Sacubitril/valsartan attenuates atrial structural remodelling in atrial fibrillation patients.

AIMS: Radiofrequency catheter ablation (RFCA) is now an established therapeutic option for patients with atrial fibrillation (AF), but the long-term recurrence rate of AF is still high. Sacubitril/valsartan (Sac/Val) is superior to valsartan in attenuating ventricular remodelling and improving clinical outcomes in heart failure patients, but whether this additional benefit exists in reversing atrial remodelling and reducing AF recurrence of RFCA-treated AF patients remains uncovered. METHODS AND RESULTS: Patients that had undergone RFCA were enrolled and randomly assigned 1:1 to valsartan (16…

Iatrogenic atrial septal defect after HotBalloon ablation of atrial fibrillation.

HotBalloon-based pulmonary vein isolation (HBPVI) has yielded encouraging clinical results in the treatment of atrial fibrillation (AF). Balloon ablation requires a larger sheath, which raises the concern for a persistent iatrogenic atrial septal defect (iASD). The present study aimed to investigate the incidence, clinical features, and the predictive factors of transthoracic echocardiography (TTE)-detectable iASD after HBPVI. All patients who underwent HBPVI of AF with pre- and post-ablation TTE were retrospectively analyzed. A 17-French steerable deflectable guiding sheath was inserted into …

Emergency Department Re-Presentation for Atrial Fibrillation and Atrial Flutter.

BACKGROUND: Identification and appropriate management of patients with atrial fibrillation (AF) is critical to mitigate the consequences of the disease. We sought to assess the frequency and pattern of the emergency department (ED) use by patients who presented with AF and/or atrial flutter (AFL) in a midsized Canadian hospital. METHODS: We conducted a retrospective cohort analysis of patients who presented to the ED with AF and/or AFL during the calendar years 2010-2012. Patients were identified using the MUSE (General Electric Healthcare, Bucks, United Kingdom) electrocardiogram database an…

Catheter Ablation Targeting Complex Fractionated Atrial Electrogram in Atrial Fibrillation.

The relatively low success rates seen with pulmonary vein ablation in non-paroxysmal atrial fibrillation (AF) patients as compared to those with the paroxysmal form of the arrhythmia have prompted electrophysiologists to search for newer ablative strategies. A decade has passed since the initial description of complex fractionated atrial electrogram (CFAE) ablation aimed at targeting the electrophysiological substrate in atrial fibrillation. Despite intensive research, superiority of CFAE-based ablation over other contemporary approaches could not be demonstrated. Nevertheless, the technique h…

Dual independent atrial tachycardias after ablation of chronic atrial fibrillation.

We report the case of a 71-year-old man with two atrial tachycardias evolving simultaneously and independently in two dissociated regions after extensive ablation for chronic atrial fibrillation. One tachycardia was a focal tachycardia originating from the right inferior pulmonary vein and activating the posterior left atrium with a 2:1 conduction block, while the other tachycardia was an atrial flutter circulating around the tricuspid annulus, activating the right atrium and the anterior wall of the left atrium. These two atrial tachycardias were successfully ablated prior to restoration of s…

Atrial fibrillation recurrence after drug-induced typical atrial flutter ablation.

BACKGROUND: Catheter ablation of typical atrial flutter (AFL) occurring in patients who take antiarrhythmic drugs for atrial fibrillation (AF) has been proposed as a curative approach for AF. The aim of this study was to evaluate the efficacy of this technique. METHODS: Forty-six consecutive patients (30 males, 16 females, mean age 67 +/- 9 years) with paroxysmal or persistent AF were submitted to right atrial isthmus ablation: 1) 33 patients (group 1) in whom typical AFL spontaneously occurred during oral treatment with propafenone (n = 19), flecainide (n = 9) or amiodarone (n = 6); 2) 13 pa…

P wave separation after atrial compartment operation for atrial fibrillation.

Surgically induced abnormalities in atrial conduction could result in unusual P wave changes. A 31-year-old woman underwent concomitant mitral valve surgery and atrial compartment operation for mitral stenosis and atrial fibrillation (AF). After operation, the AF was successfully converted to sinus rhythm, whereas an unusual electrocardiogram (ECG) with a discrete negative deflection before the T wave in V1 was noted. Electrophysiological study showed a marked conduction delay from the high right atrium (HRA) to the right atrial appendage (RAA) compartment, which resulted in a separation of P …

Right atrial mapping and linear ablation for paroxysmal atrial fibrillation.

