Long-Term Outcomes and Predictors of Stepwise Atropine Concentration Adjustment in Paediatric Myopia Management.

PURPOSE: To evaluate long-term real-world treatment responses and baseline factors in children who received low-dose atropine for myopia control and required stepwise concentration adjustments. METHODS: This retrospective study included 651 of 1032 children (651 right eyes) aged 5-12 years at initiation, treated with daily atropine for at least 5 years (mean follow-up, 6.1 years). All began with 0.025% atropine, with escalation to 0.05%, 0.05% plus 0.025% in the late afternoon and finally to 0.125% at bedtime plus 0.025% in the late afternoon if the spherical equivalent (SE) showed a myopic s…

Severe acute organophosphate poisoning managed with 2-month prolonged atropine therapy: a case report.

INTRODUCTION AND IMPORTANCE: Organophosphate (OP) poisoning is a common and potentially fatal condition that requires prompt and aggressive treatment with atropine, oximes, and supportive care. We report a rare case of OP poisoning that needed high doses of atropine and intensive care for 60 days. CASE PRESENTATION: A 39-year-old male ingested 200 ml of chlorpyrifos, an OP compound, and presented with vomiting and epigastric pain. He received an initial dose of atropine of 60 ml (36 mg, 1 ml=0.6 mg), followed by an infusion of 16 ml/h (9.6 mg/h). He developed hypoxia, cardiac arrest, delirium…

Coronary artery spasm due to intravenous atropine infusion in a child: possible Kounis syndrome.

Coronary vasospasm can result from silent myocardial ischaemia to sudden death. There are many precipitant factors including different pharmacological agents. Kounis syndrome is defined by acute coronary syndromes associated with anaphylactic or anaphylactoid reactions. We report, to the best of our knowledge, the first paediatric case of Kounis syndrome due to intravenous atropine.

Central anticholinergic syndrome vs. idiosyncratic reaction triggered by a small IV dose of atropine.

A 58-year-old male was scheduled to undergo radical gastrectomy for cancer under general anesthesia. The patient developed agitation and irregular breathing after receiving a single dose of atropine (0.5 mg) to treat bradycardia immediately prior to induction of anesthesia. Within 5 min after the atropine injection, the patient became unresponsive with facial flushing and diaphoresis. When a drop in oxygen saturation was observed, a laryngeal mask airway was inserted after administering a small bolus dose of propofol (80 mg) and the patient was ventilated with 100% oxygen. Physostigmine was no…

Predictive factors of negative outcome in initial atropine therapy for infantile hypertrophic pyloric stenosis.

BACKGROUND: The predictive factors of negative outcome in initial i.v. atropine (IA) therapy of infantile hypertrophic pyloric stenosis (IHPS) are unknown. Conservative therapy for IHPS is useful for infants because it does not have the risk of surgical and anesthetic stress, but some cases of atropine therapy result in failure. This study clarified the predictive markers of negative outcome in initial atropine therapy for IHPS. METHODS: Seventy-six patients with IHPS admitted from 1998 to 2011 were included in this study. The predictive risk factors of negative outcome in initial atropine th…

Anesthesia of captive African wild dogs (Lycaon pictus) using a medetomidine-ketamine-atropine combination.

Seven captive male African wild dogs (Lycaon pictus) weighing 25-32 kg each, were anesthetized by i.m. injection via hand syringe with a combination of 1.5 mg/kg ketamine, 40 microg/kg medetomidine, and 0.05 mg/kg atropine. Following endotracheal intubation, each animal was connected to a bain closed-circuit system that delivered 1.5% isoflurane and 2 L/min oxygen. Atipamezole (0.1 mg/kg i.v.; 0.1 mg/kg i.m.) was given at the end of each procedure (60 min following injection of medetomidine/ketamine/atropine). Time to sternal recumbency was 5-8 min. Times to standing after atipamezole administ…

Use of atropine in patients with submaximal heart rate during exercise myocardial perfusion SPECT.

BACKGROUND: Failure to reach 85% of maximal predicted heart rate (MPHR) during exercise may render a myocardial perfusion single photon emission computed tomography (MPS) study nondiagnostic for ischemia detection. Although commonly used to increase heart rate (HR) during dobutamine stress, the administration of atropine for patients failing to achieve 85% of MPHR during exercise performed for MPS is still infrequent. METHODS AND RESULTS: Patients undergoing dual-isotope MPS were considered candidates for the study when, during exercise treadmill testing, they had less than 85% of MPHR and we…

Atropine does not inhibit late asthmatic responses induced by toluene-diisocyanate in sensitized subjects.

To determine whether early and/or late asthmatic responses induced by toluene diisocyanate (TDI) are caused by a reflex mechanism involving stimulation of muscarinic receptors, we studied the effect of the muscarinic antagonist atropine on early and late airway response induced by TDI. A preliminary study was conducted in asthmatics to assess whether the selected dose of atropine provided adequate muscarinic blockade. On different days we measured the provocation dose (in milligrams) of carbachol causing a 15% decrease in FEV1 (PD15FEV1) without and with atropine premedication (0.008 or 0.012 …

Atropine-sensitive gastric excitation by stimulation of thoracic dorsal roots--antidromic activation of afferents?

In chloralosed cats, usually with ligated adrenals and paralysed with gallamine, the thoracic dorsal roots were electrically stimulated and gastric motor responses were recorded. Thoracic dorsal root stimulation regularly elicited gastric contractions after pretreatment with guanethidine and/or hexamethonium which were readily blocked by atropine. It was established that these gastric motor responses were due to peripheral stimulation of the dorsal root and not to spread to the ventral roots. Together with other results (Delbro & Lisander 1980) the findings suggest an antidromic activation of …

T wave abnormalities in top-ranking athletes: effects of isoproterenol, atropine, and physical exercise.

Eight cases of top-ranking athletes with "repolarization disorders" are reported. All subjects were asymptomatic and were otherwise suited for excellent cardiovascular performances. Seven athletes did not show any evidence of heart disease. Seven had MVP (mitral valve prolapse). Umprompted variability of ECG tracings was observed in three cases. Both isoproterenol infusions (IS) and maximal physical effort (EX) normalized T wave (abnormalities in 100% of cases, while atropine (AT) was ineffective despite an increase in heart rate greater than that caused by IS. The authors emphasize the useful…

"Paradoxical" prolongation of sinus nodal recovery time after atropine in the sick sinus syndrome.

A case of symptomatic sick sinus syndrome is presented with confirmation of sinus nodal dysfunction established by functional testing. The validity of such provocative testing and the criteria for abnormality are discussed. A newly recognized, seemingly "paradoxical" and potentially detrimental effect of atropine noted in this patient is examined. Despite an increase in sinus rate and an improvement in sinoatrial conduction time after administration of atropine, a markedly prolonged sinus recovery time after rapid atrial pacing occurred, and atrial quiescence for more than 10 seconds was seen.…
