the VATER association were found to have significant urethral abnormalities. Three had megalourethra, two had duplication of the urethra, and one each had an anterior urethral valve, congenital stricture, and hypospadias. In only five of eight was the extent of the abnormality clinically apparent. All three infants with megalourethra and imperforate anus died because of the coexistence of other severe congenital anomalies. Prior literature has described but not stressed the increased prevalence of urethral abnormalities in childre

X-linked skeletal dysplasia with mental retardation.

A syndrome compatible with an X-linked trait is described, affecting four male cousins in three sibships. The body had skeletal anomalies, including short stature, ridging of the metopic suture, fusion of cervical vertebrae, thoracic hemivertebrae, scoliosis, sarcral hypoplasia and short middle phalanges. In addition, they had moderate developmental retardation, and abducens palsies. Three of the four had glucose intolerance, and one was born with an imperforate anus. Of five female obligate carriers studied, three had fusion of cervical vertebrae, three had some shortening of the middle phala

Intestinal obstructional outcome in paediatric age group: A general surgeon's prospective.

Intestinal obstruction is one of the most common surgical emergencies seen in children. Therefore, it of interest to evaluate the pattern of disease occurrence, clinical presentation and treatment outcomes in children with intestinal obstruction. Infants (31.3%) are highly affected by intestinal obstruction than other age groups. Boy child exhibits number cases of intestinal obstruction than girl child. Imperforated anus (52.2%) is the most common congenital types of lesions causing the disease; Intussusception (69.5%) is the commoner acquired lesion and abdominal distention (32.8%) is the typ

A Precious Puzzle: Unveiling a Suspected VACTERL (Vertebral, Anal, Cardiovascular Malformations, Tracheo-esophageal Fistula, Renal Anomalies, and Limb Defects) Association in a 28-Day-Old Neonate.

VACTERL is an acronym that stands for a wide range of congenital anomalies, including vertebral anomalies, tracheoesophageal fistula, anorectal anomalies (anal atresia), cardiac anomalies, renal anomalies, and limb malformations. While not a true syndrome, the co-occurrence of these findings suggests a common underlying cause. Here, we present a case of a 28-day-old neonate with suspected VACTERL association. The baby was delivered preterm to non-consanguineous parents. The baby had vertebral abnormalities, renal defects, an imperforate anus, a posterior urethral valve, and an absent right pul

Multicentric paraspinal neuroglial heterotopia with Müllerian and renal agenesis: a variant of Mayer-Rokitansky-Küster-Hauser syndrome?

UNLABELLED: Neuroglial heterotopia is a rare congenital anomaly that mostly involves the head and neck region. We report a female fetus with multicentric paraspinal neuroglial heterotopia in the retropharyngeal and retroperitoneal spaces, right renal agenesis, left renal hypoplasia, and Müllerian agenesis. Additional findings included bilateral preaxial polydactyly of the hands, megacystis, rectovesical fistula, and imperforate anus. The karyotype was 46, XX. This fetus had the features of Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome with paraspinal neuroglial heterotopia. This is the first

Two cases of a fetus with sirenomelia sequence.

We report two cases of a fetus with sirenomelia sequence which showed oligohydramnios and a single umbilical artery. The first case was of a single fetus with symelia apus and only one leg. Prenatal diagnosis of this case was possible. The second case was of a dichorionic-diamniotic twin pregnancy in which one fetus had symelia dipus with two fused lower extremities. Prenatal diagnosis of the condition was not made. In both cases, the fetuses died shortly after birth from respiratory distress due to severe pulmonary hypoplasia. Absence of urinary tract, imperforate anus, and spine deformity we