OBJECTIVE: Catheter ablation techniques to cure atrial fibrillation (AF) are under investigation. This study evaluates a mapping-based, individualized approach to right atrial (RA) linear ablation in patients with paroxysmal AF. METHODS: In this prospective observational study, 29 patients with recurrent symptomatic AF refractory to medical therapy, underwent linear ablation between May 1998 and December 1999. Inclusion criteria were symptomatic paroxysmal AF, failure of at least 2 antiarrhythmic medications, and informed consent. Radiofrequency ablation was performed in the RA using a 3.3 Fr…

Atrial late potentials: paroxysmal supraventricular tachycardia versus paroxysmal atrial fibrillation.

The atrial signal averaged electrocardiogram has been used to detect patients at risk for paroxysmal atrial fibrillation but not yet for paroxysmal supraventricular tachycardia. The P-wave-triggered signal-averaged electrocardiogram, during sinus rhythm, was obtained from 97 subjects divided in groups as follows: 30 controls (Group C), 38 patients with documented paroxysmal atrial fibrillation (Group A) and 29 with documented paroxysmal supraventricular tachycardia (Group B). The atrial duration, root mean square of last 20 and 30 ms and the P-QRS segment were measured. Atrial late potentials …

Relation between echocardiographically determined left atrial size and atrial fibrillation.

In an attempt to define quantitatively the relation between left atrial size and atrial fibrillation, echocardiography was used to study 85 patients with isolated mitral valve disease, 50 patients with isolated aortic valve disease, and 130 patients with asymmetric septal hypertrophy. In all three groups of patients, atrial fibrillation was rare when left atrial dimension was below 44 mm (3 of 117 or 3%) but common when this dimension exceeded 40 mm (80 of 148 or 54%). In addition, when left atrial dimension exceeds 45 mm, cardioversion, while initially successful, is unlikely to produce sinus…

Integrated Management of Persistent Atrial Fibrillation.

The global incidence of atrial fibrillation is on the rise. Atrial fibrillation, a complex disease, heightens the likelihood of heart failure, stroke, and mortality, necessitating careful attention. Controlling heart rate and rhythm, addressing risk factors, and preventing strokes are fundamental in treating atrial fibrillation. Catheter ablation stands out as the primary approach for atrial fibrillation rhythm control. Nevertheless, the limited success rates pose a significant challenge to catheter ablation, particularly for persistent atrial fibrillation. Various adjunctive ablation techniqu…

Echocardiography guided management of atrial fibrillation.

BACKGROUND: The growing burden of atrial fibrillation (AF) experienced by the general population translates into an increased incidence in the intensive care setting, further aggravated by illness severity. New onset AF has been established as an independent mortality predictor. Cardiology management guidelines are based on major trials that included ambulatory patients with varying degrees of ventricular systolic and diastolic dysfunction, and with variable dependences of left ventricular filling on atrial systole. Emphasis is placed on rate control combined with anticoagulation therapy, alon…

Atrial fibrillation in Retinal Artery Occlusions.

Retinal artery occlusions, encompassing central retinal artery occlusion and branch retinal artery occlusion, are acute vascular events that can result in significant and often irreversible vision loss. These ocular emergencies are closely associated with systemic vascular risk factors, including hypertension, diabetes mellitus, advanced age, and cardiovascular comorbidities. Atrial fibrillation (AFib), the most common sustained cardiac arrhythmia, has emerged as a notable contributor to the risk of retinal artery occlusions. The prevalence of AFib is rising, particularly among older adults, p…

Electrographic flow mapping of atrial fibrillation.

PURPOSE OF REVIEW: A low ceiling of efficacy exists for the treatment of persistent atrial fibrillation via pulmonary vein isolation without adjunctive ablations, which is likely because they do not target an individual patient's specific underlying disease mechanisms. Electrographic flow (EGF) mapping is the first system that reliably displays wavefront propagation through the atria. It is a promising tool for localizing sources of atrial fibrillation, guiding targeted ablation, and visualizing conduction through the atrial substrate. RECENT FINDINGS: We describe EGF mapping with emphasis on…
