[Anatomy of the superficial parts of the face and neck].

The author presents four chapters concerning classical anatomy, an anatomical contribution to new facelifts, aesthetic anatomy, which recall … ny bony insertion. It is composed exclusively of platysma and risorius muscles. There is no parotid aponeurosis. The parotid is surrounded by a capsule. Its is limited in depth by the superficial cervical aponeurosis which lines the styloid curtain (Jost, Levet).(ABSTRACT TRUNCATED AT 400 WORDS)

Interpositional Arthroplasty by Temporalis Fascia Flap and Galea Aponeurotica Combined With Distraction Osteogenesis: a Modified Method in Treatment of Adult Patients With Temporomandibular Joint Ankylosis and Mandibular Dysplasia.

BACKGROUND: Interpositional arthroplasty (IPA) with temporalis fascia flap has been one of the most frequently performed procedures to treat temporomandibular joint (TMJ) ankylosis. However, recurrence often occurs when the flap lacks bulk or atrophies. Whether to perform IPA or distraction osteogenesis (DO) first has long been a controversial issue when patients presented mandibular dysplasia (MD). This study provided IPA a new graft material sufficient to prevent recurrence, combined the modified protocol of performing DO 6 months after IPA, and evaluated its efficacy in treating TMJ ankylos

Levator resection with suspensory ligament of the superior fornix suspension for correction of pediatric congenital ptosis with poor levator function.

PurposeTo evaluate the surgical outcome of levator resection with suspensory ligament of the superior fornix (SLSF) suspension in severe congenital ptosis with poor levator function (LF).Patients and methodsThe medical records of 25 patients who underwent levator resection with SLSF suspension between March 2011 and January 2013 were retrospectively reviewed. All of the patients had severe congenital ptosis (>4 mm) and poor LF (<4 mm). The follow-up time ranged from 12 to 18 months (median, 15 months). Data regarding eyelid position, cosmetic outcomes, and postoperative complications were eval

A clinical, ultrastructural and immunochemical study of Dupuytren's disease.

Aponeurotic tissue from seven normal subjects and from apparently unaffected branches, nodules and cords of 16 Dupuytren's patients were com … ts were present. There was no relation between clinical stage and structural alterations; the whole aponeurosis always seemed to be involved; cord retraction would seem to depend on the interactions among fibroblast-like cells and matrix components and among matrix macromolecules themselves.

Case Report: Laparoscopic repair of indirect inguinal hernia combined with ipsilateral giant subcutaneous inguinoperineal lipoma.

Lipomas are common benign mesenchymal neoplasms with slow growth, consisting of mature adipocytes, while giant inguinal lipomas are rare because of anatomical limitations. This report describes a 46-year-old woman who presented with a reducible left inguinal mass and an ipsilateral painless giant subcutaneous mass. She was diagnosed with a left indirect inguinal hernia and a separate inguino-labial lipoma superficial to the external oblique aponeurosis, which was not adherent to the inguinal canal, round ligament or canal of Nuck. The coexistence of the two lesions was incidental. The patient …

Palmar and plantar fibromatosis: a review.

Palmar fibromatosis (Dupuytren disease/contracture) is the most common type of fibromatosis, defined as a benign proliferation of fibroblasts and myofibroblasts. The disease process is most common in white, middle-aged and older men occurring at the distal palmar crease leading to nodules and contracture, which in many cases recur after surgical treatment. In a similar process, plantar fibromatosis (Ledderhose disease) is a proliferation of fibroblasts and myofibroblasts on the plantar aponeurosis of mostly middle-aged patients that may lead to painful nodules but usually does not lead to cont…

Laparoscopic Repair of Diastasis Recti: A Case Report and Literature Review.

Diastasis recti is a state with separated aponeurosis between two recti caused by weakening of the intercrossing fibers in the linea alba and it causes abdominal protrusion. Common causes comprised of increased intraabdominal pressure, or congenital weakening of myoaponeurotic layer. We describe a patient who underwent laparoscopic repair of diastasis recti. A 30-year-old woman was referred to our outpatient department for an abdominal mass that had appeared 1 year earlier. Physical examination revealed an abdominal wall defect along the midline and computed tomography showed thinning and stre…

Application of the pinwheel flap for closure of a large defect of the scalp.

A 74-year-old man had a resultant large oval scalp defect of 12×6 cm (72 cm2) following an excision of a stage IIIA melanoma. We decided to … a length of 1.5 times the half diameter of the defect (4.5 cm). The incision was made through galea aponeurosis and subsequently undermined at least 3 cm to assist flap advancement. Z-plasties increased the rotation of the flap when it was required. This yielded an excellent reconstructive result at 1-year follow-up, demonstrating the usefulness of the pinwhe …

[Dupuytren’s disease – the story of an eponym].

December 5, 1831, baron Guillaume Dupuytren presents at the surgical clinics of the Hotel-Dieu of Paris a report upon "an entirely new subje … nsverse incision. "Probably many surgeons (Cline 1808, Astley Cooper 1822) were aware of the palmar aponeurosis producing this condition before Dupuytren's lecture". "Thoroughness of his demonstration, the brilliance of the presentation and the supremacy of his surgical authority combined to link permanently the name of Dupuytren with this hand deformity" (J. …

[Genetic aspect of Dupuytren's diseases].

