A high water content lens (Scanlens 75, +14.5 dioptres) and a low water content lens (Bausch and Lomb Soflens H3, +14.5 dioptres) were compared with respect to corneal thickness response after 6 hours of wear in a provocative test on 12 aphakic eyes. The mean increase in central corneal thickness was 3.3% for Scanlens and 6.4% for Soflens. This difference is statistically highly significant (p less than 0.001). This survey describes ultrasound measurement techniques and echograms of living phakic and cataract eyes. Results of clinical echometry on emmetropic and ametropic eyes are reported. In unilateral or bilateral cataract and in unilateral aphakia, appropriate ultrasound measurements are of fundamental importance for restoring compatible vision; in unilateral aphakia, by means of combinations of contact lenses and spectacles; in cataract, by means of precalculated artificial intraocular lenses.

Vitreous fluorophotometry was used to evaluate the integrity of the blood-retinal barrier in 130 eyes with long-standing aphakia and apparently normal fundi. Monocularly aphakic guinea pigs (Cavia porcellis), prepared by removing the lens by phacoemulsification, were maintained under near-UV lighting conditions for several months. Epinephrine maculopathy is a reversible macular change that occurs in aphakic eyes being treated with topical epinephrine compounds. The first symptom is transient blurred vision, which can be followed by decreased visual acuity, flame-shaped hemorrhages, and cystoid macular edema. Onset of symptoms can range from a few days after the commencement of epinephrine therapy to several months thereafter. Epinephrine maculopathy should be differentiated from Irvine-Gass syndrome. A consecutive series of pseudophakic retinal detachments was statistically compared with a matched group of aphakic retinal detachments. Distribution of breaks, anatomic results, and final visual acuities were tabulated. Success in reattachment was equal in the two groups, but final visual acuities were better in the aphakic group. Emphasis was placed on the intraoperative ophthalmoscopic examination of pseudophakic eyes with compromised peripheral fundus visibility.

In patients treated with oral indomethacin for four weeks after intracapsular cataract extraction, five of 57 eyes (9%) had cystoid macular edema four to six weeks postoperatively as shown by fluorescein angiography. In a control group not treated with indomethacin, 15 of 34 eyes (44%) had cystoid mascular edema. Oral indomethacin, therefore, caused a statistically significant decrease in the incidence of aphakic cystoid macular edema.

After hesitation in the indication for the implantation of irisclip-lenses, experience shows that indications which have not yet been planned must be seriously considered: for instance cataract in patients with retinitis pigmentosa. The already highly restricted visual field is not additionally reduced by the wearing of the strong convex glasses. Furthermore the author reports about refinements of the implantation technique.

In a series of cataract patients excluding myopic individuals, under age 60 years, and cases in which vitreous loss occurred, retinal detachment was no less frequent after intracapsular cataract extraction and Sputnik iris supported lenses than in controls. Both groups were followed up for a minimum of two years. The detachments predominantly occurred from retinal breaks in areas of the retina that looked normal preoperatively.

A series of 75 eyes underwent pars plana vitrectomy for progressive diabetic traction retinal detachment of the macula. In all cases, the media was sufficiently clear to allow preoperative macular examination. The overall visual improvement was higher in phakic eyes (80%) than aphakic eyes (72%) and the rate of neovascular glaucoma was less in phakic eyes. Five aphakic patients and five phakic patients were fitted with high plus Bausch & Lomb Soflenses. We evaluated corneal edema, staining, vertical striae, and corneal thickness before and after a 5-hr wearing period. The high plus lenses produced average corneal-thickness changes of 9.6% and 7.1% for the phakic and aphakic groups, respectively.
