Examining Mid-Upper Arm Circumference Malnutrition z-Score Thresholds.

BACKGROUND: Anthropometric z-scores used commonly for diagnosis and determining degree of malnutrition, specifically body mass index (BMIz), weight-for-length (WLz), and mid-upper arm circumference (MUACz), are not wholly concordant, yet the proposed thresholds for classification are identical. This study was designed to critically examine MUACz thresholds and their ability to correctly classify nutrition status. METHODS: This was a 2-year, prospective single-center study of children ≤18 years seen by registered dietitians within a large pediatric institution. The sensitivity, specificity, an…

Cubitus Varus-It's More Than Just a Crooked Arm!

Although cubitus varus has been regarded as a purely cosmetic problem in the pediatric population, symptomatic elbow instability, and ulnar neuropathy from the mechanical axis malalignment have been reported in adults. This overview discusses the biomechanical axis disruption that leads to soft tissue and morphologic bony alterations in the elbow and offers a compelling argument for corrective osteotomy to treat pediatric cubitus varus.

Assessment of Lumbar Spine Instability Using C-Arm Fluoroscopy.

A 47-year-old woman was referred to physical therapy with a diagnosis of lumbar radiculopathy. Weight-bearing flexion/extension radiographs showed no change in a 13-mm (at L5-S1) spondylolisthesis measured with a neutral posture. Physical therapy with a focus on flexion-biased stabilization exercises was initiated. After failing to improve after 6 weeks, her referring physician ordered magnetic resonance imaging, which revealed a 6-mm spondylolisthesis in a supine position. Additionally, the physical therapist performed an anterior stability test of L5 on S1 under C-arm fluoroscopy, which demo…

Whole arm deletions of 18p: medical and developmental effects.

Deletions of the short arm of chromosome 18 have been well-described in case reports. However, the utility of these descriptions in clinical practice is limited by varied and imprecise breakpoints. As we work to establish genotype-phenotype correlations for 18p-, it is critical to have accurate and complete clinical descriptions of individuals with differing breakpoints. In addition, the developmental profile of 18p- has not been well-delineated. We undertook a thorough review of the medical histories of 31 individuals with 18p- and a breakpoint in the centromeric region. We collected developm…

Removal of infratemporal fossa foreign body under C-arm.

A 12-year-old boy presented to our emergency department after being shot in the face. A computed tomographic scan revealed a bullet through the posterolateral wall of the maxillary sinus into the right infratemporal area, just adjacent to the skull base. We elected transantral approach with the help of endoscopy and C-arm. The bullet was successfully removed. Little is known on the best strategy for removing the infratemporal foreign body. Our experience in this case provides a safe and effective way for such injury.

Prosthetic axillary-axillary arm loop arteriovenous graft for hemodialysis.

AIM: In this study we present our experience with an alternative technique of vascular access for hemodialysis in which a prosthetic graft was placed as subcutaneous arm loop and anastomosed to the axillary artery and vein. METHODS: Patients included in this study were those with end stage renal disease (ESRD) requiring hemodialysis access who did not have suitable vein to perform autogenous AVF in both upper limbs and Duplex examination revealed unsuitable brachial artery to perform prosthetic brachial-axillary access. RESULTS: Forty-one patients with ESRD underwent axillary-axillary arm lo…

Mid-arm and epitrochlear lymphadenopathy: a clinico-radiological surprise.

An 18-year-old man presented with multiple subcutaneous nodules over the anteromedial aspect of his right lower arm. Based on findings of sonography, histopathology, circulating filarial antigen test, and therapeutic response to diethylcarbazine, a diagnosis of filarial lymphadenopathy affecting mid-arm nodes and epitrochlear lymph nodes was made.

Pick to place trajectories in human arm training environment.

This paper presents a new method of trajectory planning in rehabilitation robotics. First were measured in healthy subject the pick to place trajectories while haptic robot is in zero impedance space. B-spline approximation is used to mathematically define the measured paths. This trajectory path serves as a central line for the rounding haptic tunnel. In addition to radial elastic and damping force an optional guidance force can be applied along the tunnel to reach the place point. The B-spline control points were observed around the robot and arm workspace. The trajectory path defined with B…

Ischemic steal syndrome following arm arteriovenous fistula for hemodialysis.

Arteriovenous fistulae in the arm are commonly used for hemodialysis in end-stage renal disease. Although physiological steal with reverse flow in the artery distal to the fistula is common, hand ischemia or infarction are rare. The ischemic steal syndrome (hand or forearm ischemia) is usually a result of arterial disease proximal or distal to the fistula and/or poor collateral supply to the hand. The diagnosis is primarily clinical; however, markedly reduced digital pressures and pulse volume recordings support the diagnosis. Management requires imaging for focal stenoses or disease in arteri…

A novel rapid and effective donor arm disinfection method.

BACKGROUND: The aim of the study was to derive a donor arm disinfection technique that was rapid, but with a disinfection efficacy equivalent to a previous "best-practice" technique. This method consisted of a two-stage procedure with an initial application of 70% isopropyl alcohol and then 2% tincture of iodine (IATI). The total time for the IATI method was 2 minutes in duration. A rapid technique (1 min in duration) was needed to obviate potential problems due to increased donor waiting time, had the IATI method been implemented at blood donation sessions. STUDY DESIGN AND METHODS: A direct…

Reverse-loop upper arm arteriovenous graft for chronic hemodialysis.

Options for an upper arm arteriovenous graft (AVG) commonly include the placement of a straight prosthetic graft connecting the brachial artery to the axillary vein. However, such configuration leads to underutilization of the upper arm veins, resulting in the loss of venous capital in the upper arm for future secondary fistula creation. In this retrospective analysis, we evaluated seven patients who had upper-arm AVGs created in a reverse-loop configuration. The prosthetic graft was created by connecting the brachial artery close to the cubital fossa and tunneled subcutaneously in a looped fa…

Objective quantification of arm rigidity in MPTP-treated primates.

Rigidity is a cardinal symptom of Parkinson's disease and is frequently used as an outcome measure in clinical and non-human primate studies examining the effects of medication or surgical intervention. A limitation of current rigidity assessment methods is that they are inherently subjective. To better understand the physiological mechanisms of rigidity and how various therapeutic approaches work, a more objective and quantitative method is needed. In this study, an automated arm rigidity testing (ART) system was developed to objectively quantify rigidity while the primate's limb was moved be…

Compartment Syndrome of the Arm After Cable-Wakeboard Accident.

A compartment syndrome is an increased tissue pressure within a closed osteofascial compartment. This compromises blood flow to the muscles and nerves within that compartment, which -if not treated adequately in an early stage-results in permanent tissue and nerve damage. It most frequently occurs in the lower leg, but can also occur elsewhere when muscles are enclosed in tight fascial compartments, such as the forearm and hand. In this report a patient is described who developed an acute compartment syndrome of the arm after a cable-wakeboard accident in which his arm was strangulated. Cable-…