Dupuytren's disease is a connective tissue disorder leading to the shortening of the palmar aponeurosis and progressive digital flexion deformity. Despite decades of both scientific and clinical investigations the precise etiology and tissue origin of Dupuytren's disease remains unclear. Several authors focused their studies on the genetics of Dupuytren's disease and postulated either autosomal dominant or autosomal recessive mode of inheritance. The limited number of members of families affected with the disease makes it difficult to determine the mode of inheritance. A particular difficulty …

[Urinary stress incontinence in women: comparison of treatments].

A technique of surgical treatment of urinary stress incontinence (USI) in women is proposed. It consists of three stages. Stage 1 (intervaginal)--anterior colporaphy with plastic reconstruction of urogenital diaphragmatic muscles. Stage 2 (an original procedure)--fixation of the neck of the urinary bladder to musculus rectus abdominis and aponeurosis by means of three caprone ligatures by dissection of the anterior abdominal wall to the peritoneum. Stage 3--posterior colporaphy, perineolevatoroplasty. Stage 3 is conducted if the patient has marked rectocele simultaneously or 6 months after the…

[An attempt to objectify the assessment of extension or recurrence of Dupuytren's disease on the basis of ultrasound].

Dupuytren's contracture is a disease of unknown etiology, leading with time to decreased hand function caused by increasing contracture of fingers. Excision of contracted longitudinal palmar fascia constitutes the most important way of treatment. Due to fact that disease has progressive course in many patients recurrence (i.e. overgrowth of longitudinal palmar aponeurosis apart from the operated site) or extension (i.e. similar process affecting formerly operated site) occurs. An attempt to objectify the assessment of recurrence or extension of Dupuytren's disease on the basis of sonographic e…

The Effects of Collagenase Clostridium histolyticum on Plantar Fibromatosis: A Case Study.

Plantar fibromatosis, also known as Ledderhose's disease, is a rare disorder of benign fibroblast proliferation involving the plantar aponeurosis (i.e., plantar fascia). Traditionally, surgical intervention has been the most common treatment for plantar fibromatosis. However, numerous studies have reported high recurrence rates of plantar fibromatosis after surgical intervention, as well as wound healing difficulties and nerve injury. Plantar fibromatosis often coexists with other superficial fibrous diseases such as Dupuytren's contracture and Peyronie's disease; immunohistochemical and ultra…

Various soft tissue defect reconstructions using anterolateral thigh and vascularized fascia lata composite free flap.

The anterolateral thigh flap (ALT) is versatile for soft-tissue reconstruction of various body defects because of its thick and vascularized fascia component. We present our clinical experience with the functional one-stage reconstruction of complicated soft-tissue defects using ALTs with vascularized fascia lata (FL). Between April 2018 and February 2022, we transferred ALTs with FL components for various soft-tissue defects in 15 patients. The FL component was used for reconstruction of hand & forearm tendon, medial and lateral patellar synovial membrane, plantar aponeurosis, abdominal wall,…

Spigelian hernia: current approaches to surgical treatment-a review.

BACKGROUND: Spigelian hernias (SpH) belong to the group of eponymous abdominal wall hernias. Major reasons for diagnostic difficulties are its low incidence reaching maximum 2% of abdominal wall hernias, a specific anatomical localization with intact external oblique aponeurosis covering the hernia sac and non-constant clinical presentation. METHODS: A literature review was completed to summarize current knowledge on surgical treatment options and results. RESULTS: SpH presents a high incarceration risk and therefore should be operated upon even if the patient is asymptomatic. Both laparosco…

Nerve conduction velocity and cross-sectional area in ulnar neuropathy at the elbow.

INTRODUCTION: In the precise localization of ulnar neuropathy at the elbow (UNE) we have noted discrepancies between electrodiagnostic (EDx) … SA and lowest MNCV were noted in the same short segment. In 54 patients with UNE at the humeroulnar aponeurosis (HUA), the highest CSA and lowest MNCV were noted proximal to the HUA. DISCUSSION: MNCV and CSA were highly correlated in UNE. Ulnar nerve slowing proximal to the entrapment at the HUA was surprising, but consistent with previous studies done on ca …

[Concomitance of Ledderhose's disease with Dupuytren's contracture. Own experience].

INTRODUCTION: Ledderhose's disease is a rare fibromatosis affecting plantar aponeurosis. Clinical picture and risk factors resemble the ones found in much more common cases of Dupuytren's disease. MATERIAL AND METHODS: The group of 101 patients with Dupuytren's disease qualified for surgical treatment constituted material for the study. Within that group incidence of Ledderhose's disease was assessed on the basis of clinical and ultrasound examination. Estimation of the presence of risk factors common for both forms of fibromatosis was also evaluated. RESULTS: Ledderhose's disease was found …

Apraxia of eyelid opening: clinical features and therapy.

PURPOSE: Botulinum toxin injection is the treatment of choice in cases of benign essential blepharospasm. However, about 10% of the patients do not get sufficient effect from this treatment, and many of them have concomitant apraxia of lid opening. METHODS: Over a 3-year period we treated 12 patients. Three had pure apraxia of lid opening and in the other nine it was associated with blepharospasm. All patients were initially treated with botulinum toxin injections with poor results. They underwent surgical treatment like blepharoplasty, limited myectomy, aponeurosis repair, and/or frontalis s…
